Product Description
There has been a growing sociological interest in both risk and its management, and with how we cope with the uncertainties of late 20th- century life. Understanding accidents is the key to understanding the risk society, for accidents are both the paradigmatic challenge for risk technologies to predict the apparently unpredictable and the ultimate indicator of the limits of those technologies.; The book traces the emergence of the "accident" as a marginal misfortune in modernist thought, and its transformation into a "preventable" misfortune in the late 20th century. It identifies accidents as being crucial to self- conscious, rational modernity in the early 20th century: accidents which "just happen" marked the limits of rational explanation and only the irrational denied their existence. However, as an examination of Contemporary Medical And Health Promotion Writing Shows, The Accident should no longer happen. The rise of risk assessment constructs the accident as an essentially preventable misfortune.; This book should be of interest to students and sociologists working in the areas of social theory, risk, health and illness, and knowledge. It should also be of interest to those concerned about cultural change in the late 20th century, and the implications of risk assessment technologies. ... Read more

Product Description
A ground-breaking new book, Beyond Human Error: Taxonomies and Safety Science deconstructs the conventional concept of "human error" and provides a whole new way of looking at accidents and how they might be prevented. Based on research carried out in the rail, nuclear, and defense industries, the authors show how, by concentrating solely on "human error," systems and sociological factors are frequently ignored in contemporary safety science. They also argue that the "information processing" view of human cognition, the foundation of the majority of safety science and ergonomics, is hopelessly simplistic and leads to ineffective or even misguided intervention strategies. Wallace and Ross explore how what they call the "technically rational" view of science can hamper the process of creating a taxonomy of error events, and the implications this has for the current orthodoxy. In laying out the limitations of the "technically rational" viewpoint, they clearly define their own alternative approach. They begin by demonstrating that the creation of reliable taxonomies is crucial and provide examples of how they created such taxonomies in the nuclear and rail industries. They go on to offer a critique of conventional "frequentist" statistics and provide coherent, easy to use alternatives. They conclude by re-analyzing infamous disasters such as the Space Shuttle Challenger accident to demonstrate how the "standard" view of these events ignores social and distributed factors. The book concludes with a stimulating and provocative description of the implications of this new approach for safety science, and the social sciences as a whole. While providing a clear and intelligible introduction to the theory of human error and contemporary thinking in safety science, Wallace and Ross mount a challenge to the old orthodoxy and provide a practical alternative paradigm. ... 5-0 out of 5 stars Insight on human and system failures
This is a quality book, easy to read considering the deep philosophical and wide conceptual ground it covers. If you have enjoyed Reason, Perrow or Dekker discussing human error then I think you will find considerable value in this book. Excellent arguments about ecological validity, root cause, Heinrich's legacy, taxonomies, distributed cognition, normal accidents, psychology of risk, and on and on. Ultimately more questions than answers, but this caused me to understand how to question a lot of the more superficial texts and methods, which may lead in interesting directions.

Not an introductory text, but an excellent addition to the field that really ties together many ideas in the safety of complex systems. Fully indexed with complete references.

Product Description
If you are litigating accidents involving emergency vehicles, you know that many factors contribute to hazardous operations. In this well-documented volume, Stephen S. Solomon, firefighter, EMT, engine captain, fire commissioner, and optometrist shows how vision is an integral component in accidents, and in their prevention.

Whether fire truck, police car, or ambulance, an emergency vehicle's fast speed, bells, whistles, sirens, and flashing lights contribute to excitement-laden and potentially hazardous situations. For the attorney or investigator who must reconstruct events, it is important to understand how some of these supposed safety features actually cause rather than help prevent accidents.

The knowledge you will gain about the anatomy of the eyes will give you the edge in accident reconstruction. You will learn how inadequately trained emergency vehicle drivers make for a potentially lethal situation. ... Read more

5-0 out of 5 stars Food Safety for Everyone
This book is a great introduction to food safety. It is accessible to a person who knows little about food safety without "dumbing it down." The book contains many credible epidemologial studies and warns the danger of food-borne diseases.

But the biggest strength of this book is the author's ability to translate all the information into practical and actionable advice. The book contains a series of checklists that you can photocopy and stick it in your kitchen.

I highly recommend this book to anyone who is serious about food safety.

5-0 out of 5 stars Protect your family - Learn from this book
A book that describes how that which sustains you may also maim you. Light reading? Hardly, but it is also not a bad or difficult read. Dr. Satin's prose and ability to address sensitive topics without bludgeoning the reader makes this book enjoyable indeed. It would be easy to assume that food borne pathogens and the illness that they cause is the other guy's problem. Dr. Satin shines a bright light on food handling and shows that we can all improve. He exposes the reader to the best that science has to offer to help us avoid illness. He repeatedly shows food irradiation to be a valuable tool that the alarmists in the food activists groups have unfairly maligned. As food irradiation becomes more available in the local grocery and restaurant, it is my hope that rational voices like Dr. Satin's carry the day.

Proof of Dr. Satin's wit shows through while debunking myths and passing on advice.
-If anybody insists that a little dirt is good for you, take them out to the garden and have them swallow a thimbleful of the stuff.
-When you are finally sure you can eat off the floor, make sure you do not!

If you can stand to take a critical eye at your own food prep and local restaurant, this book will help you avoid the dangers in your food. Protect your children, pregnant and elderly with the ideas you will garner from this text.

5-0 out of 5 stars excellent
Food Alert is a must for every kitchen. And should be placed right next to our cookbooks to remind us of the dangers of infected food.

During the last days in 1992, at the height of the Christmas season, severalpeople on the west coast began to succumb to a lethal food-borneinfection. Most of the unfortunate victims were children who had eatenhamburgers at a Jack in the Box restaurant. Toddlers who came in contactwith the children who had eaten the burgers were also infected. It infectedover 700 people in Washington, Idaho, California, Nevada. The reallyheartbreaking part of this whole mess was that several of the youngestvictims died an unusually cruel and agonizing death, and a lot of thosethat lived have had to have repeated intestinal surgery and extendedkidney dialysis, many will be infected for the rest of their lives. Foodborne illnesses can happen at home also, and can be just as fatal andthat is why this book is so important to have in our kitchens. More than600 food borne diseases are known about today. Food alert provideseverything that we need to know to protect our families

1) Learn about the 20 most causes of food contamination in your kitchen. 2)What steps you can take to lower the risk of food borne illness. 3)The causes of food borne disease.4)How to avoidbuying, handling, or eating contaminated food.5)How to tell ifyou might be suffering from a food borne illness.

5-0 out of 5 stars This is a very readable and a very sobering book. Get it!
This is an excellent book!Satin really seems to empathise with readers because he is able to explain very complicated medical and technical jargon in a way that anyone can easily understand. Not only is it a 'Who's Who'of all the causes of food poisoning, but it also a manual on how to avoidthe experience.Although the book is crammed with an unbelievable amountof information on all the food-borne diseases and how to avoid them, it isan easy book to negotiate through. There is an excellent index, glossaryand in particular the appendices on disease symptoms and food storage makethis one of the most practical and convenient books I ever read. The bookis a bit frightening because, despite the author's sense of humour, thebook tells it all - the food poisoning problems in the home, in restaurantsand fast food places, in school cafeterias, even on cruise ships, picnicsand barbecues.Satin really gets into what goes on in the food industry. I was particularly concerned with some of the terrible long-termconsequences of food poisoning. This really yanked the subject away fromthe typical image we have of food poisoning as being simply diarrhoea or anupset stomach. The book also talks about the added dangers of foodpoisoning for young kids, pregnant women, senior citizens and people whohave AIDS.Satin always refers to actual recorded cases, whichbrings thematter much closer to home, because you can see how easy it is for anyonein your family to get food poisoning.Unless you know what you are doing, eating is like playing roulette. This book will definitely improve yourchances of avoiding food poisoning. A good knowledge of food poisoning is amust these days. That's why I highly recommend this book as a 'must' read. ... Read more

I didn't actually buy this book, I found it at a library. This is a very informative book, though I'm sure it's a bit dated. It covers many self-defense techniques with both words and lots of pictures. You've got hand-to-hand, fighting with and against batons, knives, grappling techniques, things like that.A big highlight is the pictures. Epidemiology has long played a critical role in investigating outbreaks of foodborne illness and in identifying the microbial pathogens associated with such illness.Epidemiologists were the detectives who would track down the guilty culprit- the food vehicle carrying the pathogen, as well as the fateful errors that resulted in contamination or multiplication of pathogens. The first book of its kind, this volume describes the various ways epidemiologic principles are applied to meet the challenges of maintaining a safe food supply. It addresses both the prevention and control of food borne illness. Starting with a history and background of food borne illness, the book continues by describing the means of following up on an outbreak and measuring exposures. The book concludes by describing the regulatory context that shapes food safety activities at the local, national and international levels. Chapters are written by leaders in the field of public health and food safety, including experts in epidemiology, microbiology, risk assessment, economics, and environmental health and policy. This is the definitive book for students, researchers and professionals interested in how epidemiology plays a role in keeping our food safe. ... Read more

Product Description
There is growing concern globally over issues of aviation safety. Awareness of previous failures and their causes is one of the most important factors in determining risks and hazards in any new operational systems. This requires experience of accidents and failures across a broad spectrum of complex systems. Every accident occurs as a result of a chain of errors, and if one of the "links" making up that chain can be broken, the accident might be prevented - and becomes merely an "incident." If you collect detailed data from a range of "incidents," relating to how they occurred, and develop a consistent method for analyzing that data, you can create a potentially valuable resource to assist in accident prevention. Serious Accidents and Human Factors proposes an original and structured approach to accident prevention. In an interesting and readable collection of accounts of major accidents, drawn mainly from the aviation industry, Masako Miyagi investigates incident reports analytically and reveals the critical information hidden therein that could avert a full-blown accident or disaster. She applies an innovative analytical technique - multidimensional analysis of incident reports (MAIR), using Quantification Method III to validate the results and focus upon individual components identified within the causal chain of events that precede an accident. She advocates wider acceptance and use of the Incident Report Analysing System, ideally administered by a neutral and independent body, to help prevent accidents not only in aviation but in relation to all complex systems, such as nuclear power plants. Serious Accidents and Human Factors offers aviation industry personnel, as well as those involved more generally with safety, risk assessment, and accident prevention in other industries, a comprehensive understanding of the accident causation chain, events contributing to that chain, and a method for identifying and eliminating causal factors in a pro-active way. 4-0 out of 5 stars IRAS, MAIR, And Quantification Method III
"Serious Accidents And Human Factors: Breaking The Chain Of Events Leading To An Accident: Lessons Learned From The Aviation Industry" is an important addition to the library of people interested in aviation and industrial safety and Human Factors (HF). Masako Miyagi, a longtime HF researcher, uses this book to present a lot of data on HF and decision making in critical situations, and ultimately makes several recommendations for analysis and distribution of safety related data, most notably the use of Incident Reporting Analyzing Systems (IRAS). She makes extensive use of Multidimensional Analysis of Incident Reports (MAIR) and a statistical methodology called "quantification method III" throughout the book. For the mathematically inclined, it may help to read Appendix A, which explains quantification method III, before reading the book. I am fairly mathematically literate, but the methodology is cumbersome to understand and ultimately rather confusing unless you have been exposed to extensive graduate level statistics.

The illustrations are generally good (especially the color plates), but they do require careful study as they are two-dimensional results positioned in multidimensional Euclidean space. The quantitative data is expressed in terms of symbolic logic, deriving the eigensolution for latent equations that use as their coefficients the degree to which the varying factors exist simultaneously. (See p. xxii for more information on this analytical method.) Correspondence Analysis based on the "anti-test-estimation-mathematical statistics" philosophy of data analysis in Japan is also touched on (p. 240): it has striking similarities to cluster analysis in the United States, and is functionally similar to quantification method III. The resultant spatial relationships in this book are sometimes hard to conceptualize, so be prepared to read and re-read certain sections. Even for the less mathematically inclined, however, the book is a very valuable work, and is good at exploring the relationships between different factors in accidents and incidents.

Parts of the book separates data into categories labeled "degrees of danger." I am unsure exactly how the category assignments were made, but after reading descriptions of the incidents involved, I think many pilots would frequently disagree with the categorization of specific incidents. I gave the book four stars instead of five largely due to this issue. Other problems in the book are fairly minor. The author makes erroneous statements about crosswind landings (p. 91), windshear (p. 92), and the safety of go-arounds (p. 167). Other detractors are extremely peculiar word choice (e.g. "conchoidal") and frequently odd grammatical structure. I believe that some of these problems, if not all, are caused by translation errors.

In all, I found this to be a very valuable book for any safety professional, especially those specializing in aviation HF programs. There are many excellent lessons contained in this book, but sometimes deliberate pondering is required to understand them all.

Product Description
Although child protection has become a major issue, the abuse of children with disabilities has been largely ignored. International research shows that this group of children is at greatest risk from all forms of maltreatment. However, the needs of children with disabilities are not being met - first, because these children are not being acknowledged as a target population by the community at large and, second, because service providers are so overwhelmed by other problems that they have not begun to take up the challenge of addressing the special needs of this most vulnerable minority group. This book, written at the request of teachers, social workers and psychologists, is in two parts. Part 1 provides the information needed by parents, teachers and other caregivers for an understanding of personal safety skills for all children, and explains the risks and considerations peculiar to children with special needs. Possible approaches and methods for their protection are introduced. Part 2 provides practical curriculum ideas and exercises for developing safety skills in children and young people with mild to severe disabilities. The curriculum ideas are sequenced to help with the development of basic safety knowledge and skills, and may also be used in mixed ability classes. ... Product Description
Compatibility of Pharmaceutical Products and Contact Materials

Dennis Jenke

Important safety aspects of compatibility for therapeutic products and their manufacturing systems, delivery devices, and containers

Compatibility of Pharmaceutical Products and Contact Materials helps pharmaceutical, toxicology, analytical, and regulatory affairs professionals assess the safety of leachable and extractable chemicals associated with drug product packaging, manufacturing systems, and devices. The most comprehensive resource available, its coverage includes the strategies, tactics, and regulatory requirements for performing safety assessments, along with the means for interpreting results.

Structured around a logical framework for an extractables and leachables safety assessment and closely linked to the pharmaceutical product development process, Compatibility of Pharmaceutical Products and Contact Materials directly addresses the fundamental questions of "what activities need to be performed to completely, efficiently, and effectively address the issue of product safety from an extractables and leachables perspective?" and "when do the various required activities need to be performed?" Specifically, the chapters describe:

  • Pertinent regulations and practical ways to meet guidelines

  • Coordinating manufacturing, storage, and delivery systems development and qualification with therapeutic product development

  • Materials characterization and the materials screening process

  • Component and/or system qualification (illustrated by several case studies)

  • Performing validation/migration studies and interpreting and reporting the results

  • Creating a product registration dossier and putting it through regulatory review

  • Product maintenance (Change Control) from an extractables and leachables perspective

  • Likely future developments in extractables and leachables assessment

Additionally, the book's appendix provides a database, including CAS registry numbers, chemical formulas and molecular weights of extractable/leachable substances that have been reported in the chemical literature.

Detailing the interconnected roles played by analytical chemistry, biological science, toxicology, and regulatory science, Compatibility of Pharmaceutical Products and Contact Materials supplies a much-needed, comprehensive resource to all those in pharmaceutical product or medical device development. ...
