Facts About Childhood-Onset Bipolar Disorder What is childhood-onset bipolar disorder (COBPD), and how does it differ from bipolar disorder (manic-depression) in adults? All those with bipolar disorder experience mood swings that alternate from periods of severe highs (mania) to severe lows (depression). However, while these abnormally intense moods usually last for weeks or months in adults with the illness, children with bipolar disorder can experience such rapid mood swings that they commonly cycle many times within a day. The most typical pattern of cycling among those with COBPD, called ultra-ultra rapid or ultradian, is most often associated with low arousal states in the mornings followed by increases in energy towards late afternoon or evening. It is not uncommon for the initial episode of COBPD to present itself as major depression. But as clinical investigators have followed the course of the disorder in children, they have observed a significant rate of transition from depression into bipolar mood states.     (3 pages, 110KB) Research findings, clinical experience, and family accounts provide substantial evidence that bipolar disorder, also called manic-depressive illness, can occur in children and adolescents. Bipolar disorder is difficult to recognize and diagnose in youth, however, because it does not fit precisely the symptom criteria established for adults, and because its symptoms can resemble or co-occur with those of other common childhood-onset mental disorders. In addition, symptoms of bipolar disorder may be initially mistaken for normal emotions and behaviors of children and adolescents. But unlike normal mood changes, bipolar disorder significantly impairs functioning in school, with peers, and at home with family. Better understanding of the diagnosis and treatment of bipolar disorder in youth is urgently needed. In pursuit of this goal, the National Institute of Mental Health (NIMH) is conducting and supporting research on child and adolescent bipolar disorder.
    A Cautionary Note
    Effective treatment depends on appropriate diagnosis of bipolar disorder in children and adolescents. There is some evidence that using antidepressant medication to treat depression in a person who has bipolar disorder may induce manic symptoms if it is taken without a mood stabilizer. In addition, using stimulant medications to treat attention deficit hyperactivity disorder (ADHD) or ADHD-like symptoms in a child with bipolar disorder may worsen manic symptoms. While it can be hard to determine which young patients will become manic, there is a greater likelihood among children and adolescents who have a family history of bipolar disorder. If manic symptoms develop or markedly worsen during antidepressant or stimulant use, a physician should be consulted immediately, and diagnosis and treatment for bipolar disorder should be considered.

    Introduction: Why Bipolar Disorder is a problem.
    Bipolar Disorder in children and adolescents has long been an overlooked health problem, and the subject of much professional debate. While it is fairly well known to the general public that "manic-depressive illness" is fairly common, affecting up to 1.0 to 1.5 % of the adult population at some point in life, it is not commonly recognized that bipolar illness in childhood and adolescence is also a major health problem: adolescents run a similar rate of risk as do adults, and a significant number more also report notable and disturbing subclinical levels of mood problems similar in quality to bipolar disorder. Bipolar disorder in children and adolescents is arguably more significant of an issue than it is in adults. For example, if it is assumed that 1% of children and adolescents at any given time suffer from bipolar disorder, this naturally occurs during critical phases in child development, and not only can show itself with behaviors and feelings not typically viewed as part of a "depression" or "mania" but can interfere with the normal developmental processes of childhood. Add to this the diagnostic difficulties in distinguishing between disorders that have similar characteristics in children (such as delinquency, anxiety disorders, psychosis, ADHD, and learning disorders) and there is a significant number of children and adolescents both correctly diagnosed and misdiagnosed or undiagnosed.

    Bipolar Disorder Bipolar disorder (also known as manic-depression) is a serious but treatable medical illness. It is a disorder of the brain marked by extreme changes in mood, energy, and behavior. Symptoms may be present since infancy or early childhood, or may suddenly emerge in adolescence or adulthood. Until recently, a diagnosis of the disorder was rarely made in childhood. Doctors can now recognize and treat bipolar disorder in young children. Early intervention and treatment offer the best chance for children with emerging bipolar disorder to achieve stability, gain the best possible level of wellness, and grow up to enjoy their gifts and build upon their strengths. Proper treatment can minimize the adverse effects of the illness on their lives and the lives of those who love them. Families of affected children and adolescents are almost always baffled by early-onset bipolar disorder and are desperate for information and support.     By Karen Dineen Wagner, M.D., Ph.D. Bipolar disorder (manic depression) can have its onset in childhood. Symptoms in young children can present differently than in adults and are frequently mistaken for those of attention deficit/hyperactivity disorder (ADHD); therefore, the bipolar diagnosis is often missed. Children tend to have less discreet episodes of mania and depression than adolescents or adults. Often these children are irritable, explosive and excessively active. Some children show more typical features of mania, including grandiosity, excessive talking, high energy, difficulty sleeping and impulsive behavior such as precocious sexuality. Teens with bipolar disorder are more likely than younger children to show distinct episodes of mania and depression. Their grandiosity and impulsivity during mania may result in social and academic problems as well as substance abuse. Some diagnosed with conduct disorder or substance abuse may have unrecognized, underlying bipolar disorder. Similarly, some adolescents diagnosed with schizophrenia because of psychotic thoughts may have bipolar disorder with psychotic features. A psychiatrist must thoroughly evaluate a child or adolescent to make a diagnosis of bipolar disorder. The evaluation generally includes interviewing the child, a physical examination, blood tests to ensure no underlying medical problem exists, and review of school records. Parents can further describe the problems and medical and family history. Often, a family history of depression or bipolar disorder can be a clue to diagnosis. Bipolar disorder tends to be inherited. Children with immediate relatives who have mood disorders are more likely to have bipolar disorder.

I just want to point out that bi-polar disorder (also known as manic-depression) can be overlooked in children and may be misdiagnosed as ADHD , depression, and/or Oppositional Defiant Disorder (ODD) . Previously the psychiatric community believed that bi-polar had a later onset. That theory is changing, but slowly. Many (maybe most) of the adults diagnosed with bi-polar seem to go through multiple diagnoses before finding help, partly because bi-polar is difficult to diagnose, and partly because doctors seem to be leery about "labeling" a child with bi-polar. Part of the confusion is that kids with bipolar may have very different symptoms from adults. For example, they may cycle frequently in the same day.

Different from the normal ups and downs that everyone goes through, the symptoms of bipolar disorder are severe. They can result in damaged relationships, poor job or school performance, and even suicide. But there is good news: bipolar disorder can be treated, and people with this illness can lead full and productive lives. More than 2 million American adults, or about 1 percent of the population age 18 and older in any given year, have bipolar disorder. Bipolar disorder typically develops in late adolescence or early adulthood. However, some people have their first symptoms during childhood, and some develop them late in life. It is often not recognized as an illness, and people may suffer for years before it is properly diagnosed and treated. Like diabetes or heart disease, bipolar disorder is a long-term illness that must be carefully managed throughout a person's life. "Manic-depression distorts moods and thoughts, incites dreadful behaviors, destroys the basis of rational thought, and too often erodes the desire and will to live.     What is Bipolar Disorder? Bipolar disorder (also referred to as manic-depressive illness) usually starts in adult life, before the age of 35. Although rare in young children, it can appear both in children and teenagers. This illness can affect anyone. If one or both parents have bipolar disorder , the chances are greater that their children will develop the disorder. Recognizing The Warning Signs Of Bipolar Disorder Bipolar disorder may begin with manic or depressive symptoms. The manic symptoms include:
    • Severe changes in mood, when compared to peers, either unusually happy or silly, or highly irritable.
      Unrealistic highs in self-esteem.
      Great energy increase; can go with little or no sleep for days without tiring.
      Increased talking - talks too much, too fast; changes topics too quickly; cannot be interrupted.
      Distractibility - attention moves constantly from one thing to the next.
      High risk behavior - such as, jumping off a roof and believing no harm will occur to them.

Related Links Bipolar disorder with onset during childhood or early adolescence appears to be as common in prevalence as adult onset bipolar disorder and may be a different (possibly more severe) variant of the disorder. It is difficult to recognize and diagnose, because the symptoms and behaviors associated with the disorder manifest themselves differently than adult onset bipolar disorder, and often appear similar to (or can co-occur with) other more common childhood disorders such as attention deficit hyperactivity disorder (ADHD), disruptive behavior disorders, conduct disorders, or even normal "mood swings" of early and middle adolescence. Some controversy surrounds the diagnosis and continued research is needed to improve our understanding and treatment of this mood disorder in children and adolescents.
