Understanding and Coping With Postpartum Depression

Coping Thirteen years ago, I miscarried the baby my husband and I had struggled so hard to conceive, an experience I wrote about for the August 1988 issue of Parents' Press. Two years and two more miscarriages later, I managed yet another pregnancy, aided and encouraged by my husband and a support group, and some mild medical intervention. This time we were more fortunate: our daughter Margaret Isobel, a vigorous nine-pounder, came wailing into the world on Aug. 22, 1989. Again, I reported back to Parents' Press about the highs and lows of parenting after infertility. The low was postpartum depression. What follows here is a revised version of my 1991 account, with resource updates. The pregnancy was physically easy, but labor was hard: 24 erratic hours, followed by three hours of fruitless pushing before we decided that our obstetrician was right: there was no way Margaret would emerge except through my abdomen. I was disappointed to have gone so long only to end with a C-section, but exhilarated to finally stop hurting and to see our little girl.

Post-partum depression can be a problem for many new mothers. How can we help new moms avoid this problem? News 5 Medical Team reporter Dr. Walt Larimore explains. About 10 to 15 percent of new mothers experience post-partum depression. It can range from mild feelings of loneliness to severe psychotic episodes. We may be able to learn something from a new British study about how to combat post-partum depression. In the study, British woman, after giving birth, were either visited by a midwife six to seven times during the two weeks after delivery, or received normal care, seeing her doctor after six to eight weeks. Researchers studied about 2,000 women who received either standard care, or had additional visits by midwives 10 to 12 weeks after birth. The women who received the extra visits had better overall mental health. Children of mothers with post-partum depression can have increased problems with development and behavior, so preventing the problem is important for the children too.

    Fish oil studied as method to prevent post-partum depression HOUSTON–-(March 26, 1999)–-Fish oil capsules containing Omega-3 fatty acids are being studied as a preventive measure for post-partum depression. Omega-3 fatty acids are found in cell membranes throughout the body, including those in the brain's nerve cells. Over the years, changes in the American diet have lead to a decrease in dietary Omega-3, found in cold-water fish such as cod, tuna, salmon and sardines. "While Omega-3 has decreased, Omega-6 fatty acids, also found in the cell membranes, have increased," said Dr. Lucy Puryear, director of The Women's Clinic in the Department of Psychiatry at Baylor College of Medicine in Houston. "We think this shift is affecting brain chemical receptors and impacting the brain's levels of serotonin, which is thought to be linked to depression." Studies have shown that people with depression have lower levels of Omega-3. "We also know that during pregnancy the fetus is taking Omega-3 from the mother to make brain and nervous tissue," Puryear said. Women with post-partum depression experience sleep difficulties, excessive crying, loss of energy and appetite, and a loss of interest in activities. The condition generally begins within the first four weeks but can occur up to four-to-six months after delivery.

    There are a lot of surprises when a couple finds out they are expecting their first child.  As you go through pregnancy you experience all kinds of things you never realized would happen!  Then the big day arrives and your baby is born another day of surprises but they’re all worth it when you see your new bundle for the first time! For most of us, pregnancy is not an easy time.  At the very least, it is uncomfortable!  And giving birth is definitely not easy.  You go into labor tired and generally miserable from the discomfort of being 9 months (or more!) pregnant.  Labor may be long or short, but in any case it will be incredibly intense.  Giving birth is probably the hardest work anyone will ever do.  Then when your baby is born there is just nothing like the incredible emotional high of meeting your child!  Every time you look at him/her you can hardly believe this new little person is yours to love and care for. Especially with a first child, we often expect things to get "back to normal" once we get home from the hospital.  But there are a whole host of additional surprises in store.  Until you’ve experienced it, there’s just no way to imagine how tired you can get when you need to feed this little person every couple of hours around the clock.  If you’re nursing, your milk comes in after a few days and that is usually another shock!  Breast-feeding is applauded as such a wonderful experience, but maybe no one warned you that the first few weeks will probably be uncomfortable and challenging to put it mildly!

    Laurie Barclay, MD NEW YORK (MedscapeWire) Apr 12,2002 — Several studies summarized at the 223rd national meeting of the American Chemical Society on April 8 suggest that docosahexaenoic acid (DHA) fatty acid supplements given to nursing mothers may improve early infant development. DHA supplements may also reduce the incidence of postpartum depression. "We believe that the high incidence of postpartum depression in the United States may be triggered by a low dietary intake of DHA," presenter David J. Kyle, PhD, from the Mother and Child Foundation and Advanced BioNutrition Corp in Columbia, Maryland, said in a news release. "The higher the intake of DHA, the lower the incidence of depression." A 1998 study by Joseph Hibbeln of the National Institutes of Health found a significant inverse correlation between DHA intake and incidence of clinical depression, and a more recent study by Hibbeln found the same relationship between DHA levels in breast milk and incidence of postpartum depression. During pregnancy, the placenta pumps DHA from the expectant mother to the fetus, increasing the mother's susceptibility to depression.

Pregnancy also increases levels of endorphins, the body's feel-good chemicals. Endorphin levels fall abruptly after delivery, adding to risk of depression. Hormone levels stabilize in almost all women, including those who become depressed, two weeks after delivery; but it is possible that in some women who are highly susceptible to the effects of changing hormone levels shortly after delivery, the mood deepens and persists because of other past and present circumstances. A history of depression increases risk. One-third of women with previous depression experience it post-partum.Women with postpartum depression are more likely to have had previous mental illnesses, depression during pregnancy, and a family history of mood disorders, especially bipolar disorder.     A considerable percentage of the female Population suffer from PND. Government Statistics reveal that between 10% and 15% of new mothers become clinically depressed after the birth of their child. These same statistics also record that suicide is the most common form of maternal death in the first year after the birth. As with any form of Depressive illness, Post-natal depression can be so severe, it may require hospitalisation.
    Symptoms of Post-natal depression
    Post-natal depression conforms to all of the symptoms of a typical depressive episode (persistent low mood, appetite loss irritability physical and mental fatigue etc), and can be, mild, moderate or severe. However, what makes this form of depression different is that is occurs during a period, which although exhausting, is socially expected to be one of great joy and happiness. This can intensify the emotional feelings of guilt and inadequacy. One in two mothers will experience the ‘baby blues’. This is a rapidly passing phase of insecurity and upset, which usually occurs three or four days after the birth. However, these blues usually evaporate within a week or so, as the new mothers learn to adapt and cope. The fundamental difference between these ‘blues’ and Post-natal depression is that, women with PND do not bounce back. If they do manage to establish a new routine, they will feel pressured and exhausted, and rather than recovering from the ‘Baby Blues’, will sink into a deep depressive belief that things are getting worse and worse.

    Postpartum depression and the more severe postpartum psychosis are of major concern today. On this site you'll learn about the causes, symptoms and treatments for the condition. The terms PPD and postnatal depression are also used as synonyms of postpartum depression.
    What are the baby blues?
    Feeding, diapering and taking care of a new baby can be overwhelming. About forty to eighty-five percent of women experience intense highs and lows as they adjust to life with a new baby. Feeling anxious and crying for reasons she can’t explain are actually normal behaviors for the mother of a new baby. With feedings coming up every few hours, the mother of a new baby is likely to feel tired and have trouble falling asleep after her deep sleep has been interrupted. She may have trouble concentrating and feel irritable. Sudden weight gain and weight loss are both normal, as are fluctuations in appetite. These symptoms make up the baby blues . According to most research, these symptoms may last several weeks, often peaking during the first week following delivery. They usually go away by themselves.

    No one knows the exact causes of post partum depression. PPD is more than the baby blues; it lasts longer and is more severe. The “baby blues” typically resolves within about ten days after delivery. Post partum depression lingers. The baby blues cause mild sadness and mood swings. PPD causes severe depression and loss of interest in life.
    After pregnancy, progesterone levels fall drastically. Like progesterone, the levels of estrogen, a hormone that seems to be important in memory and mood, fall 90 to 95 percent in the first 48 hours following delivery. Thyroid levels may also drop sharply after birth. A thyroid deficiency can produce symptoms that mimic depression, such as mood swings, severe agitation, fatigue, insomnia, and anxiety. A link between estrogen levels and postpartum depression has not been established [McEwen, 1993] and experts cannot agree on the role that hormone fluctuations play in post partum depression.
    Post partum depression researchers are also investigating genetics as a cause of the disorder.
