Guidelines for Documentation of Attention Deficit/Hyperactivity Disorder Documentation of Attention Deficit/Hyperactivity Disorder (AD/HD) must be provided by a clinician trained in the assessment of AD/HD and experienced in assessing the needs of adult learners, e.g., a licensed psychologist or psychiatrist. Documentation must be based on an evaluation conducted within three years of the studentÂ’s application for assistance in order to determine the current impact of this condition and appropriate accommodations. It should be typed on official letterhead, and include the professional credentials, address and phone/fax numbers of the evaluator. The evaluation must provide the following information: 1. Welcome to FDISK's ADD/ADHD page. For the uninitiated ADD is the abreviation for Attention Deficit Disorder, a poorly named disorder that affects roughly 5% of the American population. I believe ADD is an improper name because of the word "deficit." A better name would be Inconsistent Attention Disorderthe reason becomes apparent when you read the description. Dave sent over another unique name, "Actively, Divinely, Different." I like it, but I generally don't feel divine so I can't say it really describes me... ADD can be roughly described as "a neurological syndrome that is usually genetically transmitted [and can be] characterized by distractibility, impulsivity[,] restlessness" and the ability to hyperfocus. The "H" in ADHD stands for Hyperactivity which is often found in people with ADD. It is still becoming understood and is a topic of considerable debate. Some feel that the diagnosis of ADD or ADHD is a cop out or farce. They say that everyone has these symptoms and that we are giving people an excuse for poor behavior. What those people are missing, however, is that the average ADD person doesn't want an excuse. They just want to be able to function better. Other people feel that ADD is the yuppie flu of the 90'sThe age of technology at the speed of light has created a race of distracted, impatient people. Well, in 1975 I was labled by doctors with the term "hyperactive." ADD wasn't well known then, but they knew my behavior wasn't normal. I was the classic ADD case..all before it became the popular "catch all diagnosis" that disbelievers seem to think it is.

Doctor in the House Many students are easily distracted and have difficulty paying attention and staying on task. These students are often diagnosed with ADD, attention deficit disorder, or ADHD, attention deficit with hyper activity. The diagnosis is often made using parent and teacher surveys occasionally supplemented with a computer test. Based mainly on adultsÂ’ observation, these are effective means to determine the severity of the problems. The results of the surveys are commonly used to decide if a student should be placed on Ritalin or other medication. At its best, medication is usually recommended as only part of the treatment plan.     What is attention-deficit/hyperactivity disorder?
    Attention-deficit/hyperactivity disorder (ADHD) is an illness characterized by inattention, hyperactivity, and impulsivity. The most commonly diagnosed behavior disorder in young persons, ADHD affects an estimated three percent to five percent of school-age children. Although ADHD is usually diagnosed in childhood, it is not a disorder limited to children-ADHD often persists into adolescence and adulthood and is frequently not diagnosed until later years.
    What are the symptoms of ADHD?
    There are actually three different types of ADHD, each with different symptoms: predominantly inattentive predominantly hyperactive/impulsive , and combined Those with the predominantly inattentive type often:
    • fail to pay close attention to details or make careless mistakes in schoolwork, work, or other activities
    • have difficulty sustaining attention to tasks or leisure activities
    • do not seem to listen when spoken to directly
    • do not follow through on instructions and fail to finish schoolwork, chores, or duties in the workplace

    Attention Deficit Disorder (ADD) and Attention Deficit Hyperactivity Disorder (ADHD) are found in cultures around the world. They are true medical conditions Most societies consider ADD and ADHD as part of a life process and do not view them negatively. Our society has a perception of "normal" which does not include ADD and ADHD. The emphasis; therefore, is on treatment of ADD and ADHD. There seems to be a genetic component of ADD and ADHD. If one person in the family has the condition, there is a high likelihood that someone else also has it.
    Current research paints a portrait of ADHD as a frontal lobe dysfunction. PET and SPECT scans indicate excessive slow activity in central and frontal regions of the brain as well as decreased metabolism in the prefrontal region of the brain. The primary symptoms of ADHD
    • problems with sustained attention (inattention) poor impulse control (impulsivity) inappropriate activity levels (hyperactvity) depression
    The primary symptoms of ADD
    • lethargy lack of motivation daydreaming anxiety depression
    ADD and ADHD are usually treated with one or more of the following:
    • medication behavior modification EEG Neurofeedback
    Biofeedback (EEG Neurofeedback) Theory The brain produces waves that are detectable as electric current. There are five detectable brainwaves - Alpha, Beta, Theta, Gamma and Delta. Of these, Biofeedback generally addresses Alpha, Beta and Theta waves.
