      What is It?
        A carotid endarterectomy is a surgical procedure in which a doctor removes fatty deposits from one of the two main arteries in the neck supplying blood to the brain. Carotid artery problems become more common as people age. The disease process that causes the buildup of fat and other material on the artery walls is called atherosclerosis, popularly known as "hardening of the arteries." The fatty deposit is called plaque, the narrowing of the artery is called stenosis. The degree of stenosis is usually expressed as a percentage of the normal diameter of the opening.
        Why is the Surgery Performed?
          Carotid endarterectomies are performed to prevent stroke. Two large clinical trials supported by the National Institute of Neurological Disorders and Stroke (NINDS) have identified specific individuals for whom the surgery is highly beneficial when performed by surgeons and in institutions that can match the standards set in those studies. The surgery has been found highly beneficial for persons who have already had a stroke or experienced the warning signs of a stroke and have a severe stenosis of 70 percent to 99 percent. In this group, surgery reduces the estimated 2-year risk of stroke by more than 80 percent, from greater than 1 in 4 to less than 1 in 10. In a second trial, the procedure has also been found highly beneficial for persons who are symptom-free but have a severe stenosis of 60 percent to 99 percent. In this group, the surgery reduces the estimated 5-year risk of stroke by more than one-half, from about 1 in 10 to less than 1 in 20.

What is a brain aneurysm?
A brain aneurysm is a weak bulging spot on the wall of a brain artery very much like a thin balloon or weak spot on an inner tube. Aneurysms form silently from wear and tear on the arteries, and sometimes can form from injury, infection, or inherited tendency.

An aneurysm is the "ballooning" of an artery caused by the weakening of the arterial wall. Apart from occasional low back pain, an aneurysm in the abdominal aorta usually causes no symptoms. However, large aneurysms can burst without warning, causing internal bleeding and usually ending in death. Fortunately, aneurysms can be treated surgically before rupture occurs. Web posted at: 8:51 p.m. EST (0151 GMT) BOSTON (CNN) The best treatment for some brain aneurysms may be no treatment, according to a new study. Aneurysms are weak spots on blood vessels that balloon out from the vessel wall. If they break, they can cause a stroke. About 5 percent of Americans have had a brain aneurysm or will develop one. Researchers at the Mayo Clinic found that for a small aneurysm less than three-eighths of an inch wide the chance that it will break is less than one-twentieth of 1 percent a year. In contrast, the risk of death or brain damage from surgery to repair such an aneurysm is about 10 percent. However, if the aneurysm is larger or if a patient has already had an aneurysm that ruptured, the chance that the blood vessel will burst increases substantially, making surgery possibly worth the risk, the researchers concluded.

What Is an Abdominal Aortic Aneurysm?
An abdominal aortic aneurysm (AAA) is a bulge in the aorta (the aorta is the main blood vessel coming from the heart that supplies blood to all organs) in your abdomen. Aneurysms may occur in any blood vessel in the body, but the most common place is in the abdomen below the renal arteries (blood vessels that provide blood to your kidneys). The aneurysm may continue to grow larger until, like a balloon, it bursts ("ruptures"). The larger an aneurysm becomes, the easier it grows. Aneurysm rupture can be a life-threatening event. The goal of all aneurysm operations is to prevent the aorta from rupturing. A normal aorta below the renal arteries measures about 2.3 cm in diameter (1 inch) in men and 1.9 cm in diameter (3/4 inch) in women, but varies with age and body size. An aorta is considered aneurysmal when it grows more than 50% over its normal size. Aneurysms are four times more common in men than women and occur most often after 55-60 years of age.

H EART C ARE P ROGRAM AORTIC VALVE DISEASE
The aortic valve is located between the left Pumping chamber (ventricle) and the aorta. This valve allows blood to flow from the heart to the body and leaflets which open and close with inability to close or open are the two major problems with this valve. To compensate for aortic stenosis (restricted opening), the heart has to squeeze harder to support normal blood flow, causing the heart muscle to enlarge Eventually, as the stenosis increases, the heart begins to fail. Symptoms of aortic Stenosis are shortness of breath, fainting episodes due to a lack of blood flow to the brain, chest pain due to lack of blood flow to collapse due to irregular heart rhythms that occurs when the aortic valve does not completely close and blood flows back from the aorta into the heart. Aortic Valve Replacement Valves are replaced because they rarely can be repaired. Three types of replacement valves are available: mechanical, bioprosthetic and human Mechanical valves, which normally last a lifetime, are synthetic and made of plastic, cloth, and metal. The moving parts are coated to help prevent the formation of a blood clot on the valve However blood thinners (Coumadin) must be used. Stroke or bleeding problems may occur with mechanical valves, so strict follow-up with your local physician is needed to regulate your blood thinning medication. Bioprosthetic (animal) valves are sterilized for human use and may not require blood thinners. Stroke and bleeding problems rarely occur with this type of valve, however, they tend to wear out and need to be replaced in seven to 10 years. Human valves are obtained from donors after death. These valves are superior to mechanical and bioprosthetic valves as they are most like your natural valve. Blood thinners are not necessary. The longevity of these valves is 15 to 20 years.

Antibodies to Chlamydia pneumoniae are rarely found in children less than five years old; the primary infection usually occurs between ages 8 and 16. Reinfections occur throughout childhood, and the rate of infection is higher among men than women. In non-elderly people it causes mild respiratory symptoms, but may cause pneumonia in the elderly. Indications of the presence of Chlamydia pneumoniae have been found in the majority of studies which have looked for them. C pneumoniae was found in 11/25 aortas, 5/9 iliac arteries, 2/5 femoral arteries, and 1/2 iliac veins, by PCR in AAA specimens. It was also seen in vascular tissue from 3/6 "ostensibly normal" liver transplant donors, which showed some atherosclerotic changes. "The figures are perhaps conservative as it is possible that more specimens would be found to be C pneumoniae positive if a larger amount of tissue was examined."
