Imbalance of lung proteases and antiproteases probably plays a role in the development of emphysema . Individuals with a homozygous deficiency who also smoke have a greatly increased risk of developing severe panlobular emphysema before the age of 35. Such a homozygous deficiency accounts for less than 1% of COPD. Bronchiectasis , particularly in the lower lobes, is encountered in a few patients with a -1-antitrypsin deficiency ( Fig.1

The Cleveland Clinic Foundation There are many inherited metabolic diseases that may have a pathologic impact on the liver. In many cases the liver component of these diseases is only an epiphenomenon of a more generalized systemic disorder. Examples of such epiphenomena are glycogen and lipid storage diseases in which hepatomegaly is a manifestation of the underlying metabolic defect although the liver is not necessarily the major target organ. However, there are three genetically determined diseases in which the liver may be the principal target organ, with manifestations of acute, subacute, or chronic disease that may become evident in early or later life. These are hereditary hemochromatosis (HH) , a major disorder of iron overload; Wilson's disease , a genetic disorder of copper overload; and alpha

Policy
Aetna covers alpha 1-antitrypsin inhibitor therapy for selected patients with emphysema due to alpha 1-antitrypsin deficiency when ALL of the following criteria are met:
  • Patient is a non-smoker; and Patient has PiZZ, PiZ(null) or Pi(null,null) phenotype (homozygous) alpha 1-antitrypsin deficiency; and Patient has progressive emphysema with a documented rate of decline in FEV1.
  • Once initiated, therapy will usually be continued for the remainder of the patient's life. Recipients of alpha 1-antitrypsin inhibitor therapy should be immunized against hepatitis B. It is also recommended that this medication not be used in patients with immunoglobulin antibody IgA deficiency who are known to have antibodies against IgA (anti-IgA antibody). These patients may experience severe reactions, including anaphylaxis to IgA, which may be present in human alpha 1-antitrypsin inhibitor. Note: This coverage may be subject to precertification review in plans that include precertification requirements. Initial approval will be given for a six-month period upon evaluation of written documentation of pulmonary function tests (PFT) results. Dosing is limited to 60 mg/kg of patient body weight, administered intravenously every week. Prior to the re-approval for continued use at six-month intervals, written documentation of repeat PFT results is required to evaluate the maintenance or improvement of baseline function.

    ALPHA 1-ANTITRYPSIN DEFICIENCY - A specific enzyme (alpha 1 antitrypsinase) that when absent genetically can result in panacinar emphysema (lung disease) and liver disease. The incidence is approximate 1 in 10,000 people. There is no specific treatment for this condition other than supportive care for the liver and lung complications . Medications such as alpha-1proteinase inhibitor is given regularly to these patients.

    One of the most common inherited disorders among caucasians. Incidence is 1 per 2-7000 population. The phenotypic variations in AAT gene is expressed as the Protease inhibitor (Pi) alphabetical nomenclature. The Pizz variant most often characterizes the ATT deficiency state. Among Pizz infants with jaundice 25% will experience cirrhosis within the first decade of life, 25% will show persistent liver enzyme elevation progressing to cirrhosis in the second decade, 25% will resolve the biochemical change completely and show only mild fibrosis on biopsy. The liver injury in all these cases is probably attributed to release of proteases.
