An anal fissure is a small split in the skin of the anus (back passage). This may be caused by passage of a hard stool and is associated with raised internal anal sphincter pressure (muscle spasm of the back passage). What symptoms does an anal fissure cause?
An anal fissure causes pain on opening the bowels and there is often some blood on the toilet paper after wiping. Some people also notice itching around the anus. Anal fissure is often confused with piles (haemorrhoids), which are usually not painful. Is an anal fissure dangerous?
An anal fissure is painful and irritating but is not dangerous. As some other conditions cause pain and bleeding from the bottom, it is important that you visit your doctor for an examination to confirm the diagnosis. What tests are needed?

Anal Fissures and Pruritus Ani ("itchy butt")
Many patients present to our office with symptoms of severe itching around the anus. Others come thinking they have hemorrhoids because they have experienced bright red rectal bleeding. On physical exam, physicians often find an area that is very sore and irritated with very fine cracks in the skin. These cracks are called fissures. Some fissures are more external on the skin and readily visible. Other fissures are situated more deeply inside. Your physician will be able to determine at the exam which type of fissure you’ve had and what the appropriate treatment is. If yours are the ones that are more visible, then this handout is appropriate for you.
Many people feel that their itching is due to being "dirty down there". Many times, it is over-cleansing that causes the problem. After having a bowel movement, either wash with a washcloth and warm water or use Tucks Medicated Wipes. A less expensive method is to use baby wipes that do not contain alcohol. Scrubbing is not necessary. Just gentle cleansing.
It is important to use hypoallergenic soaps and shampoos. Many people with this condition have fair skin. They are very sensitive to perfumes and dyes. I like to recommend Aveeno, Oilatum, or Lever 2000 soaps. Anything that states that it’s hypoallergenic would probably suffice. For those who really have problems, they will also need to get a hypoallergenic shampoo. We use Envi.

What is an anal fissure?
An anal fissure is a small tear or break in the skin at the outer tip of the anal canal. The anus is the opening through which bowel movements pass. Anal fissures are a common disorder, particularly in persons with irregular or hard bowel movements.
How does it occur?
Tears in the anal skin occur during bowel movements when hard, dry stools are passed. They may also be associated with:
  • straining at bowel movements when you are constipated
  • stress or tension
  • waiting too long to have a bowel movement (BM)
  • constipation after surgery

If you have ever had a painful papercut on your finger, you understand all about anal fissures. The anus is the ring-like sphincter or valve at the end of your rectum. It opens and closes on your command to allow a bowel movement when convenient. An anal fissure is a small tear in the rim of the anus - just like a tiny papercut.
One problem with a papercut is that every time you use your hands, you tend to break open the small wound which delays healing causes more pain. The same holds true with an anal fissure. Every time you have a bowel movement, the anus is stretched and the fissure can possibly be reopened causing more symptoms. This makes some anal fissures chronic and difficult to heal. This problem is quite common and may cause considerable misery.
What causes an anal fissure?
The anal opening is only so big. To have a bowel movement the anal sphincter relaxes and open up as much as it can, but the stool must be still be soft enough to squeeze through the opening. If the stool is dry and hard, the anal sphincter is forced to open even wider than normal. This difficult passage can cause a tear in the rim of the anal opening, resulting in a fissure. Other causes of a fissure include severe diarrhea, inflammatory bowel disease, and sexually related trauma.
Classification of fissures
Fissures are graded as superficial or deep. A superficial fissure does not extend full thickness through the skin; a deep fissure results in exposing underlying muscle fiber. Fissures are also graded as acute or chronic. An acute fissure is less than thirty days duration; a chronic fissure is present for greater than thirty days.

An anal fissure is a tear in the internal lining of the anus. This area is called the anal mucosa. This break in the anal lining often reopens during bowel movements. The result is bright red blood and intense pain.
What is going on in the body?
An anal fissure is a tear in the bowel lining. It causes painful, bloody bowel movements. It is often confused with hemorrhoids, which are painful swellings at the anus caused by enlarged veins. A fissure can be acute or chronic. The chronic condition is often associated with a buildup of tissue at the external end of the tear. This is called a sentinel pile and may be noticed by an affected person. The internal end may also have a buildup of tissue that an affected person can rarely see.
When a fissure is present, the mucosa of the anus opens each time it is stretched to allow for a bowel movement. This continual opening prevents healing. It can also leave scar tissue. Most chronic fissures are in the center of the back of the anus.
The anal sphincter muscles need to relax for a person to have a bowel movement. A person with a fissure may have muscles that are too tight. This makes bowel movements painful.

An anaesthetic cream is sometimes prescribed if there is a lot of pain, but sensitivity to these preparations may occur. Chronic fissures (a fissure that has not healed in about 6 weeks) may require a more active intervention. Traditionally this involves surgery usually with some form of stretching of the anal sphincter. More recently, Glyceryl trinitrate spray (GTN) has been used with reasonable success.

In other words.....I had four surgeries very, very close to my "barking
spider". From someone who had to endure alot of PAIN to heal
the fistula, my advice would be to stop allowing your boyfriend to
use the "tradesman's entrance". His desire, or need, to
please himself this way can lead to alot of medical pain for you if
the anal fissures worsten, and there is no way that they will get
any better if this continues. My boyfriend also liked the "trademan's entrance" before my operations and I'm still not sure that this was the cause of a fistula, or absess, to form. The inside lining of your rectum can easily become infected by several causes. I'm not a doctor, so in lament terms, this is the only way I can express it. View Top Level Message Thread:

ANAL FISSURES From: Haines Ely One of the neatest letters to the editor I have seen in a long time was written in the NEJM by Stephen R. Gorfine M.D.titled Topical Nitroglycerine Therapy for Anal Fissures and Ulcers, NEJM 333(17) 1156-7, 1995. Anal sphincter hypertonicity is associated with painful anal fissures and ulcers. A derangement of nitric oxide regulation may cause this spasticity. Nitroglycerine was applied as 200mg in an ointment and the investigators asked the fourty patients treated to rate their pain on a scale of one to ten. The mean pain scores before and after treatment were 4.8 and 1.4. The patients were instructed to apply the ointment QID. Fissures healed in two weeks in 30% of the patients, in four weeks or less in 70% of patients. Side effects were limitted to mild transient headaches.
