ARTHRITIS AND RELATED DISORDERS: FIGHTING SELF-SABOTAGE Overview Faculty Web Resources Print Resources "Arthritis" is a term that refers to more than 100 different types of rheumatic diseases that affect people of all ages. In addition to causing pain in the joints and connective tissues, these diseases can also affect many other parts of the body, including the kidneys, lungs, eyes, skin, and central nervous system. Some particularly insidious forms of arthritis, like rheumatoid arthritis and systemic lupus erythematosus, can even cause the immune system to attack the body’s own cells. According to Dr. Fatima Mili from the Federal Centers for Disease Control and Prevention, “Arthritis is not a major cause of mortality like heart disease or cancer, but it has a great impact on the quality of life.” The aches and pains of rheumatic diseases often lead to secondary problems like sleep loss, fatigue, and depression—problems that can, in turn, increase the frequency and intensity of arthritis pain. This vicious cycle of symptoms makes treating arthritis even more of a challenge.

Henoch-Schönlein Disease (Anaphylactoid or Allergic Purpura) Henoch-Schönlein purpura, or HSP, is an inflammation of blood vessel walls (aseptic vasculitis). It is associated with joint pain, abdominal symptoms, and kidney involvement. Its chief symptom is purpura, a pinkish to reddish-brown raised spot on the skin which begins with an itching feeling. HSP occurs most often in children, who usually notice joint and abdominal pain before purpura form. However, in adults, purpura is usually the first symptom. The characteristic purpura is painless and often occurs on the arms, legs, and buttocks. Other symptoms include bruising, abdominal pain, apathy, headache, seizures, blood in the stool, and arthritic pain in the joints. HSP may be triggered by infection (hepatitis B; ß-hemolytic Streptococcus rheumatoid arthritis and systemic lupus erythematosus (discussed below), cancer, and food products (dyes, preservatives). Treatment focuses on minimizing bleeding and may involve administration of steroids. The disorder usually ends on its own after 4 to 6 weeks, although relapses are common.

What is the difference between arthritis and other rheumatic diseases?
Who treats arthritis and other rheumatic diseases? Arthritis and other rheumatic diseases may be treated by your physician and/or other medical specialists and healthcare providers. Several physicians from different medical specialties may be involved in the treatment at the same time. This multidisciplinary team approach is particularly important in managing the symptoms of a rheumatic disease, especially as many symptoms are chronic and change in severity over time. Some of the more common medical professionals involved in the treatment of arthritis and other rheumatic diseases may include the following:
  • Primary Care Physician A primary care physician is one who has specialized education and training in general internal medicine, family practice, or another first-level-of-care area. Primary care physicians are those who provide patients with any/all of the following:

Definition
Autoimmune disorders are conditions in which a person's immune system attacks the body's own cells, causing tissue destruction.
Description
Autoimmunity is accepted as the cause of a wide range of disorders, and it is suspected to be responsible for many more. Autoimmune diseases are classified as either general, in which the autoimmune reaction takes place simultaneously in a number of tissues, or organ specific, in which the autoimmune reaction targets a single organ. Autoimmune disorders include the following:
  • Systemic lupus erythematosus . A general autoimmune disease in which antibodies attack a number of different tissues. The disease recurs periodically and is seen mainly in young and middle-aged women.
  • Rheumatoid arthritis . Occurs when the immune system attacks and destroys the tissues that line bone joints and cartilage. The disease occurs throughout the body, although some joints may be more affected than others. Goodpasture's syndrome. Occurs when antibodies are deposited in the membranes of both the lung and kidneys, causing both inflammation of kidney glomerulus (

Discoid Lupus Erythematosus
Discoid lupus erythematosus is a chronic, recurring disorder characterized by clearly defined round, red patches on the skin. Its cause is unknown. The disorder is more common in females, most often women in their 30s. The age range is much wider than the usual range for systemic lupus erythematosus. Symptoms Diagnosis Systemic Lupus Erythematosus Systemic lupus erythematosus (lupus) is an autoimmune disease that results in episodes of inflammation in joints, tendons, and other connective tissues and organs. Different tissues and organs become inflamed in different people, and the severity of the disease ranges from mild to debilitating, depending on the number and variety of antibodies that appear and the organs affected. About 90 percent of the people who have lupus are young women in their late teens to 30s, but children, mostly girls, and older men and women can also be affected. Occasionally, certain heart drugs (hydralazine, procainamide, and beta-blockers) can cause a lupus-like syndrome that disappears after the drug is discontinued.

Understanding Autoimmune Diseases
Preface
This booklet contains information about autoimmune diseases . You will not find everything there is to know about autoimmune diseases here, however. In fact, the information presented here may prompt you to think of more questions about autoimmune diseases because autoimmune diseases are complex.
We have tried to anticipate which terms will need further definition and clarification. Those terms, when they first appear in the text, are in italics. Italicized words and phrases are defined in the Glossary at the end of the booklet.
You will find more in-depth and detailed resources at your local library or through your health care provider. The Internet is a valuable source of information as well. Start by doing a search on the National Institutes of Health (NIH) Web site at http://www.nih.gov for information on the broad range of research conducted by NIH, including autoimmune diseases.

Study: Breast implants don't cause disease
By JAN GILBREATH,
WASHINGTON, D.C., March 15 (UPI) After studying data from 20 research projects conducted by institutions around the world, a panel of medical specialists has concluded that silicone breast implants do not increase a woman's risk of connective tissue diseases.
The findings, to be published Thursday in the New England Journal of Medicine, expanded on the results of a 1996 study ordered by a federal judge overseeing the cases involving silicone-gel-filed breast implants. The judge, Sam C. Pointer Jr. of Alabama, in 1996 created an independent panel of scientists to advise the court on scientific evidence of the health effects of the implants. The study of other researchers' studies called meta-analysis was originally conducted by scientists at the University of North Carolina at Chapel Hill's School of Public Health, Oregon State University, and Ottawa General Hospital and were published in 1998.
The results published Thursday were authored by Drs. Esther C. Janowsky, Lawrence L. Kupper, and Barabara S. Hulka, all of the University of North Carolina at Chapel Hill.

Lupus Erythematosus

Lupus is a systemic, inflammatory disease most commonly seen in women. The disease affects joints and organs, often with a characteristic rash. Lyme Disease Arthritis can be a secondary consequence of Lyme disease, a condition caused by a bacterial infection transmitted by a tick which feeds on deer. The name of the disease is derived from the town in Connecticut where it was first recognized. Mixed Connective Tissue Disease (MCTD) Traditional Mixed Connective Tissue Disease (MCTD)is considered an overlap of SLE, Scleroderma, and Polymyositis but the disorder can be an overlap of any combination of the connective tissue diseases. Osteoarthritis Information about symptoms and treatments for osteoarthritis, often called "wear-and-tear arthritis" or "old person's arthritis".

Morbidity
Breast Implant Morbidity
For more than a decade, breast implant morbidity has been the subject of much controversy in both the public and medical profession alike. Implants that had been used for breast augmentation and breast reconstruction for nearly thirty years were suddenly forced off the market by the Food and Drug Administration amidst reports that the silicone-filled implants caused a multitude of chronic, life-threatening diseases. (litigation) Lawsuits followed by the hundreds, and major implant manufacturers soon found themselves in major legal battles. Patients alleged that the implants had caused their cancer, arthritis, or other chronic illnesses. Insufficient study and questionable professional testimony led to a judgement in favor of the patients and the downfall of at least one major silicone implant manufacturer. Interestingly, however, with the litigation concluded and the patients compensated , the issue of implant morbidity has recently received little or no coverage from the media. This is unfortunate since the issue of implant morbidity has only recently begun to be clarified. However, does breast implant morbidity really exist? And if it does exist, how does it occur? Exploring these questions further primarily necessitates clarifying the silicone breast implant controversy.
