Their close proximity to the ear is why some patients experience ear symptoms and can often hear the joint pop or crack. Some TMD problems are caused by dysfunction of this disk. When healthy it acts as a sort of "shock absorber" for the joint. Teeth The teeth are like the third leg of a tripod, the TMJs being the other two legs. The alignment of your bite and the functioning of the TMJs are intimately connected. They all suffer from what has come to be known as temporomandibular joint (TMJ) disorders. HOW THE CHEWING MUSCLES AND JAW JOINTS WORK The structures that make it possible to open and close the mouth are very specialized and work together when you chew, speak and swallow. These structures include the muscles, ligaments, bones and joints of the jaw. Five pairs of muscles allow you to open and close your mouth. They also control forward, backward, and side-to-side movements of the lower jaw. Also involved in these movements are the temporomandibular joints. Each of these important joints has two sections, connected by a disk, that make possible the hinge and gliding actions needed to open the mouth widely (Fig. 1). It is the Temporo-Mandibular Joint (TMJ), the joint where the mandible (the lower jaw) joins the temporal bone of the skull, immediately in front of the ear on each side of your head. A small disc of cartilage separates the bones, much like in the knee joint, so that the mandible may slide easily; each time you chew you move it. But you also move it every time you talk and each time you swallow (every three minutes or so). It is, therefore, one of the most frequently used of all joints of the body and one of the most complex. You can locate this joint by putting your finger on the triangular structure in front of your ear. Then move your finger just slightly forward and press firmly while you open your jaw all the way and shut it. The motion you feel is the TMJ. You can also feel the joint motion in your ear canal. These maneuvers can cause considerable discomfort to a patient who is having TMJ trouble, and physicians use these maneuvers with patients for diagnosis. How Does the TMJ Work?
