    Gout is a disease that causes sudden, severe attacks of pain, tenderness, redness, warmth, and swelling in some joints. It usually affects one joint at a time, especially the joint of the big toe. It can also affect the knee, ankle, foot, hand, wrist, and elbow. The pain and swelling associated with gout are caused by uric acid crystals that deposit in the joint.
    Uric acid is a substance that normally forms when the body breaks down cells and proteins. It usually is dissolved in the blood and passes through the kidney into the urine. In people with gout, because they make too much uric acid or don't excrete it in the urine, excess uric acid crystals settle in joints or other tissues, triggering inflammation. Having too much uric acid is called hyperuricemia. Factors leading to increased levels of uric acid and then gout include excessive alcohol intake, hypertension, kidney disease, and certain drugs. Ankylosing Spondylitis is a chronic inflammatory disease of the spine that can fuse the vertebrae to produce a rigid spine. Spondylitis is a result of inflammation that usually starts in tissue outside the joint. The most common early symptoms of spondylitis are constant sacroiliac and low back pain and stiffness that continue for months.

    Autoimmune disorders are conditions in which a person's immune system attacks the body's own cells, causing tissue destruction.
    Description
    Autoimmunity is accepted as the cause of a wide range of disorders, and it is suspected to be responsible for many more. Autoimmune diseases are classified as either general, in which the autoimmune reaction takes place simultaneously in a number of tissues, or organ specific, in which the autoimmune reaction targets a single organ. Autoimmune disorders include the following:
    • Systemic lupus erythematosus . A general autoimmune disease in which antibodies attack a number of different tissues. The disease recurs periodically and is seen mainly in young and middle-aged women.
    • Rheumatoid arthritis . Occurs when the immune system attacks and destroys the tissues that line bone joints and cartilage. The disease occurs throughout the body, although some joints may be more affected than others. Goodpasture's syndrome. Occurs when antibodies are deposited in the membranes of both the lung and kidneys, causing both inflammation of kidney glomerulus (

Recent studies have shown that the HLA-A2 molecule has two alpha domains separated by a groove which is made of eight antiparallel beta strands (Ebringer, 109). However, since these "arthritogenic" polypeptides in this groove have yet to be identified, this theory will remain unanswered and its use as a model to answer questions about AS will be unknown.     Autoimmune disease results from the breakdown in the body's normal mechanisms of tolerance to self antigens, leading to immune responses being mounted to intrinsic antigens. Ehrlich, a famous Nobel laureate in immunology, eloquently expressed the lack of immune responses to self antigens in health as "horror autotoxicus". Immunological tolerance is determined early in life during thymic development, when T cells recognising self antigens undergo clonal deletion or are rendered anergic. Not all self reactive T cells however, are deleted by this mechanism. Clonal deletion does not remove those T cells that react with "sequestered" antigens e.g. brain, testes, ocular lens.
    Classification of Autoimmune Disease
    Human autoimmune disease may be classified in terms of: (i) HLA Association Susceptibility to most autoimmune disease is strongly influenced by HLA Class II alleles. Within the HLA Class II associated diseases, a further subclassfication in terms of organ involvement is possible. Organ specific autoimmune diseases are characterised by auto-antibody patterns that are primarily directed against a single organ or closely related organs e.g. antibodies against the thyroid gland in Hashimoto's thyroiditis, antibodies directed against pancreatic B cells in IDDM. In contrast, systemic or non-organ specific disease is characterised by a variety of auto- antibodies specific for nuclear and cytoplasmic proteins involved in DNA replication, DNA transcription and messenger RNA translation e.g., antinuclear antibodies in Systemic Lupus Erythematosus.

    Juvenile Ankylosing Spondylitis What is juvenile ankylosing spondylitis?
    Juvenile ankylosing spondylitis (JAS) is a type of arthritis that affects the spine and the sites where the muscles, tendons, and ligaments are attached to bone. “Ankylosing” means stiff or rigid, “spondyl” means spine, and “itis” refers to inflammation. The disease causes inflammation of the spine and large joints, resulting in stiffness and pain. The disease may result in erosion at the joint between the spine and the hip bone (the sacroiliac joint), and the formation of bony bridges between vertebrae in the spine, fusing those bones. In addition, bones in the chest may fuse. JAS is considered to be a multifactorial condition. Multifactorial inheritance means that “many factors” are involved in causing a health problem. The factors are usually both genetic and environmental, where a combination of genes from both parents, in addition to unknown environmental factors, produce the trait or condition. Often one gender (either males or females) is affected more frequently than the other in multifactorial traits. Multifactorial traits do recur in families because they are partly caused by genes. A group of genes on chromosome 6 code for HLA antigens play a major role in susceptibility and resistance to disease. Specific HLA antigens influence the development of many common disorders. Some of these disorders, such as JAS, are autoimmune related and inherited in a multifactorial manner. When a child has a specific HLA antigen type associated with the disease, he/she is thought to have an increased chance to develop the disorder. The HLA antigen associated with JAS is called B27. Children with the B27 HLA antigen are thought to have an increased chance (or "genetic susceptibility") to develop JAS; however, it is important to understand that a child without this antigen may also develop JAS. This means HLA antigen testing is not diagnostic or accurate for prediction of the condition. Males are affected with JAS three times more often than females.
