Syndrome d'Adie
29 décembre 1998 Salutations J'ai visité votre site que j'ai trouvé très intéressant mais je n'ai pas trouvé ce que je cherchais. Selon le diagnostic de l'ophtalmologiste que j'ai consulté, j'ai la "chance" tout à fait exceptionnelle d'être atteinte d'une maladie rarissime des yeux qui se nomme le syndrome d'Adie, bilatérale de surcroit. Si rare que je n'arrive pas à trouver le moindre renseignement sur cette anomalie. Brève description: dilatation constante des deux pulpilles. Très difficile quand on demeure à la campagne, entouré de champs recouverts par la neige, particulièrement quand il fait soleil. Très difficile quand une partie de son travail exige une excellente vue. J'ai des yeux ajustés en permanence pour la nuit. Je vous serais très reconnaissante si vous pouviez m'indiquer une piste de recherche pour avoir plus de renseignements sur cette déficience visuelle car je n'ai encore rien trouvé de substensiel. Bonsoir Lyne, D'abord, je te souhaite une très heureuse année malgré les difficultés qui t'atteignent. Je ne peux te répondre directement, car mon ordinateur donne des renseignements confus. Pour ce qui est du syndrome d'Adie, je ne suis pas une experte, mais cela ressemble beaucoup à une forme d'aniridie (absence d'iris), ce qui crée une photophobie sérieuse (sensibilité à la lumière). Les gens atteints de ce problème doivent porter les lunettes de soleil pour protéger la rétine. Peut-être que tu pourrais chercher dans ce domaine pour trouver plus de renseignements. Bonjour Lyne, Après une petite recherche concernant le syndrome d'Adie, je t'adresse en fichiers joints deux définitions de ta pathologie. Elles sont toutes deux tirées de livres d'ophtalmologie donc à prendre avec le recul nécessaire. En effet, dans ce genre d'ouvrages, qui s'adressent à des professionnels, la description s'efforce d'être exhaustive et ne peut donc s'appliquer en l'état à un individu donné. Personne n'est mieux à même de parfaire cette approche que le médecin ophtalmologiste qui assure ton suivi médical. J'espère néanmoins que cela t'aidera un peu. Treatment of Accommodative Disorders
Accommodative Insufficiency Differential Diagnosis This disorder is usually of functional etiology, however it may occur with primary ocular disease, systemic or neurologic disorders. A thorough history should rule out any disease process responsible for the accommodative insufficiency before any treatment program is begun. DDx nonfunctional disorders (From Scheiman and Wick, Clinical Management of Binocular Vision, J.B. Lippencott Co. 1994.):
  • Drugs (alcohol, phenothiazides, antihistamines, cycloplegics, CNS stimulants, marijuana)
  • Iridocyclitis, glaucoma, trauma, Adie's syndrome, iris sphincter tear
  • Diabetes, multiple sclerosis, myotonic dystrophy, sinusitis, dental caries, posterior communicating artery aneurysm, Parkinsonism, Wilson's disease, midbrain lesions
  • Mumps, measles, scarlet fever, tonsillitis, lead and arsenic poisoning
  • Pineal tumor, Parinaud's syndrome, anterior poliomyelitis, fascicular nerve III lesion, herpes zoster, Horner's syndrome Management options include:
  • Spectacle correction Uncorrected refractive error may cause accommodative problems. Therefore, the first step is to correct any refractive error with spectacles, including small amounts of cylinder correction and hyperopia.

    An unusual cause of anisocoria - the antipodes of ocular autonomic dysfunction
    Frederick E. Lepore A 44-year-old woman presented with anisocoria, ptosis OD, and right-sided headache and neck pain. She was found to have Horner's syndrome OD, Adie's syndrome OS, and fibromuscular dysplasia. The diagnostic pitfalls associated with bilaterally abnormal pupils and carotidynia are explored.
