Etiology and Incidence
Although acute appendicitis is the most common disease of the appendix, other potential pathologic conditions affecting the appendix include swallowed foreign bodies, pinworms, fecaliths, carcinoids, cancer, villous adenomas, and diverticula. The appendix may also be involved in idiopathic ulcerative colitis or the ileocolitis of Crohn's disease. Except for hernia, acute appendicitis is the most common cause in the USA of an attack of severe, acute abdominal pain that requires abdominal operation. Because symptoms and signs vary widely and because delay before operation is so hazardous, it is accepted that nearly 15% of operations for acute appendicitis lead to other findings at laparotomy or even to findings of no disease.
Symptoms and Signs
Classic right lower quadrant tenderness is located at McBurney's point (junction of the middle and outer thirds of the line joining the umbilicus to the anterior superior spine). Rovsing's sign (pain felt in the right lower quadrant resulting from palpation of the left lower quadrant) suggests the possibility of appendicitis. The psoas sign (an increase in pain from passive extension of the right hip joint, which stretches the iliopsoas muscle) or adductor pain (pain produced by passive internal rotation of the flexed thigh) may suggest both the anatomic location of the appendix and progression of the inflammatory process.

Is it an Inflammation of the Appendix? The appendix is a thin, worm-shaped pouch that projects out from the first section of the large intestine. In some animals, the appendix plays an important role in the digestive process. In humans, the appendix is small (just under 4 inches long) and seems to have no function at all. In most cases of appendicitis, no cause is detected. But for reasons yet unknown, the appendix can become obstructed with a particle of undigested food, bowel material or other matter. The appendix will then become swollen, inflamed and filled with pus. If this is left unattended, the appendix will eventually burst, flooding the normally sterile abdominal cavity. Infection can then spread to surrounding organs, leading to blood poisoning, shock or death. IF YOU SUSPECT APPENDICITIS... don't give your child laxatives-they can stimulate the intestine and cause the appendix to rupture sooner. don't give pain medication to your child-the location and severity of the pain are important clues to a proper diagnosis.

Appendicitis is common, with a lifetime occurrence of 7 percent. Abdominal pain and anorexia are the predominant symptoms. The most important physical examination finding is right lower quadrant tenderness to palpation. A complete blood count and urinalysis are sometimes helpful in determining the diagnosis and supporting the presence or absence of appendicitis, while appendiceal computed tomographic scans and ultrasonography can be helpful in equivocal cases. Delay in diagnosing appendicitis increases the risk of perforation and complications. Complication and mortality rates are much higher in children and the elderly. (Am Fam Physician 1999;60:2027-34.) A ppendicitis is the most common acute surgical condition of the abdomen. Approximately 7 percent of the population will have appendicitis in their lifetime, with the peak incidence occurring between the ages of 10 and 30 years. Despite technologic advances, the diagnosis of appendicitis is still based primarily on the patient's history and the physical examination. Prompt diagnosis and surgical referral may reduce the risk of perforation and prevent complications.

Causes
It is possible for matter to become logged in the appendix, leading to bacterial infection, or appendicitis. If the condition is untreated, the appendix will eventually burst, leading to septicemia and eventually death.

Appendicitis symptoms
The appendix is a wormlike expansion of the large bowel (cecum) measuring one to three inches in length. It is located in the lower right portion of the abdomen at the beginning of the large intestine. Normally it is about as thick as a lead pencil and is pinkish gray in color. It serves no function in humans that we know of and is thought to be a residuum of our primitive past. When an acute inflammation is involved, severe pain will be felt in the lower right part of the abdomen. By this time the appendix has usually become filled with pus. If not treated right away the infection can spread through the wall of the appendix and can turn into gangrene and rupture. Because of this, appendicitis is considered an emergency situation. Causes: Appendicitis may be caused either by bacterial inflammation or by a viral infection occurring in the digestive tract. A bacterial infection can happen by a rigid particle of stool blocking its passageway causing pressure upon the blood vessels in the area. The inflammation can cause infection, a blood clot, or rupture of the appendix. Before the age of antibiotics came along, appendicitis was known as a common abdominal surgery but it occurs less frequently today. Most often it is seen in young adults in their twenties through forties, but can occur in infants and older people. Many years ago people died from appendicitis including small infants. Rarely does it occur in children under three years of age anymore. And for some unknown reason the condition occurs less often today than it did twenty years ago.

What is appendicitis?
Appendicitis is the most common cause of emergency surgery during childhood, occurring in about 80,000 children each year. It is more common in boys and is rare in infants under 1 year of age. Most cases occur between 6 and 20 years of age. The rate of perforation is high in children younger than age 4, due to their inability to describe symptoms and difficulty in examining them. What are the symptoms of appendicitis?
There is a wide spectrum of symptoms and not all children have all symptoms. The most common symptoms include:
  • Pain in the center and right side of the abdomen; this usually increases in severity as time passes, and becomes worse when moving, taking deep breaths, coughing, sneezing, and being touched in the area. Nausea and vomiting Lack of hunger Fever and chills Diarrhea or constipation Urinary tract symptoms such as frequency of urination and/or painful urination
The first symptoms of appendicitis are usually abdominal pain and not eating well. The pain usually begins in the center of the abdomen, around the navel. Later, it can move downward and to the right side, near the area of the appendix. The location of the pain is often difficult for preschool children to describe.

Emergency Medical Test for Appendicitis
Your child is doubled over in pain. Concern has brought you both to the Emergency Room, but after initial evaluation, the situation still isn't clear. Is this appendicitis or not? An exciting report in the September 15, 1999 Journal of the American Medical Association describes a new test that can answer the question without either the long ordeal of "let's wait and see" or having exploratory surgery. Its purpose is not known, but some researchers think it may help trigger the immune system to fight disease. Getting along without one is no problemwhich is a good thing, since the most common reason for abdominal surgery is removal of an inflamed appendix. What causes appendicitis?
The appendix is a "dead end." When something (such as a hard pellet of stool, a foreign body, a seed or other piece of undigested food) blocks the only opening from the colon, inflammation soon sets in. In other instances, infection from bacteria in the intestinal tract causes swelling that gradually closes the opening. There is no way to predict or prevent appendicitis. It can occur in anyone. However, it is most common in adolescents and young adults and rare in children under age 2. Is appendicitis serious?
When it's not treated, it can be very serious. A completely obstructed appendix becomes more and more inflamed and eventually bursts, casting out its infected contents. The entire lining of the abdominal cavity may become contaminated, causing a life-threatening condition called peritonitis.

    What is appendicitis?
    Appendicitis is an inflammation of the appendix. The appendix is a small, fingerlike tube located where the large and small intestines join. How does it occur? In most cases the inflammation may be caused by an obstruction of the opening of the appendix by a piece of stool, by a parasite, or by an infection. What are the symptoms? The symptoms of appendicitis include:
    • Abdominal pain, usually starting in the middle of the abdomen around the umbilicus (belly button), then shifting to the right lower side of the abdomen. Nausea. Vomiting. Pain when touched on the right side of the abdomen. Fever, usually not very high.
    How is it diagnosed? Sometimes it is difficult to diagnose appendicitis. Your doctor will review your symptoms and examine you. The doctor will probably ask you to give a urine sample. The doctor may order the following lab tests:
    • Complete blood count (CBC) to determine your white blood cell count.
