Finding a lump in your breast can be scary, but breast lumps are a normal condition that affects more than 60 percent of all women at some point in their lives. 80-85 percent of breast lumps that are biopsies turn out to be benign (non-cancerous) especially in women under 50 years old. Generalized breast lumpiness
Generalized breast lumpiness is known by several names including fibrocystic breast disease, fibrocystic breasts and benign breast disease. Technically not a disease, these terms are used to describe a wide range of benign breast conditions including generalized breast changes, lumps, cysts, nipple discharge, infection and inflammation. A cyst is a fluid filled sac. Benign breast lumps are symptoms and exam findings of fibrocystic breast changes include:

They often enlarge and become tender and painful just before the menstrual period and may seem to appear overnight. Cysts are rarely malignant and may be caused by a blockage of breast glands. Cysts can feel either soft or hard. When close to the surface of the breast, cysts can feel like a large blister, smooth on the outside, but fluid-filled on the inside. However, when they are deeply imbedded in breast tissue, a cyst will feel like a hard lump because is covered with tissue. How are cysts diagnosed and treated?

Benign Breast Conditions Format for Printing A benign breast condition is any non-cancerous breast abnormality. According to the American Cancer Society, when breast tissue is examined under a microscope some type of abnormality is common in nine out of every ten women. Though not life-threatening, benign conditions may cause pain or discomfort for some patients. Some (not all) benign conditions can signal an increased risk for breast cancer. Depending on the type of benign breast condition and the patient's medical situation, treatment may or may not be necessary. These changes can include an increase in the number of breast cells (hyperplasia) or the emergence of atypical breast cells (atypical hyperplasia). That is why physicians carefully monitor patients with abnormal breast cells, to ensure that if cancer develops at a later date, it is detected and treated early. Department of Surgery, Peterborough District Hospital, Thorpe Road, Peterborough, UK A randomized study was performed on patients in whom the clinical decision had already been made to excise a breast lump. The objective was to assess the difference in diagnostic results of 21 and 23 G needles in the fine needle aspiration cytology (FNAC) of breast lumps. Following induction of anaesthetic, (local or general), FNAC was performed with either a 21 or 23 G needle. The breast lump was then excised and the histology and cytology results analysed routinely. One hundred and twenty-five excised breast lumps were included. Sixty-one had FNAC performed with a 21 G needle and 64 had FNAC performed with a 23 G needle. Of the 61 21 G FNAC, histology revealed 45 breast cancers. Of the 64 23 G FNAC, 50 patients had breast cancer. There was no statistical difference between these two results. There is no difference in the cytological yield when a 21G needle is compared with that of a 23 G needle. Fine needle aspiration cytology (FNAC) of breast lumps is an important part of the triple assessment of the palpable breast lump (clinical examination, imaging, [mammography or ultrasound], and FNAC). It has been shown that FNAC can reduce the number of open breast biopsies.

There are many types of benign breast lumps. The commonest types are as follows:
  • Breast cyst
    Breast cysts are sacs filled with fluid within the breast. They are the commonest benign breast lumps in women aged between 35 and 55 years. Usually, they can be diagnosed and treated easily. A fine needle is put into the cyst and the fluid inside is drained out. Fibroadenoma
    This is a solid lump of hard, fibrous tissue. Fibroadenomas are the commonest benign breast lumps in women younger than 20 to 30 years. In general, if a fibroadenoma were bigger than 4cm across, most doctors would suggest that it should be surgically removed. Smaller lumps in women under 30 years do not have to be removed as long as the diagnosis is certain. However, some women prefer to have them removed to relieve their anxiety. Smaller lumps in women over 30 probably should be removed to avoid the risk of missing a cancer.

Before your menstrual period begins, and sometimes during your period, you can have some tenderness, pain, or lumps in your breasts. This could be due to the extra fluid that collects in the breast tissue (cyst) as well as changes in the body's hormonal levels. This is normal. But if the lumps do not disappear before your next period, contact your doctor as soon as possible. Any noticeable change, thickening or localized swelling in your breast that was not there before may be a lump. Eighty-five percent of all breast lumps found are benign. Some common benign breast problems that appear as lumps are:
  • Cystic Disease (fibrocystic breast disease) is the most common cause of breast lumps in women. These lumps or cysts are fluid-filled sacs that enlarge and become tender and painful before the menstrual cycle. This condition is responsible for at least half of all breast biopsies performed. It tends to involve both breasts. These lumps are movable, and if large may feel round and firm. Cystic disease usually disappears after menopause.

Considerations Return to top Normal breast tissue, including glands and connective tissue, is present in both males and females of all ages. This tissue is responsive to hormonal changes. For this reason, a breast lump has different meanings in different situations. Since some lumps and some changes are considered normal, understanding the structure of the breast and how it functions may help eliminate some concerns.
  • The first lump that concerns parents is often the lump found beneath the nipple of the newborn infant. This is true breast tissue that is enlarged in response to estrogens that the baby receives from the mother during pregnancy. It resolves on its own as the estrogen effects wear off. The breast enlargement may be found in both male and female infants and is not only normal, but also expected. The next most worrisome lump is that found in girls, often as young as 6 years old. A "breast bud" generally heralds the onset of puberty. They most often appear around 9 years of age. Often only one breast is involved or if both are involved, one side is larger than the other. The breast may be somewhat tender. This is normal development and should not be cause for worry.
