Grossman, MD Abstract Key words: Arthrogryposis multiplex congenita Introduction Arthrogryposis refers to a joint that is in a fixed position. Arthrogryposis multiplex congenita is diagnosed when two or more joints in more than one limb are fixed from birth ( Fig 1 ). A joint becomes fixed in a given position because of unevenly impaired joint motility. Unevenly impaired joint motility results from segmental muscle weakness or hypotonia, or from a sustained asymmetrical posture. Segmental muscle weakness or hypotonia is due to an imbalance between agonistic and antagonistic muscles. Sustained asymmetrical posture results from movement constraints due to reduced uterine volume or when thick skin bands prevent joint movement. Arthrogryposis multiplex congenita (AMC) may be caused by neurological and non-neurological causes. Non-neurological causes of AMC are cartilaginous abnormalities and physical constraint to movement. Neurological causes of AMC include central and peripheral nervous systems abnormalities ( Table 1 Cartilaginous Abnormalities Cartilaginous abnormalities should be consider the cause of AMC in neonates that are longer that expected for their gestational age, and those with hyperextensible and transparent skin, blue sclera, abnormal ear lobe shape and craniosynostosis.

Arthrogryposis Program
What causes arthrogryposis? There are many possible causes for arthrogryposis multiplex congenita (AMC), but no specific known cause. Generally, problems with joint growth and development, decreased fetal movement, and spinal development during the first 3 months of pregnancy may lead to the development of AMC. AMC is not an inherited disorder, so a person with AMC will not pass it on to his children. Distal arthrogryposis, which is rarer than AMC, is inherited, and can be passed from parent to child.
Is there a cure for arthrogryposis multiplex congenita (AMC)? Currently, there is not a cure for arthrogryposis, but there are several treatments that can help children who have arthrogryposis live fuller lives. Splinting and bracing can help improve range of motion. Surgical intervention can improve joint position. Parents often ask about muscle transplants; however, these are not currently a valid treatment option. Muscle transfers may be used to provide power to a flexible joint, such as creating elbow flexion by the transfer of triceps to create "biceps" in one arm. Each individual must be evaluated for total body function before these transfers are considered.

*Faculty of Agriculture, University College Dublin, Belfield, Dublin 4, Ireland Arthrogryposis (crooked joints) is a congenital malformation that involves the posture and function of the limbs in newborn animals and humans. Arthrogryposis is characterised by degrees of flexion rigidity of a range of joints including the phalangeal, metacarpophalangeal, carpal and metatarsophalangeal joints. The condition has been reported in human infants, calves, pigs, foals and sheep. A variety of aetiological agents including intrauterine infection with Border Disease Virus Akabane virus or Cache Valley Virus have been implicated in the pathogenesis of arthrogryposis in sheep. Evidence for inherited arthrogryposis is rare in sheep, although it has been reported in Australian Merino sheep. We used embryo transfer to test the hypothesis that arthrogryposis is an inherited disorder in a population of pedigree Suffolk lambs in which the in-flock incidence rate of arthrogryposis of the forelimbs was 58 per cent. The problem was initially identified in a flock of 35 pedigree Suffolk ewes. Fifty two live lambs were born to the 35 breeding ewes. Thirty lambs were born with a forelimb arthrogryposis characterised by bilateral flexion rigidity of the metacarpophalangeal and carpal joints. (Figure 1). A controlled breeding study was undertaken to test the hypothesis that the arthrogryposis was an inherited disorder associated with the introduction of the ram to the flock.

Landsforeningen for Arthrogryposis Multiplex Congenita (AMC) ACM er et syndrom, dvs an sammling med symptomer. Symptomene består av bevegelsesinnskrenkninger (kontrakter) i flere ledd og slappe lammelser i en eller flere av musklene som beveger det aktuelle leddet. AMC' hovedoppgave er å stimulere til økt oppmerksomhet rundt diagnosen. Dette er knyttet opp mot helsevesenet. AMC arbeider for at den enkelte med AMC-diagnose og dennes familie skal få vite mest mulig om diagnosen - medisinsk og behandlingsmessig, samt de trygde- og sosialrettigheter den enkelte har. AMC er en av 5 små diagnosegrupper som deltar i Trenings- og Rådgivningssentret ved Sunnaas sykehus. TRS-prosjektet er en del av Regjeringens handlingsplan for funksjonshemmede. AMC ble stiftet 1987.
