Introduction
Table Of Contents Up to 12% of ninth graders have had a panic attack. About 1-2% of all adults have multiple panic attacks. If you look at adults with panic disorder, 20% had their first panic attack before age 10. The first question is, What is a panic attack? Here are the official criteria:
Panic Attack
A discrete period of intense fear or discomfort, in which four (or more) of the following symptoms developed abruptly and reach a peak within 10 minutes:
  • 1. palpitations, pounding heart, or accelerated heart rate 2. Sweating 3. Trembling or shaking 4. Sensations of shortness of breath or smothering 5. feeling of choking

Does your website make you money? WHAT IS A PANIC ATTACK? A panic attack is a sudden rush of uncomfortable physical symptoms coupled with thoughts of impending doom (making a scene, heart attack, not being able to breathe). Although the first panic attack may take place in a distinct situation, further episodes are unpredictable as to time or place. One or more uncomfortable physical symptoms will be present in a severe form during an actual panic attack, such as increased heart rate, dizziness or lightheadedness, shortness of breath, inability to concentrate, and confusion. After a number of panic episodes, the individual can become afraid of being a helpless victim of panic. He or she may hesitate to be alone, to venture far from home, or to be in public places. Even when not experiencing an anxiety attack, the person with panic attacks often becomes increasingly nervous and apprehensive. He or she attempts to remain physically and psychologically tense in preparation for the next attack.

Panic disorder with agoraphobia Definition Return to top Panic disorder, one of the anxiety disorders, is characterized by repeated and unexpected attacks of intense fear and anxiety. Panic attacks are usually not related to a particular situation (although attacks in patients with social and specific phobias are usually expected or "cued" to a recognized or specific stimulus) and typically "peak" within ten minutes of their onset. Agoraphobia refers to a fear of being in places where escape might be difficult, or where help might be unavailable in case of a panic attack. Causes, incidence, and risk factors Return to top The exact cause of panic disorder is unknown, but it is associated with multiple physiological factors. Panic disorder can occur with or without agoraphobia , but agoraphobia develops in more than one-third of cases. The development of agoraphobia may involve learned behavior, since it reflects a fear of experiencing panic attacks in unprotected settings. Sometimes the association of panic attacks occurs with areas where they have happened.

Treatment is essential One of my trickiest moments as a medical student was in my psychiatry finals when I was introduced to an apparently healthy young woman. I was told that she had a large collection of traffic tickets for abandoning her car in a one-way street at the entrance to a small bridge which crossed a river which ran through the centre of town. The examiner invited me to ask a few relevant questions to help establish the diagnosis. Ask a few questions Fortunately after a few nerve racking seconds I was able to put the pieces of the jigsaw together. I asked the following questions:
  • did she have a fear of being in some sort of situation where there was no obvious or immediate escape? how would she feel strolling on a long beach or in the crowd at a football match? did she prefer to stay at home as much as possible - was it a "safe haven" for her?

    - Phillip W. Long, M.D. (Internet Mental Health) The word "agoraphobia" originally meant "fear of the open marketplace" and meant fear of open spaces. Now it's usually used as a term to describe the fear of going into unfamiliar territory. Agoraphobics have had panic attacks before and are afraid of having a panic attack away from home or away from someone they feel safe with. Agoraphobics might describe their symptoms as panic, panic attacks, anxiety, nervousness, fear, spells, rapid heartbeat, hyperventilation, fear of death, fear of going crazy or the fear of fear itself. Agoraphobics experience anxiety about being in places or situations from which escape might be difficult. They are afraid panic-like symptoms might occur. Agoraphobic fears could include being outside the home; being in a crowd; being on a bridge; and traveling. Agoraphobia usually causes people to avoid these situations or have someone accompany them - sometimes causing serious interruptions and inconveniences and often leading to feelings of low self-worth. Agoraphobics often have a lifestyle which is severely limited by their condition.

    Agoraphobia/Avoidance Behaviour
    While not all people who develop an anxiety disorder develop agoraphobia, many people do. This in turn complicates our experience and adds to the stress and anxiety many of us feel. In the past agoraphobia was known as the fear of open spaces or fear of the 'market place'. Now it is recognised as a fear of having a panic attack in which help or escape may be difficult or we will make a fool of ourselves or embarrass ourselves in some way. The avoidance behaviour can be categorised in a number of ways:
    1. As an overall defence against ongoing panic and anxiety. People may have 'boundaries' of where they can and can't go. Sometimes these boundaries may mean they can't leave the house or perhaps even leave one room. 2. A person may have had a panic attack or panic like symptoms in a particular situation or place and avoid going back into the situation or place for fear of having a panic attack or panic like symptoms.
