WHAT YOU SHOULD KNOW
Balanitis is an inflammation of the head of the penis. A form of the disease called balanoposthitis typically occurs in uncircumcised infants and boys.
Causes
In adult males, a sexually transmitted disease is often the culprit. In uncircumcised boys, the problem is typically the result of an infection brought on by poor hygiene. Especially if the foreskin is tight, secretions can accumulate under it, providing an environment in which germs can flourish. Laundry detergent from clothing can cause further irritation and swelling of the area.
Signs/Symptoms
The hallmarks of this ailment are soreness and swelling of the head of the penis. Swelling may draw the foreskin tight, and pus may form underneath it. Erections and urination are likely to be painful.
Care
To prevent this problem, uncircumcised boys should be taught to pull back the foreskin and wash the area thoroughly several times a week. If an infection develops, the doctor may prescribe an oral antibiotic or an antibacterial cream. Surgery to remove the foreskin (circumcision) may be required in some cases to prevent further attacks.

Balanitis is usually caused by poor hygiene in uncircumcised men. Bacterial, fungal, viral, or non-infectious inflammation may occur. Exposure to caustic soaps or failure to properly rinse soap off while bathing can also result in inflammation. Several other diseases, including Reiter's syndrome and lichen sclerosis et atrophicus can also produce balanitis.

Definition
Balanitis is an inflammation of the head and foreskin of the penis. Description Balanitis generally affects uncircumcised males. These are men who have a foreskin, which is the "hood" of soft skin that partially covers the head of the penis. In balanitis, the head and foreskin become red and inflamed. (In circumcised men, who lack a foreskin, these symptoms only affect the tip of the penis.) The condition often occurs due to the fungus Candida albicans , the same organism that causes vaginal yeast infections in women. Balanitis (which is also referred to as balanoposthitis) can be caused by a variety of other fungal or bacterial infections, or may occur due to a sensitivity reaction to common chemical agents. Uncircumcised men are more at risk for balanitis due to the presence of the foreskin. The snug fit of the foreskin around the top of the penis tends to create a damp, warm environment that encourages the growth of microorganisms. Most of the organisms associated with balanitis are already present on the penis, but in very small numbers. However, if the area between the head and foreskin is not cleansed thoroughly on a regular basis, these organisms can multiply and lead to infection. Diabetes can increase the risk of developing the condition.

Methods: The medical literature was searched from 1983-1998 using key words balanitis, lichen, and sclerosis using the MEDLINE system.
Results and Conclusions: Balanitis xerotica obliterans can be distinguished from other genital dermatoses with similar characteristics through patient history, clinical findings, and laboratory evaluation. Tzanck smear and cutaneous biopsy, along with a rapid protein reagin test, will provide a definitive diagnosis. Treatment with high-dose topical corticosteroids relieves symptoms, and therapy focuses on prevention of disease progression. Introduction Balanitis xerotica obliterans is a subcategory of lichen sclerosus et atrophicus limited to the male genitalia.

  • Phimosis is a tight preputial ring often made of scar tissue preventing retraction of the foreskin. It may be primary, or secondary to recurrent infection
  • Phimosis may produce urinary obstruction with ballooning of the foreskin
  • In adults phimosis can lead to chronic inflammation and carcinoma
  • This is different to a simple non-retractile foreskin , a normal finding in many boys
  • Occasionally the two can be hard to differentiate and phimosis may only be diagnosed in retrospect when there is no improvement
Management
  • Boys aged under 7 with an asymptomatic non-retractile foreskin require no treatment and may be followed up annually
  • Parents may be reassured that the condition nearly always resolves over time
  • Referral may be considered if there are symptoms or parental concerns
  • An obvious true phimosis requires a referral
Balanitis/ Balanoposthitis (Infection of the glans/glans and foreskin)
  • Infection may be bacterial or fungal (the latter is more common in diabetics)
  • Infections are sometimes recurrent
Management
  • Each episode should be treated with local hygiene and antibiotics
  • Oral antibiotics may be required if there is systemic illness
  • Proven fungal infection should be treated appropriately
  • In recurrent infection advice should be given about genital hygiene and the use of soaps and bubble baths which can exacerbate this condition

Symptoms:

Signs and tests: Your dermatologist or urologist may be able to diagnosis the cause of your balanitis by examination alone. Additional tests, such as skin culture for viruses, fungi, or bacteria, are often needed, however. Occasionally, a skin biopsy is required.
Treatment: Treatment depends on the cause of the balanitis. For example, infectious balanitis may be treated with topical or oral antibiotics. Some balanitis which occurs in association with skin diseases may respond to topical steroid creams. In severe cases, circumcision may be the best option.
Expectations (prognosis): Most cases of balanitis can be controlled with topical medication and good hygiene. Surgery is not usually necessary. Outcomes are nearly always positive.
