Brain Aneurysm Foundation Established
Patients with brain aneurysms often present with intracranial hemorrhage. There can be varying degrees of neurologic injury as a result of the initial hemorrhage. Recovery after treatment can often be long and, at times, requires inpatient or outpatient physical and occupational therapy. For patients with unruptured aneurysms, there is often more time for patient education regarding the potential risks of hemorrhage or neurologic impairment from their aneurysms and the risks of treatment. After treatment, the process of recovery requires the full support and understanding of the patient's family, as well as the willingness of the patients themselves. Many of the patients and their family members share common concerns. In response to many of these concerns, members of the Brain Aneurysm/AVM Center (at the Massachusetts General Hospital in Boston) have worked closely with patients and their families to establish the Brain Aneurysm Foundation. The purpose of the foundation is to serve the needs of brain aneurysm patients and their families through support networks, the expertise of its Medical Advisory Board, newsletters, educational materials and public education about the condition. Two other goals of the foundation are to promote and support research, including clinical and basic scientific, on the cause(s) and treatment of brain aneurysms (and related conditions) as well as to provide financial assistance to needy aneurysm patients and their families. The foundation has already begun working in an advisory capacity with several hospitals in other states to set up support groups. The foundation has also recently published

An aortic aneurysm is the dilatation (widening or bulge) of a portion of the aorta, usually at a weak spot in the aortic wall. The aorta is the largest artery in the body. It carries all the blood that is pumped out of the heart and distributes it, via its many branches, to all the organs of the body. The aorta projects upwards from the heart in the chest and then arches downwards, travelling through the chest (the thoracic aorta) and into the abdomen (the abdominal aorta). The normal diameter of the abdominal aorta is about one inch. What causes an aortic aneurysm and who is at risk? Most aortic aneurysms occur in the abdominal aorta, the main cause being arteriosclerosis. This is a condition in which fatty deposits are laid down in the walls of arteries, which are less elastic and weaker as a result. Major risk factors for arteriosclerosis are smoking and high blood pressure, although it also probably runs in families. Other causes of aortic aneurysm include:
  • trauma to the aorta, for example a crush injury to the chest following a car accident

Open repair of Abdominal Aortic Aneurysm Surgery
An aneurysm is a swelling or dilation of a weakened wall in an artery. Arterial aneurysms have a tendency to grow and burst causing life threatening bleeding. Treatment of this disease has been ongoing for many years through open surgical repair of the dilated artery. Using an abdominal incision, open surgical repair of abdominal aortic aneurysms allows complete removal of the aneurysm with replacement using a piece of Teflon tubing. Within the last five years a new approach to treating aortic aneurysms has been developed. This is a minimally endoscopic invasive approach that was pioneered by Surgical Care Associates in this area.
For more information regarding this endovascular aneurysm repair, please refer to Stent Grafts of Aneurysms.

http://www.brain-surgery.com/aneurysm.html
A Stroke is the ultimate "Brain Attack." It can strike the young, healthy, productive head of a household during a soft, shared moment with a spouse anytime. Nothing is more frightening. One might hear: "I'm having the worst headache of my life!" Or, "Oh, my neck is so stiff!" Then the victim might vomit, perhaps have a convulsion which could leave them comatose and all bystanders truly frightened. In the emergency room even the doctors look worried. The CAT scan shows "blood around the brain," something doctors call a "subarachnoid hemorrhage." There could possibly be other problems, such as a blot clot in the brain itself ("intracerebral hemorrhage) and/or something called "hydrocephalus" (water on the brain, building up after blood cells have gradually clogged, the brain's drainage system). There is lots to be done, and in a very short time frame. Backtracking a bit, it is important to get an instant education on the "Brain Attack."
Some basic questions:
  • What is a brain aneurysm?
  • How serious is the problem?

Abdominal Aortic Aneurysm
Bandolier has been asked by a GP whether there is enough evidence about the effectiveness of screening for abdominal aortic aneurysm (AAA) for it to be introduced into his practice. To try and answer this we searched MEDLINE from 1993 to the present to see if there were any new reports which helped. There was no single source of information which brought this subject together, but we did find some interesting papers.
RCT for AAA
A recently published English randomised trial in Chichester [1] has tried to examine the incidence of rupture following AAA screening. It was the most useful report and worth examining in some detail.
The setting was 15,775 men and women aged 65-80 years identified from GP registers and FHSA lists. These people were then randomised by computer to control and screening groups. Those randomised to screening were invited for abdominal ultrasonographic scanning by letter from their family practice; one reminder was sent if there was no reply.
Aneurysm was defined as maximum aortic diameter of 3 cm or more. Annual re-scanning took place if the diameter was 3 - 4.4 cm, and at three-monthly intervals if it was 4.5 - 5.9 cm. This protocol was continued until February 1994 or until the patient died, underwent surgery or declined further follow-up. Aortic diameters of 6 cm or more, and increase of diameter of 1 cm or more in a year, or developments of symptoms attributable to the aneurysm all constituted criteria for considering surgery.

What Is an Abdominal Aortic Aneurysm?
An abdominal aortic aneurysm (AAA) is a bulge in the aorta (the aorta is the main blood vessel coming from the heart that supplies blood to all organs) in your abdomen. Aneurysms may occur in any blood vessel in the body, but the most common place is in the abdomen below the renal arteries (blood vessels that provide blood to your kidneys). The aneurysm may continue to grow larger until, like a balloon, it bursts ("ruptures"). The larger an aneurysm becomes, the easier it grows. Aneurysm rupture can be a life-threatening event. The goal of all aneurysm operations is to prevent the aorta from rupturing. A normal aorta below the renal arteries measures about 2.3 cm in diameter (1 inch) in men and 1.9 cm in diameter (3/4 inch) in women, but varies with age and body size. An aorta is considered aneurysmal when it grows more than 50% over its normal size. Aneurysms are four times more common in men than women and occur most often after 55-60 years of age.

Abdominal Aortic Aneurysm Overview An abdominal aortic aneurysm (AAA) is a bulge in the aorta (the aorta is the main blood vessel coming from the heart that supplies blood to all organs) in your abdomen. In the case of an abdominal aortic aneurysm (AAA), an aneurysm develops in the part of the aorta that extends through the abdomen. The bulge occurs below the renal arteries (kidneys) and may extend into the iliac arteries (legs). Most AAAs occur in association with advanced atherosclerosis, an accumulation of fatty deposits on the vessel wall.

Abdominal Aortic Stent-Graft Program The aneurysm may continue to grow larger until, like a balloon, it bursts ("ruptures"). The larger an aneurysm becomes, the easier it grows. Most people may not experience any symptoms of an AAA until it expands or bursts, resulting in massive bleeding. Aneurysm rupture can be a life-threatening event. The goal of all aneurysm operations is to prevent the aorta from rupturing. Traditionally, surgeons have repaired an AAA by replacing the affected portion of the aorta through a large abdominal incision. This “open” procedure requires general anesthesia, an average hospital stay of five to 12 days, and a prolonged recovery period of weeks or even months.
