Vitiligo (also called "leukoderma") is a skin condition in which there is loss of pigment from areas of the skin resulting in irregular white spots or patches, even though the skin has normal texture. Vitiligo may appear at any age. Although it is a progressive condition, many people experience years or decades without developing new spots. The cause of vitiligo is not greatly understood, and there may be many causes that result in the condition.
Vitiligo is more noticeable in darker skinned people because of the contrast, although when they tan, even lighter skinned people are affected. This condition affects about 1% to 2% of the US population, or about 3 to 6 million people. In some countries, the incidence is even higher. Worldwide, there are thought to be more than 100 million people with the condition. Vitiligo would appear to be as old as the recorded history of man - it is mentioned in the Bible, and there are references to it in ancient Egyptian, Greek and Chinese writings.
For years, many dermatologists have told their patients that vitiligo is untreatable, or have offered only limited treatments such as steroid creams or PUVA. Some are so bold as to inform their patients "You're lucky it's not fatal. or "You're lucky you don't have cancer." And while it's true the condition is not fatal or physically debilitating, this is of little consolation to most sufferers. Many doctors are equipped only to prescribe the PUVA system (Ultra Violet A light treatments and psoralen medication), and although still appropriate and effective for some patients, new and promising technologies such as Protopic ointment, Elidel Cream, Narrow Band UVB light, pseudocatalase, pigment transplantation, excimer lasers, and other procedures, are showing equal or better success. More and more doctors are becoming aware of the great promise seen in new technologies being developed around the world to treat vitiligo.

Melanin, the pigment that determines color of skin, hair, and eyes, is produced in cells called melanocytes. If these cells die or cannot form melanin, the skin becomes lighter or completely white. Affected skin is particularly sensitive to sunlight.

2. Aetiological Hypothesis Various theories are suggested as the aetiology of vitiligo; the same mechanism may not apply to all cases. Autoimmune Hypothesis Autoimmune destruction of cutaneous melanocytes with total loss of melanocytes and melanin pigment in the skin of the affected area. Lymphocytic infiltration on skin biopsy indicate lymphocytes are involved in the destruction process. Frequently associated with other autoimmune diseases; such as alopecia areata, autoimmune thyroid disorders, Addison¡¦s adrenal disease, atrophic gastritis and pernicious anaemia, diabetes. Serum autoimmune antibodies against melanocyte, thyroid, adrenal, islet-cell, gastric parietal cell, and intrinsic factor have been demonstrated. Neurogenic Hypothesis A compound is released at peripheral nerve endings in the skin which is toxic to melanocytes and inhibits melanogenesis. It is postulated that in the dermatomal variant, the affected area shows sympathetic nerve dysfunction. Catechol neurotransmitters probably destroy the melanocytes instead. There is little support for this hypothesis. Self-destruct Theory of Lerner Defect of a natural protective mechanism in melanin synthesis within melanocytes, leading to accumulation of toxic precursors (phenolic compounds) which destroy melanocytes. This hypothesis is based on the lethal effects produced by chemical compounds (phenols) on functional melanocytes; the resulting leukoderma is indistinguishable from idiopathic vitiligo.

Vitiligo is more noticeable in darker skinned people because of the contrast. The cause of vitiligo is unknown, but autoimmunity may be a factor. This condition affects about 1% of the U.S. population.
    Lesions appear as flat depigmented areas with a darker boarder. The edges are sharply defined but irregular. Frequently affected areas are the face, elbows and knees, hands and feet, and genitalia. Symptoms Return to top
    • Family history of vitiligo Sudden or gradual onset of flat normally textured areas of skin with complete pigment loss
    Signs and tests Return to top Examination is usually sufficient to confirm the diagnosis. In some cases, a skin biopsy may be needed to rule out other causes of pigment loss.

This disease tends to involve periorificial areas such as around the eyes, ears, mouth, nose, genitals, and rectum. Extensor elbow, hands and feet are also common sites. Areas of post inflammatory hypopigmentation are the most common cause of decreased pigmentation but are usually easy to diagnose because of a preceding history of swelling, redness, or irritation.
