Publication Date: August 2000 Osteoporosis: Progress and Promise About Osteoporosis Osteoporosis is a skeletal disorder characterized by compromised bone strength predisposing to an increased risk of fracture. Bone strength reflects the integration of two main features: bone density and bone quality. Osteoporosis is the most common of the bone diseases that affect Americans. Although it is the underlying cause of most fractures in older people, the condition is silent and undetected in many cases until a fracture occurs. Osteoporosis is a major health risk for 28 million Americans. In the United States today, 10 million individuals already have osteoporosis and 18 million more have low bone mass, placing them at increased risk for this disease. American women are four times more likely to develop osteoporosis than men. One out of every two women and one in eight men over 50 will have an osteoporosis-related fracture in her or his lifetime. Osteoporosis may be attributed to three factors: (1) accelerated bone loss at menopause in women or as men and women age; (2) suboptimal bone growth during childhood and adolescence resulting in failure to reach peak bone mass; and (3) bone loss secondary to disease conditions, eating disorders, or certain medications and medical treatments.

Many individuals rely on extra amounts of Calcium and Vitamin D when trying to treat and/or prevent
bone loss, and when unsuccessful, they generally resort to using drugs specifically formulated to stop
the progression of this disease.  Long-term effects of many of these medications are still unclear since

    What is Osteoporosis?
    Osteoporosis is a silent disease in which bones become fragile and more likely to break. Bone loss usually occurs slowly over time. If not prevented or if left untreated, osteoporosis can progress without symptoms until a bone breaks. These broken bones, also known as fractures, occur typically in the hip, spine, and wrist.
    Any bone can be affected, but of special concern are fractures of the hip and spine. A hip fracture almost always requires hospitalization and major surgery. It can impair a person's ability to walk unassisted and may cause prolonged or permanent disability or even death. Spinal or vertebral fractures also have serious consequences, including loss of height, severe back pain, and deformity.
    Millions of Americans are at risk. While women are four times more likely than men to develop the disease, men also suffer from osteoporosis. Indeed, twenty percent of men over the age of fifty have osteoporosis.
    Fortunately, osteoporosis now can be diagnosed before fractures occur by measuring bone density. With early treatment, not only can fractures be prevented, but most patients can also avoid the pain and mobility problems associated with severe osteoporosis.

    RETURN TO CHANNEL HOME Osteoporosis is a condition where loss of bone develops to the point where our bones break easily. It is a serious condition that increases slowly as we age. Generally osteoporosis is asymptomatic-we are unaware that we have it-until there is a fall or injury resulting in a fracture. We all get osteoporosis as we age so it is important that we learn about it and try to do something to slow it down or reverse the process. Common areas of the body where osteoporosis fractures occur:
    • The wrist - usually following a fall on the outstretched hand. The hip - often from slipping and twisting during a fall. The spine - from bending forward or heavy lifting.
    Bone tissue is a rigid, living structure that is constantly renewing itself. It is not an unchanging skeleton made of calcium. Microscopic holes are constantly created by bone eating cells called osteoclasts. Once these bone cavities are created, bone building cells called osteoblasts form new bone. This process rejuvenates the bone and repairs any damage. In early childhood and adolescent years our bones grow rapidly. Good dietary

    Often we speak of healthy bones and strong teeth, and most of us remember our parents' admonition to "drink your milk." Though we may have resisted then, as adults we are now more aware of the need for calcium, and perhaps calcium supplements, as well.
    An adequate intake of calcium can help prevent the bone loss associated with osteoporosis, a condition where the structure, or matrix, of bone is no longer as solid or as strong as it should be. The bone matrix is composed of a mineral component that includes both calcium salt and phosphate salt, and an organic component that is largely collagen fibers. The calcium and phosphate salts combine to form the hydroxyapatite crystals that give bone its amazing ability to withstand stress. Collagen is the main protein of skin, tendon, cartilage, bone and connective tissue. It is collagen that provides flexibility to bone. The mineral component makes up most of the mature bone matrix, with the remainder basically comprised of collagen and water.
    When viewed through a microscope, bone matrix resembles a sponge, with a stronger “compact” tissue found on the outer surface of the bone, and a softer, or “spongy” tissue towards the middle.
