SETTING STANDARDS At a recent HIV/AIDS conference that I attended, a doctor described the miraculous recovery of one of his patients, who had come back from severe wasting and TB and was healthy enough to begin fighting her HIV successfully. He talked about her medication schedule in detail, but failed to mention nutrition. During the question-and-answer period that followed his presentation, I asked him about this patient's nutrition support. He apologized for his oversight and repeated to the audience an expression he had been told by one of his patients at his clinic in Haiti: "Taking medicine without eating properly is like washing your hands and drying them in the dirt." Abstract
HIV/AIDS and Nutrition
Diet and nutrition are critical issues for people living with HIV/AIDS [1]. Malnutrition is a common complication associated with this chronic disease process, both early on and throughout disease course progression. The majority of individuals affected by the disease may be at risk for compromised nutritional status resulting from a depressed immune system [2]. Further, nutritional deficiency has become a greater concern as HIV-infected people increasingly survive previously life-threatening complications. Although much has been learned about nutrition and HIV/AIDS over the course of the epidemic, knowledge gaps continue to exist [11]. In addition, the published literature concerning HIV/AIDS and nutrition is dispersed among various disciplines. This scatter and seepage in the published literature, coupled with lacunae in the body of knowledge, are important given the high morbidity and mortality rates associated with HIV/AIDS-related malnutrition if not adequately managed.
Methodology

"Initial trials have confirmed that coconut oil does have an anti-viral effect and can beneficially reduce the viral load of HIV patients" , University of the Philippines' Emeritus professor of pharmocology Dr. Conrato S Dayrit said. A minimum of 50 ml of coconut oil would contain 20 to 25 grams of lauric acid, which indicates that the oil is metabolized in the body to release monolaurin which is an antibiotic and an antiviral agent. Among the saturated fatty acids, lauric acid has the maximum antiviral activity, he said. Based on this research, the first clinical trial using monolaurin as monotherapy on some of the HIV patients was conducted recently. Dr. Dayrit's conclusions after the study: " This initial trial confirmed the anecdotal reports that coconut oil does have an anti-viral effect and can beneficially reduce the viral load of HIV patients. The positive anti-viral action was seen not only with the monoglyceride of lauric acid but with coconut oil itself. Optimism regarding their prognosis would be foolish. Except for the lucky few who stop "treatment" in time, they will die. Death is the expected biochemical consequence of taking AZT, for the fundamental action of the drug is to terminate DNA synthesis, the very life process itself. For more than a decade now, we've been told that the human immunodeficiency virus causes acquired immune deficiency syndrome, which in turn causes death. In the war on AIDS, it's only a matter of time. The AIDS issue may well become America's Vietnam of the 90s. Think back to the mid-60s, if you're old enough. You'll remember that as the United States got increasingly involved in the Vietnam War, the government was adamant about the correctness of its official line: We had to fight Communism in Southeast Asia because if we didn't, the world's nations would fall like dominoes under Soviet sway. The war was in our national interest. The war was winnable. Along with these constantly repeated "facts" came the denigration of anyone who looked at things differently, anyone who had the temerity to question or protest what administration officials were telling us. Fast forward about 30 years. One of the chief architects of America's Vietnam involvement, ex-Secretary of Defense Robert S. McNamara, has revealed that he and other top administration officials, including President Johnson, had their doubts about the official doctrines relatively early into the war. But nobody said anything publicly. Nor did they admit that the protesters might have a point. On the contrary, they continued to vilify them. The administrators had already invested so much in their flawed policy that they'd lose face by admitting that their assumptions had been wrong. So they simply continued having Americans march headlong into the Vietnam quagmire for years, at a devastating cost in American and Asian lives, suffering, and expense. Ultimately, of course, there was the expense of governmental credibility, as well.

Global watch Feeding hope: nutrition plays key role in HIV/AIDS care FAO/WHO publish new manual for people living with HIV/AIDS 25 February 2003, Geneva/Rome A good diet is one of the simplest means of helping people live with HIV/AIDS and may even help delay the progression of the deadly virus, two UN agencies said today.
A new manual published jointly by FAO and the World Health Organization (WHO) recognizes the relationship between infection and nutrition and offers simple and practical dietary suggestions for the estimated 42 million people living with HIV/AIDS.
By bolstering the immune system and boosting energy levels, balanced nutrition can help the body fight back against the ravages of the disease. By maintaining body weight a good diet can support drug treatments and prevent malnutrition.
"The relationship between HIV/AIDS and malnutrition is a particularly extreme example of the vicious cycle of immune dysfunction, infectious disease and malnutrition," said Dr David Nabarro, WHO Executive Director for Sustainable Development and Healthy Environments.
"The nutritional aspect of HIV/AIDS has been ignored for a long time. The attention was always focused on drugs," said Kraisid Tontisirin, Director of FAO's Food and Nutrition Division.

PEOPLE LIVING WITH HIV/AIDS HAVE INCREASED NUTRIENT NEEDS
When infected with the HIV virus the body's defence system - the immune system - works harder to fight infection. This increases energy and nutrient requirements. Further infection and fever also increase the body's demand for food. Once people are infected with HIV they have to eat more to meet these extra energy and nutrient needs. Such needs will increase even further as the HIV/AIDS symptoms develop.
HIV/AIDS reduces food intake
People with HIV/AIDS often do not eat enough because:
  • the illness and the medicines taken for it may reduce the appetite, modify the taste of food and prevent the body from absorbing it; symptoms such as a sore mouth, nausea and vomiting make it difficult to eat; tiredness, isolation and depression reduce the appetite and the willingness to make an effort to prepare food and eat regularly; there is not enough money to buy food.
