About Benign Prostatic Hyperplasia What is benign prostatic hyperplasia (BPH)? Benign prostatic hyperplasia (BPH) is one of the most common diseases affecting the prostate gland. BPH occurs when there is an increase in the number of the smaller glands and stroma of the prostate gland, which causes the prostate to become enlarged. BPH is a type of tumour that is benign or non-cancerous. It is the most common benign tumour in men, and it becomes much more common as men get older. One study has shown that BPH is present in 20% of men aged 41-50 years, 50% of men aged 51-60 years, and over 90% of men older than 80 years , although the proportion of men with symptoms was much less. BPH is not a life-threatening condition, but if left untreated for a prolonged period of time, it has the potential to cause problems with the function of the bladder, and less commonly, the kidneys.

The physical examination includes a digital rectal examination (DRE), and symptom evaluation is obtained from the results of the AUA Symptom Index. Digital rectal examination (DRE) DRE is a somewhat uncomfortable procedure that typically takes less than a minute to perform. The doctor inserts a lubricated, gloved finger into the patient’s rectum to feel the surface of the prostate gland through the rectal wall to assess its size, shape, and consistency. Healthy prostate tissue is soft, like the fleshy tissue of the hand where the thumb joins the palm. Malignant tissue is firm, hard, and often asymmetrical or stony, like the bridge of the nose. If the examination reveals the presence of unhealthy tissue, additional tests are performed to determine the nature of the abnormality. AUA Symptom Index The AUA (American Urological Association) Symptom Index is a questionnaire designed to determine the seriousness of a man's urinary problems and to help diagnose BPH. The patient answers seven questions related to common symptoms of benign prostatic hyperplasia. How frequently the patient experiences each symptom is rated on a scale of 1 to 5. These numbers added together provide a score that is used to evaluate the condition. An AUA score of to 7 means the condition is mild; 8 to 19, moderate; and 20 to 35, severe.

If symptoms do not threaten the man’s health, he may choose not to be treated. If symptoms are severe enough to cause discomfort, interfere with daily activities, or threaten health, treatment is usually recommended. Watchful waiting Men with mild symptoms may choose to return for annual examinations. The physician will perform an examination that includes a DRE, PSA tests, and a urinary flow rate. The patient will be asked to describe symptoms in order to determine if the condition is worsening. Medication 5-Alpha reductase inhibitors Side effects include reduced libido, impotence, breast tenderness and enlargement, and reduced sperm count. Long-term risks and benefits have not been studied. Women who may be pregnant must avoid handling dutasteride capsules and broken or crushed finasteride tablets because exposure to the drugs may cause serious side effects to the fetus. Intact tablets are coated to prevent absorption through the skin during normal handling. It is therefore not life threatening. It is described as "hyperplasia" because the number of cells that make up the gland increase, causing the prostate to enlarge.
What is the prostate gland?
It is a small gland located at the base of the urinary bladder , the size and shape of a walnut, weighing about 20g (0.7 ounces). It produces fluid for the nourishment of sperm. The tube or passage that conveys urine from the urinary bladder to the penis (the urethra ) passes through the gland, making it vulnerable to anything that causes the gland to enlarge. The internal urethral sphincter is a muscle at the base of the bladder . It closes off the urethral opening into the bladder during the expulsion of semen ( orgasm or ejaculation ) and when it is not convenient to pass urine . This sphincter is important because it is very near the prostate gland , and anything affecting the gland may affect the sphincter and therefore the ability to control urine and semen flow.

WHAT IS BENIGN PROSTATIC HYPERPLASIA?
The Urinary Tract and Urination
The process of urination begins when waste fluids flow out of the kidneys into two long tubes called ureters. The ureters empty into the bladder, which rests on top of the pelvic floor, a muscular structure similar to a sling running between the pubic bone and the base of the spine. As the bladder fills to its capacity of 8 to 16 oz of fluid, its nerves send signals of fullness to the spinal cord and the brain. The brain regulates the muscles in the urinary tract partly by means of a pathway of nerve cells and neurotransmitters (chemical messengers) called the cholinergic system. While the bladder is filling, the brain signals the bladder to relax. As the bladder swells, the sensation becomes conscious and the conscious aspect of the brain voluntarily contracts the muscles to prevent urination. At the time of urination, the spinal cord initiates the voiding reflex, an automatic process that contracts the muscles surrounding the bladder, called the detrusor muscles , and relaxes the internal sphincter , a strong muscle encircling the bladder neck. When the internal sphincter is open, urine flows out of the bladder into the urethra, the tube that carries urine from the bladder out through the penis. These muscles are involuntary; that is, they require no conscious effort for contraction or relaxation. The prostate gland is located between the bladder and the rectum and wraps around the

Benign prostatic hyperplasia occurs later in adult life, starting around the age of 40, but commonly occurring between the ages of 50 and 70. Due to bulging of the gland centrally into the urethra and the base of the bladder, the symptoms result from prostatic urethral obstruction. Often the patient ignores or tolerates his symptoms for a considerable length of time and may present in acute retention or even renal failure. Examination of patients with symptoms of bladder outflow obstruction must include a rectal assessment of the prostate. In benign hyperplasia, the gland is usually uniformly enlarged with a smooth surface. Normal examination of the prostate gland does not exclude malignant disease. Dipstick analysis may detect blood and/or protein but this is a non-specific finding. Culture or microbiological examination may detect infection. Patients with troublesome symptoms should be referred to specialist units (Urology) for further investigations, including imaging and flow studies.

Principal Investigator: Kathryn Hirst, Ph.D. enign prostatic hyperplasia is a common condition in older men characterized by voiding symptoms and prostate abnormalities. Treatment ranges from `watchful waiting' to invasive surgical therapy with potentially serious side effects. The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) has sponsored a collaborative group of seven clinical centers and a coordinating center to investigate the efficacy and safety of surgeries considered minimally invasive to treat benign prostatic hyperplasia (BPH). Minimally invasive surgical therapies use heat effects to alter or destroy prostatic tissue in situ. Basic approaches include laser, microwave, or ultrasound energy to deliver heat to the target area. In its first year, the collaborative group is designing and developing a protocol to conduct a clinical trial. The Biostatistics Center serves as the coordinating center for the collaborative group.

This is when (especially older men) have difficulty to pee, i.e. the urethra has narrowed and causes an obstruction to a normal, smooth, strong flow of urine. With slow and cumbersome flow of urine the toxicity in the urine penetrates directly into the prostate gland causing infection over a period of time. This in turn causes the prostate to enlarge, further slowing down the flow of urine. BPH Tissue occurs in 50 - 75% males over the age of 50 years. Introduction Benign prostatic hypertrophy (BPH) is characterized by enlargement of the prostate gland in males that begins around age 40. As the gland enlarges, it can cause compression of the urethra and surrounding tissues, and thereby cause symptoms of urinary obstruction.
BPH is a nearly universal condition of older men affecting at least half of men over the age of 50. This number increases to 80% of men in their 70s. The cause is not well understood. It is not possible to determine which men will be affected with the condition and there are no genetic or other markers for assessment.
There is no relationship whatsoever between BPH and prostate cancer. BPH can, however, cause a significant amount of inconvenience, discomfort and medical morbidity for some men.
The presence of other potential causes of urinary symptoms and the complication of urinary retention must be excluded. Once this is accomplished, the management of bothersome symptoms and prevention of complications become the focus of care. The extent of treatment is usually dictated by the man's level of inconvenience and his desire to pursue the various interventions.
There are three approaches to the management of BPH: "watchful waiting", medication, and surgery.
