Nearly 80% of women who have just given birth find themselves having a case of the “baby blues.” This is characterized by apprehension or anxiety associated with the new responsibilities of being a mom, along with a mild feeling of sadness or the “blue feeling.” This usually lasts anywhere from a few days to a few weeks after the delivery, and subsides just as effortlessly as it began. For some women, however, the feelings of sadness and anxiety are more intense and don’t just go away. Here begins the line that separates the normal reactions associated with a new baby, and postpartum depression (PPD). bodyOffer(19885) About 10-15% of women suffer from true PPD. These women experience symptoms such as anxiety, irritability, insomnia, feelings of guilt, difficulty concentrating, persistent crying and overall sadness. These usually surface within the first six weeks and on an average last from three to six months. It is thought that hormonal changes play a major role in both baby blues and PPD. After delivery, hormone levels drop rapidly and can affect a woman’s moods. This, in association with the fatigue from giving birth and the lack of sleep most experience early on in a baby’s life, can alter a new mom’s state of mind a great deal. A mother suffering from PPD will perpetuate her own symptoms of depression by not taking advantage of the opportunities she gets to escape the stresses of new parenthood. She will worry excessively which increases anxiety and adds to the exhaustion already felt because of the existing insomnia she is most likely experiencing.

ne out of every 10 women will suffer from clinical depression during pregnancy which puts them at increased risk for postpartum depression. Many expectant mothers resist taking antidepressant medications, fearing (the potential and often unknown) dangers that drugs may pose to fetal development and to the long-term health of the child. prominent PMS researcher, Barbara Parry, MD, of the University of California, San Diego, recently established that PMS is related to light and that phototherapy (light therapy) can alleviate PMS symptoms. Why did the phototherapy work? When people are exposed to bright light, they produce serotonin, a neuropeptide (a chemical that carries messages between nerves) that helps us feel calm, alert and happy during the day. At night, when it is dark, the brain produces less serotinin and more melatonin, a neuropeptide that helps us sleep deeply. Researchers at UCLA (Rapkin et al., 1098) have shown that serotonin levels drop just before ovulation in all women. Rapkin shows that this drop correlates with the onset of PMS symptoms, and that serotonin levels rebound with the onset of menstruation when PMS symptoms decrease. This and other recent research suggests that PMS occurs in women with low base levels of serotonin, when serotonin levels drop further at ovulation, these levels fall low enough in PMS sufferers for symptoms to appear.

My bleeding had stopped but now it has started up again. Is this normal? At first your lochia or vaginal discharge is much like a heavy period. By day 3-7 it slows down and turns pink or brown in colour. By two weeks the flow is light. Usually the lochia disappears by 6 weeks after birth. Sometimes it will start up again.This may mean you are doing too much and you need to slow down a little. Here are some reasons to call your healthcare professional. If you have vaginal bleeding more than a heavy period, if you have bleeding that soaks a pad in less than two hours, or if you pass a clot bigger than a plum. Post Partum Recovery Difficulties with breastfeeding and the added stress of being a new parent can contribute to a delay in Post-Partum Recovery. Post Partum blues is a mild depression occurring within the first couple of hours to the first week or two after delivery. This can happen to an estimated 50 - 70% of women to varying degrees. The Post-Partum blues can last anywhere from a few hours to a week or two, and in some cases as long as six weeks. Usually with time and support women can concur this blues period but it is estimated that approximately 15% of women experience a depression which is more intense than the post partum blues. This can lead to Post Partum depression. Women who have a personal or family history of depression, have a very high stressed work or home life, seem to be more susceptible to this form of depression. The rate of depression also increases as a woman ages. Signs of depression are similar to the blues but also include withdrawal from family and friends, loss of interest in normal activities, mood swings and in extreme cases thoughts of harming the baby or oneself.

Occurrence: Up to 80 percent of all new mothers experience some signs.
Symptoms: Mild depression, irritability, weeping, frustration and fatigue.
Time frame: 3-14 days after delivery.
Treatment: Monitoring, support and TLC
Depression

Women at greater trisk of postpartum depression are those with depression following a previous pregnancy (about 70% risk), adolescent mothers (30% risk) and women with a history of depression in the past not associated with pregnancy (30%). For most women, this depression is transient, resolving within six months. However, for some mothers a more lasting type of baby blues occurs. These women may have more severe depression that interferes with their ability to function. Lack of sleep with caring for the new baby, medical complications of childbirth, and other pressures associated with a new baby can worsen the depression. Fathers can also suffer from baby blues. For them it is usually a much milder condition. It should be discussed with a doctor if it interferes with daily activity or lasts more than a few weeks. Most likely postpartum depression is triggered by changes in levels of progesterone and estrogen after delivery although the exact cause is not known.     QUESTION: Dear Dr. Heller, I have been told that Paxil is one of the more mild medications for depression or anxiety...as compared to Prozac, Zoloft, etc. Is that true? Also, do you know if taking Paxil would be effective for postpartum depression? Do you know what the most prescribed medication is for postpartum depression? ANSWER: No, Paxil is not "mild" compared to the others. In fact Paxil often has more side effects than the other SSRI's.
    Post partum depression may be due to low thyroid - and I'd look at that possibility. Finding out what diagnoses were present mildly before the delivery is very helpful as well ( screening test ), particularly in choosing medical treatments.

This syndrome is best seen as an adjustment disorder affecting 7-10 percent of women.
