Balanitis
A Disease of the Uncircumcised Male
Balanitis (or Balanoposthitis) . This is inflammation of the glans penis (the tip of the penis) and male foreskin. It rarely, but can occur, in the female clitoris. It has been shown that uncircumcised males have a higher incidence of penile inflammatory disorders and that 5-10% of all uncircumcised boys eventually undergo circumcision because of the complications of balanitis. Careful hygiene and cleansing of uncircumcised boys may reduce this infection and its complications, which is often thought to be caused by the accumulation of smegma . Routine circumcision completely eliminates these problems. Alternative name: balanoposthitis
Causes, incidence, and risk factors:
Balanitis is usually caused by poor hygiene in uncircumcised men. However, exposure to caustic soaps or failure to properly rinse the soap while bathing may result in inflammation.
Prevention:
The foreskin should be retracted daily to adequately clean and dry the area under the foreskin. Alternatively, circumcision completely eliminates this problem.
Symptoms:
*redness of foreskin or penis
*foul-smelling discharge
*painful penis and foreskin
Treatment:
In cases of inflammation that is causing problems with urination, an immediate circumcision may be recommended. Several topical ointments are also available and may provide temporary relief of this problem. However, reoccurring balantis should be treated with a complete circumcision

Information Balanitis Basic Information Balanitis is an inflammation of the tissues at the head of the penis caused by a bacteria or yeast infection. It is the most frequent cause of penile swelling. It is seen almost exclusively in uncircumcised men whose foreskin as well as penile head may be affected. It is usually caused by poor or improper hygiene. Yet there can be a number of other causes including:
    • trauma from pulling allergies to medications, creams, lotions catheter use irritation from dyes in clothing such as colored underwear irritation from latex condoms and/or nonoxynol-9 spermicide irritation from tight, ill-fitting condoms urine, if allowed to remain on the penis and/or to collect under the foreskin, can macerate this area tears on the penis
    Also improper hygiene can lead to an overgrowth of yeast, a secondary infection sometimes associated with balanitis. In fact it is thought that at times balanitis can be transmitted by sexual contact with a partner who has a yeast infection. This is a disease that affects men only of all ages though the incidence of infection in the U.S. is not really known. What is known is that men who are sexually active, uncircumcised, are diabetic or who have improper or poor hygiene are at the most risk for getting balanitis.

    It is extremely common and is generally not serious though it can cause a great deal of concern to men and their partners.
    Where does it come from?
    There are a number of possible causes; Poor hygiene. The foreskin should be withdrawn and then the area can be gently washed. This should be done daily to stop matter building up which can cause inflammation. Over keen hygiene. Washing too hard under the foreskin or too frequently causes irritation. Dermatitis is when the sensitive skin of the glans and foreskin becomes inflamed by the use of chemicals/products such as; - perfumed soaps and cosmetics - washing powders (biological) and fabric softeners - chemicals on hands transferred to penis when touching it - condoms or their spermicidal lubricant - sweat/urine collecting under the foreskin Infection. Thrush (candida) is the most common cause here. This is a yeast germ that women often have though it is not easily passed on from women to men. The yeast germ however can collect under the foreskin.     What causes balanitis?
    The most common causes are:
    • irritants: neglected hygiene and tight foreskin (particularly in young boys) lead to irritation by smegma (a cheesy substance secreted under the foreskin).
      seborrhoeic dermatitis: an inflammatory rash of uncertain cause that is commonly seen on the scalp, behind the ears and in the skin folds.

    BALANITIS CIRCUMSCRIPTA PLASMACELLULARIS (BALANITIS OF ZOON) The erythematous, velvety appearance of this lesion of the glans penis clinically resembles erythroplasia of Queyrat (bowenoid squamous cell carcinoma in situ of the glans penis). The lack of atypia plus the combined features of peculiar epidermal changes and plasma cell infiltrates enable one to make the distinction histologically. The keratinocytes appear to be shrunken and develop fusiform as well as angulated shapes. They are rather evenly separated by spaces. The suface cells are often very flattened, and the epidermis may be exceedingly thin or focally eroded.
    Low power view. Most of the inflammatory cells beneath the epidermis are plasma cells. High power view from the area of the black rectangle. Some of the cells that are migrating into the epidermis are lymphocytes, and a few are PMN's. Plasma cells are prominent at and beneath the dermoepidermal junction. High power view from the area of the red rectangle. PMN's, lymphocytes, and plasma cells are at the dermoepidermal junction. The PAS stain did not demonstrate fungi. Pigment incontinence is present.
