Product Description
This manual provides the information and materials needed to conduct an eight-session patient education programme for people with Parkinson’s disease and their carers, complementing medical treatment. This programme was developed within an interdisciplinary European consortium, comprising research and clinical centres in Germany, Spain, Finland, Italy, The Netherlands, Estonia and the United Kingdom.

In addition to dealing with the motor symptoms of Parkinson’s disease, many people also struggle with the psychological and social effects. In fact, people at every stage of the disease can be faced with problems such as depression, anxiety, stressful social interactions, and difficulties communicating, all of which can disrupt their lives. This programme draws upon basic psychological principles and presents specific strategies that people can use to manage these difficulties. The ultimate goal of the programme is to empower people with Parkinson’s disease and their carers to improve their own quality of life. Although the programme is standardised, flexibility is built into the programme to facilitate its use in different cultures, and with different types of patient and carer groups.

Patient Education for People with Parkinson’s Disease and Their Carers: A Manaual is essential reading for all health care professionals and trained volunteers working with people with Parkinson’s disease and their carers. ... Read more

Product Description
Audiences: A professional reference for clinicians evaluating and treating patients with Parkinson's disease including physicians, physical therapists, occupational therapists, speech therapists, nurses, and personal trainers.

Parkinson's disease, the most prevalent hypokinetic movement disorder, is expected to occur with increasing frequency among aging populations worldwide. While we await the discovery of a cure, combining physical interventions with traditional medical interventions can significantly improve overall quality of life. Health Professionals' Guide to Physical Management of Parkinson's Disease fills a void in this area of clinical management by comprehensively addressing the physical management of Parkinson's disease in a single volume.

Written for clinicians, Health Professionals' Guide to Physical Management of Parkinson's Disease will prove to be a useful resource for physical therapists as well as other health care professionals who manage patients with Parkinson's disease. The text expertly distills and blends diverse research-based sources with the author's own extensive clinical experience. The result is a concise manual that provides not just the science but also the clinical application necessary in the selection of physical interventions.

The text begins with an introduction to Parkinson's disease and the umbrella of care required in the management of patients, giving readers a clear explanation of medicinal, surgical, assisted, and self-management strategies depending on the stage of disease. After laying this groundwork, the text continues by focusing on two types of dyskinetic movements: dystonia (which can influence posture) and tremor. Also reviewed is the use of supportive devices and adaptive equipment to minimize the effects of dystonia and tremor. The discussion moves on to balance deficits (Hoehn and Yahr stage 3), transfer problems, and the neurologic and musculoskeletal impairments that contribute to them. The text concludes with a qualitative and quantitative assessment of gait and includes specific treatment guidelines addressing characteristic gait problems.

Health Professionals' Guide to Physical Management of Parkinson's Disease is an easy-to-navigate reference for clinicians who do not have the time or the need to read a book cover to cover, but want answers fast. The chapters are written as independent units extensively cross-referenced, and they can be read in any order. Throughout the text, readers will find clinical decision trees, flow charts, and sample forms to guide their selection of intervention strategies. The book contains many user-friendly features:

  • Numerous illustrations demonstrate exercise considerations, evaluative techniques, and guidance on implementing interventions.
  • The 11 reproducible handouts address safety and compensatory strategies to improve mobility in patients.
  • Access to a variety of patient help features--including case examples specific to chapter topics and instructions for handling problems--engage readers and put answers within fingertip reach.
With new research showing exercise to be beneficial in delaying the onset of the physical impairments caused by Parkinson's disease, this is a timely resource that will make clinical practices more productive, improve patient outcomes, and advance the treatment of patients with Parkinson's disease. ... 5-0 out of 5 stars A valuable, practical, efficient resource for health care staff teams
"Health Professionals' Guide to Physical Management of Parkinson's Disease" is a compact, well organized clinicians' resource that aids in finding effective strategies for physical management of common symptoms of Parkinson's disease. The chapters are carefully researched, written as cross-referenced independent units, organized by presenting problem areas. A broad but concise introduction begins with a summary of medical, surgical assisted and self-management treatment strategies that are used effectively across seven specified stages (modified Hoehn and Yahr, p. 4) of disease progression. Two types of dyskinesias (or involuntary movement disorders) are presented for treatment focus: dystonia and tremor. Further development of balancing deficits treatment strategies, transfer problems, and qualitative and quantitative gait assessment are included, with a frequent goal of improving ambulation abilities. Blalck and white photographs, illustrations and specific case examples help document clearly the approaches suggested. "Health Professionals' Guide to Physical Management of Parkinson's Disease" is a valuable, practical, efficient resource for health care staff teams who work with the PD population. Its usefulness in homecare settings also demands further exploration, with good clinical supervision.

Product Description
Treatment approaches for Parkinson's Disease are determined by several factors: accurate diagnosis, severity of symptoms and functional impairment, adequacy of clinical response to pharmacological agents and other treatments. Often, the decision to initiate treatment, change the dosage of an existing agent or add another agent to a treatment regimen depends on careful monitoring of the timing and extent of clinical response. Optimal management of Parkinson's Disease is achieved through thorough assessment of many considerations affecting each patient, and incorporation of current evidence-based treatment protocols accordingly.

The Parkinson's Disease treatment paradigm is currently undergoing changes as new agents and formulations emerge. This improved paradigm promises to provide physicians and patients with more effective symptom control, and perhaps, delayed disease progression.Filled with relevant clinical insights and guidance, this pocket volume will provide easily accessible information to help physicians integrate the updated treatment guidelines into clinical practice.

This pocket guide is the first portable, concise and up-to-date resource of its kind addressing the complexities of treatment decisions for the various symptoms and challenges in PD.The volume provides practical clinical guidelines on assessment (staging) and treatment of early and advanced PD, as well as addresses associated complications. In addition to diagnosis and assessment, the handbook covers pharmacological treatments, including therapies for neuroprotection and functional impairment, dosage and administration guidelines, surgical procedures, non-pharmacological management, and management of both motor and non-motor complications. ... Product Description
It was nearly four weeks after his 45th birthday when Bill Schmalfeldt was diagnosed with Parkinson's disease. In addition to having to get used to having what he always thought of as a old man's disease, Bill had to adjust to the idea that his condition would only deteriorate. After seven years, Bill learned about an experimental clinical trial of deep brain stimulation for patients in the less-advanced stages of Parkinson's. He was one of 30 people to sign up for this Phase I trial testing the safety and tolerability of the procedure. In this group, 15 patients were randomized to the surgical group while 15 were asked to just continue taking their medicine as before so they could be compared to the surgical group in the months and years to come. The author's proceeds from the sale of this book and/or download will be donated to the National Parkinson Foundation to assist in their marvelous work towards finding a cure and new and better treatments for PD. ... 5-0 out of 5 stars He Puts His Money Where His Mouth Is
Diagnosed with Parkinson's disease at age 45, Bill Schmalfeldt refused to go quietly. Instead, he volunteered for experimental Deep Brain Stimulation surgery.

"I make a living telling people about the benefits of clinical research," writes Schmalfeldt, a former Navy corpsman, now a media guy, author, and blogger. "What kind of a hypocrite would I be if I didn't take the opportunity to put my money where my mouth is?"

Still, being a DBS clinical trial volunteer takes moxie. Neither Schmalfeldt nor his doctors could predict how things would turn out. He doesn't flinch from blunt appraisals of living with an incurable, degenerative disease, and deploys gallows humor for those special moments: the drilling-holes-in-the-head part; how his deep brain sounds via implanted electrodes, in real time; how even the best hospital does inscrutable things to scrambled eggs; and how to deal with able-bodied slackers in the handicapped seats.

You've heard of "being a partner in your own care"? Schmalfeldt's the poster guy for that, and an advocate for other "Parky" people. In choosing to be a clinical trial volunteer, then persisting in writing about it and its aftermath, he's rejected the role of passive patient. Whether he's ribbing the staff, loving his wife, or querying God (without a whiff of proselytizing, thank you), this is a hero's story. Best of luck, Bill!

5-0 out of 5 stars Honest, funny and informative
I worked with Bill some years ago when he hosted audio programs about health topics for a federal agency. I learned a lot from him, and was curious to read about his experiences with Parkinson's Disease, particularly his participation in a clinical trial of deep brain surgery. Bill tells his story both with brutal honesty and with humor, without ever getting maudlin. He explains why he chose to enter the study and details his progress. In the end, though, this is more than a book about the course of a disease. You get a window into the daily life of a remarkable man and a warm, loving couple that can sustain a fighting spirit and sheer joy in being together, whatever the challenges they face.

5-0 out of 5 stars Engaging - author brings his story to help others in their journey.

I am ordering this book today. After reading this first chapter of this book to review it...I was surprised by the way Bill could add humor and emotion to a difficult disease. His writing is engaging to the point I want to read more. PD is a disease that presents itself in many ways to many people. We need to educate ourselves in order to contribute anything to our own life as well as others suffering from PD. Thank you, Bill. Excellent job! Can't wait to get my book.

5-0 out of 5 stars Come into the OR with Bill
"Deep Brain Diary: My Life as a Guy with Parkinson's Disease and Brain Surgery Volunteer" is Bill Schmalfeldt's personal story about living with a neurological disease that afflicts over a million Americans.Written in the style of a diary, Schmalfeldt talks about how he discovered he had the disease at age 45, why he decided to participate in an experimental clinical trial that involved brain surgery, and his recovery and life afterwards. He takes you into the operating room with him, remaining awake for the seven- hour surgery to implant probes deep into his brain to alleviate some of his symptoms. With a wry and sardonic sense of humor and writing style, Schmalfeldt weaves an easy-to-read tale of his personal struggle with the disease, pulling no punches over his frustration over the mixed results of his surgery.

Now, "Deep Brain Diary" is available not only in hardcover, but as a paperback, a PDF download, an Amazon "Kindle" book, and soon, as an iPad book offering.

"This book is written not only for the Parkinson's disease patient," Schmalfeldt said, "but for anyone who knows, cares for, or loves someone who has this beast of a disease.I just want folks who are touched by PD to realize that they are not alone. If they've been embarrassed by drooling in public, I've been there. If they've been humiliated by staring onlookers during an episode of dyskinesia (uncontrollable movements caused by medication), I've been there. If they've found themselves frozen in place while walking, I've been there.Parkinson's affects each patient differently. But the one thing I want people to take away from this book is that Parkinson's disease is NOT a death sentence. It's a LIFE sentence."

Schmalfeldt said that the book was also meant to highlight the importance of clinical trials in the search for cures, as well as new and better treatments for diseases. In 2007, Schmalfeldt volunteered for a clinical trial at Vanderbilt University Medical Center in Nashville to test the safety and tolerabilty of deep brain stimulation in early PD.

"In my clinical trial, we started out with 30 people.Fifteen of us were randomized to the surgical group, 15 were randomized to the control group, meaning they would continue to take their regular medication and then be compared at various intervals with those of us who had the surgery to see if our rates of progression differed," he said."The theory this clinical trial is trying to prove is that doing deep brain stimulation (DBS) surgery earlier in the course of PD will be beneficial to patients, and may even slow down or reverse the progression of the disease."

Schmalfeldt noted that the study is currently seeking the funds to expand to medical centers around the country, enrolling hundreds of new Parkinson's patients.Schmalfeldt said 100 percent of the author proceeds from sale of his book will be donated to the National Parkinson Foundation as well as the Vanderbilt University Medical Center's DBS Research Fund.

"Deep Brain Diary" is available as a 249-page hardback at Lulu.com as well as from online booksellers such as Amazon.com, Barnes & Noble.com and Booksamillion.com for $35.The paperback version is available at Amazon, Barnes & Noble and Books-A-Million for $15.A PDF download is available for $5 at Lulu.An Amazon "Kindle" version is available for $5.The book is currently being formatted for Apple's iPad book store as well.

"Deep Brain Diary" is Schmalfeldt's first try at non-fiction.His previous works, "...by the people...", "Undercover Trucker: How I Saved America by Truckin' Towels for the Taliban," and "Hunky Dunk," are available at Amazon, Barnes & Noble and Books-A-Million.

5-0 out of 5 stars Navigating a Complex Illness
Parkinson's Disease patients might ironically refer to their ailment as "the gift that keeps on giving." They live in a constant struggle to navigate the complexities of a chronic condition that presents an ever-changing landscape of physical and mental symptoms, treatment side-effects and societal perceptions that can sometimes feel like a game of "Whack-a-Mole" for patients, caregivers and clinicians. Now four experts in the treatment of Parkinson's have collaborated on a comprehensive road map for health professionals, "Parkinson's Disease - A Guide to Patient Care."

This book, with an emphasis on substance over style, is constructed as a systematic reference manual designed to help medical professionals maintain an optimal balance among all the various opposing forces faced by their Parkinson's patients. The format is essentially an organized set of lists, detailing characteristics of the disease itself, its impact on the daily lives of people who live with it, and the overlapping array of technical and practical interventions available to the medical professionals who confront it. Each chapter is comprised of concise, readily-referenced bullet points of information essential to the development and evolution of complex treatment plans required for effective management of this frustratingly complex illness. Redundancies abound, since each element is interconnected, and each issue encountered must be weighed and considered against the particular pattern of symptoms presented by the patient, each as individual as they are.

There is an obvious tendency for this type of "technical manual" to become cold and clinical, but threading through this volume is a distinct undercurrent of compassion and encouragement for Parkinson's patients and those close to them. It is this fundamental thoughtfulness and humility that distinguish the book, challenging health professionals to use their mole-whacking abilities to give their patients one more gift--that of hope.
