Body Dysmorphic Disorder
SYMPTOMS
Preoccupation with an imagined defect in appearance. If a slight physical anomaly is present, the person's concern is markedly excessive. The preoccupation causes clinically significant distress or impairment in social, occupational, or other important areas of functioning. The preoccupation is not better accounted for by another mental disorder (e.g., dissatisfaction with body shape and size in Anorexia Nervosa).

May 15th, 2001 In Snow White, the princess’ evil stepmother gazes into the looking glass, asking, “Mirror mirror on the wall who’s the fairest of them all?” Here in the real world, a person who gazes into a mirror thinking, “Mirror, mirror on the wall, I know I’m the ugliest of them all,” may be suffering from BDD, or Body Dysmorphic Disorder - certainly something far more dangerous than the stepmother's vanity. In a recent interview with Talk Magazine, even beauty goddess Uma Thurman admitted to struggling with the disorder - one that has its victims feeling that they are simply not attractive. The lanky 6-foot blonde admitted to "being troubled about her weight" ever since giving birth to her daughter, Maya, in July 1998. Thurman, whose beauty and bodacious body are celebrated world-wide, has recently been named the spokesmodel for Lancome's Miracle perfume. She also stars in a new movie released this week, Golden Bowl.

W ith her long strawberry-blond hair, large green eyes and beautiful complexion, 22-year-old Jennifer would be considered attractive by any standards. She reminded Katharine A. Phillips of the captain of a high school cheerleading squad, but there had been no crisp fall afternoons on the sidelines of the football field for Jennifer: She had dropped out of high school, and was steadily dropping out of public life. "I'm too scared to go out - everyone will see how ugly I am," Jennifer told Phillips, the assistant professor of psychiatry who diagnosed the young woman with body dysmorphic disorder (BDD). Phillips, director of the Body Dysmorphic Disorder and Body Image Program at Butler Hospital, is one of the nation's foremost experts of BDD, in which sufferers are obsessed with perceived flaws in their appearance to the extent that it disrupts their life, and at its extreme can lead to thoughts of suicide. Jennifer purchased clothes through catalogs, went to the food store during the middle of the night, and rarely ventured out of her parents' home during the day because she believed her skin was pimpled and scarred, Phillips wrote in her book on the disorder, "The Broken Mirror."

PROVIDENCE, R.I. When Jennifer sought psychiatric help, she was 22 years old with long strawberry-blond hair, large green eyes and a beautiful complexion. She believed she was magnificently ugly: I do go out sometimes, but mostly at night when no one can see me. Sometimes I go to a 24-hour grocery store at midnight, when I know no one else will be there. For a long time I've bought most of my clothes through catalogs. I'm too scared to go out - everyone will see how ugly I am. Jennifer had body dysmorphic disorder (BDD), a debilitating disorder in which sufferers are obsessed with imagined flaws in their appearance. In Providence, Rhode Island, Good Medicine met two young women who did just that, battling what American doctors have labelled Body Dysmorphic Disorder or BDD. Essentially this means an individual is obsessed with a very small defect in their appearance or imagines a defect in their appearance where there is none.
Dr Ralph Albertini and Dr Katherine Phillips specialise in the treatment of BDD. They say while there are close links with Obsessive Compulsive Disorder, BDD is much more serious. Dr Phillips says patients with BDD have a lot more shame, a sense of embarrassment and unworthiness and also tend to think about suicide and, in some cases, even attempt suicide.

Today the distressing experience that is Body Dysmorphic Disorder, no doubt you’ve heard by now of Botox a popular product based on the toxin that causes botulism. People are injecting it into their wrinkles in order to look young and smoothed skinned. Well you won’t be surprised to hear that Botox is absolutely booming in the United States where sales last year topped $300 million. What is Body Dysmorphic Disorder?
People with body dysmorphic disorder (BDD) worry about their appearance. They worry, for example, that their skin is scarred, their hair is thinning, their nose is too big, or something else is wrong with how they look. When others tell them that they look fine or that the flaw they perceive is minimal, people with this disorder find it hard to believe this reassurance.
People with BDD think a lot about their perceived appearance flaw, generally for at least an hour a day. Some say they're obsessed. Most find that they don't have as much control over their thoughts about the body flaw as they would like.
In addition, the appearance concerns cause significant distress (for example, anxiety or depression) or significant problems in functioning. Although some people with this disorder manage to function well despite their distress, many find that their appearance concerns cause problems for them. For example, they may find it hard to concentrate on their job or school work, which may suffer, and relationship problems are common. People with BDD may have few friends, avoid dating, miss school, and feel very self-conscious in social situations.
The severity of BDD varies. Some people experience manageable distress and are able to function well, although not up to their potential. Others find that this disorder ruins their life.

"The essential feature of Body Dysmorphic Disorder (historically known as dysmorphobia) is a preoccupation with a defect in appearance. The defect is either imagined, or, if a slight physical anomaly is present, the individual's concern is markedly excessive. The preoccupation must cause significant distress or impairment in social, occupational, or other important areas of functioning... Complaints commonly involve imagined or slight flaws of the face or head such as hair thinning, acne, wrinkles, scars, vascular markings, paleness or redness of the complexion, swelling, facial asymmetry or disproportion, or excessive facial hair. Other common preoccupations include the shape, size or some other aspect of the nose, eyes, eyelids, eyebrows, ears, mouth, lips, teeth, jaw, chin, cheeks or head. However, any other body part may be the focus of concern (e.g., the genitals, breasts, buttocks, abdomen, arms, hands, feet, legs, hips, shoulders, spine, larger body regions, or overall body size). The preoccupation may simultaneously focus on several body parts. Although the complaint is often specific, it is sometimes vague. Because of embarrassment over their concerns, some individuals with Body Dysmorphic Disorder avoid describing their 'defects' in detail and may instead refer only to their general ugliness. Most individuals with this disorder experience marked distress over their supposed deformity, often describing their preoccupations as 'intensely painful', 'tormenting', or 'devastating'. Most find their preoccupations difficult to control, and they may make little or no attempt to resist them. As a result, they often spend hours a day thinking about their 'defect', to the point where these thoughts may dominate their lives. Significant impairment in many areas of functioning generally occurs. Feelings of self-consciousness about their 'defect' may lead to avoidance of work or public situations.
