Introduction
Nutrition may be a cofactor in HIV progression.
  • Poor nutritional status and infection affect the immune system and interact with each other. Development of opportunistic infection and malignancies.
  • Malnutrition is a very serious problem in AIDS, since the timing of death is directly related to the degree of body mass depletion and is independent of the underlying cause of wasting. When wasting of lean body mass approaches 55% of normal, death is imminent. This occurs independently of the reason for weight loss. "Slim Disease" - Earliest cases in Uganda, in late 1970s, characterized by unexplained wasting. In AIDS, CACHEXIA is present. There is a loss in lean body mass. Superimposed on this may be starvation, a loss of body fat mass.
    Malnutrition Causes Infections
    Protein-calorie malnutrition (PCM) causes depression of cellular immunity with abnormal T cell and macrophage function. This could theoretically worsen the immunodeficiency of people with AIDS, making them more susceptible to infection and worsening existing infections. Virulence may be influenced by a host's nutritional state. Malnutrition may contribute to life-threatening infectious disease.

    1. Why is it important for people with HIV to get medical care? New treatments that fight HIV have been shown to improve health and to help people with AIDS live longer, so receiving care from a doctor who is knowledgeable about HIV is important. Treatments are also available to prevent some illnesses, such as Pneumocystis carinii pneumonia (PCP) . And, there is a new blood test called a "viral load test" that measures the amount of virus in the blood. This test gives people with HIV and their doctors better information on when to start treatment, which may be before any symptoms appear. The viral load test can also tell how well treatment is working. People with HIV should seek early medical care from a doctor knowledgeable about HIV, to find out about drugs that fight HIV and to receive care for other HIV related conditions. Another reason to seek medical care is to have tests done for other infections, such as tuberculosis (TB) and sexually transmitted diseases (STDs) and to receive treatment as needed. These infections can hasten the progression from HIV infection to AIDS. It is especially important for pregnant women with HIV to get medical care, since they can take medication that will significantly reduce the risk of their babies being born with HIV. The progression of HIV infection in young children is frequently different from that in adults; some children become sick more quickly. It is important that children with HIV infection receive medical care as early as possible.

With concern about immunity and the potential of certain foods to provide energy and strength, diets advertising these sought-after results have gained popularity in mainstream media. Individuals can be bombarded with alarming advice. Here are two extreme examples: In 1995, Michael Kent Bilbrey was arrested in Chandler, Arizona for selling a quack potion as a cure of AIDS. He was charged with theft and ordered to pay a restitution of $39,300 after his "potion" was found to have no scientific justification since it consisted of nothing more than cranberry juice, saline solution and household bleach.

    What are HIV and AIDS? AIDS stands for Acquired Immune Deficiency Syndrome. AIDS is caused by HIV (Human Immunodeficiency Virus). HIV attacks the body's immune system, mostly white blood cells (T-cells). Your immune system is what fights against infections to keep your body healthy. Wasting and chronic diarrhoea are late stage manifestations of HIV disease in Central Africa and is thought to be caused mainly by gastrointestinal parasitic infections. However, other chronic opportunistic infections, particularly tuberculosis may also be important. Such wasting undoubtedly contributes to fatigue, lethargy and reduced exercise tolerance. Malnutrition in HIV infection and AIDS Weight loss in HIV is a multifaceted problem. Anxiety, depression and socio-economic factors may be primary factors leading to an inadequate intake of food. Loss of appetite occurs during febrile illnesses, despite increased calorie and protein needs. Many drug treatments used for the prophylaxis or treatment of complications in HIV disease may adversely affect food intake. Diarrhoea resulting in malabsorption of nutrients occurs due to intestinal damage caused by parasitic, bacterial and viral infections. Also, it has been suggested that HIV itself may cause intestinal damage. Furthermore, protein-energy malnutrition causes a decrease in intestinal enzyme secretion and may also contribute to malabsorption.

The Basis of the Discoveries I have indicated throughout this book the existence of pollutants in food and other products. These pollutants were identified using a testing device of my invention known as the Syncrometer(TM). Complete instructions for building and using this device are contained in this book. Therefore anyone can repeat the tests described and verify the data. The Syncrometer is more accurate and versatile than the best existing testing methods. A method for determining the degree of precision is also presented. However at this point it only yields positive or negative results, it does not quantify. The chance of a false positive or a false negative is about 5%, which can be lessened by test repetition. It is in the public interest to know when a single bottle of a single product tests positive to a serious pollutant. If one does, the safest course is to avoid all bottles of that product entirely, which is what I repeatedly advise. These recommendations should be interpreted as an intent to warn and protect the public, not to provide a statistically significant analysis. It is my fervent hope that manufacturers use the new electronic techniques in this book to make purer products than they ever have before.

    By CATHERINE MGENDI In a church compound in Nairobi's Langata area, Dr Nelly Oburra is quietly putting to the test her beliefs on the role of nutrition in managing Aids. HIV/Aids may be killing more Africans because of an underlying malnutrition problem, she contends. "Kenya is undergoing a huge, silent epidemic of malnutrition," says the paediatrician, who has worked extensively with HIV-positive children and, more recently, adults. "We do not take food seriously. Food appears to be simple, but it is a very important and complex matter in this era of HIV/Aids." Oburra, who has been conservatively using nutritional supplements to revitalise Aids patients since 1990, says making a conscious effort to eat right at the individual and household level has the potential to "dramatically" and positively alter the course of the HIV/Aids epidemic. Oburra, who is based at the Anglican Church of Kenya's (ACK) St Barnabas Medical Centre on the Otiende Estate, is one of a small group of medics who acknowledge the important role of nutrition in managing various ailments and is emphatic that all diseases originate from a lack of one kind of nutrient or another. "Nutrition is the medicine of the 21st century," she asserts. "However, knowledge of nutrition is presently assumed and over-shadowed by medical technologies such as drugs and vaccines."

    Dealing with Cholesterol in HIV Disease: Diet and Exercise Strategies
    Changes in blood lipid levels occur in any infection, including HIV infection. Typical changes include an increase in triglycerides with a reduced amount of total cholesterol. Many attributes of HIV disease and its treatment predispose to worsening of cardiovascular disease risk factors. The types of changes we now see with the use of anti-HIV therapies include elevation in both triglycerides and total cholesterol. Medication-induced changes that now concern us greatly, especially those related to the use of protease inhibitors, include a significant increase of LDL-C or "bad" cholesterol with a decrease in HDL or "good" cholesterol.1 The risk for cardiovascular disease appears to be much higher in patients receiving protease inhibitor-based therapy. When cardiovascular disease risk is combined with other risk factors such as family history, insulin resistance, and body shape changes, a multi-disciplinary approach is required. Diet and exercise are two therapies that are self-driven by the patient.
