LOS ANGELES, CA October 26, 1999 The drug clomipramine is more effective than desipramine in the treatment of body dysmorphic disorder (BDD), a disorder in which a person is preoccupied with an imagined or slight defect in their appearance. These results are presented in an article in the November issue of the Archives of General Psychiatry , a member of the JAMA family of journals.
Eric Hollander, M.D., and colleagues from the Mount Sinai School of Medicine, in New York, randomized 29 patients to initially receive either clomipramine or desipramine for treatment of BDD for the first eight weeks of the drug trial. The participants then "crossed over" to use whichever drug they had not yet used for the remaining eight weeks of the trial. The trial was double-blinded (neither the participants nor the psychiatrists and psychologists who evaluated the participants were aware of which treatment the participants were receiving). Dr. Hollander presented the results of the study today at the American Medical Association's 18th Annual Science Reporters Conference at the University of California at Los Angeles.

Phillips, M.D. , of Butler Hospital in Providence, RI (NARSAD Young Investigator) is studying the Prevalence of Body Dysmorphic Disorder In Medical, Surgical, and Psychiatric Populations. Body dysmorphic disorder (BDD), a preoccupation with an imagined defect in appearance, is a severely disabling condition that often goes undiagnosed. Instead, individuals with BDD often seek costly, risky, nonpsychiatric treatment, such as cosmetic surgery, that does not relieve their symptoms. Dr. Phillips will develop an instrument to asses the prevalence of this disorder in several patient populations, including psychiatric outpatients. Katharine A. Previous Next Treatment of Patients with Body Dysmorphic Disorder BDD has been considered an obsessive-compulsive disorder because some of the clinical features are similar (age of onset, intrusive thoughts and recurring behaviors); thus, the treatment of patients with BDD has involved the use of similar medications. Some preliminary study results suggest a limited response to selective serotonin reuptake inhibitors, but no controlled treatment studies have been published. Other agents, such as lithium, neuroleptics, trazadone and benzodiazepines, have not proved effective. Hollander and colleagues performed the first systematic double-blind, controlled study of the pharmacologic treatment of patients with BDD. They compared clomipramine (a potent serotonin reuptake inhibitor) with the active control desipramine (a selective norepinephrine uptake inhibitor). Patients enrolled were between the ages of 18 and 65 who met the Diagnostic and Statistical Manual of Mental Disorders

What is Body Dysmorphic Disorder
BDD is thought to be a subtype of obsessive-compulsive disorder. It is not a variant of anorexia nervosa or bulimia nervosa.
The person with an eating disorder says, "I am so fat." The person with BDD says, "I am so ugly."
Not yet an official diagnosis, but may achieve that status soon. Sufferers are excessively concerned about appearance, body shape, body size, weight, perceived lack of muscles, facial blemishes, and so forth. They are convinced these flaws exist in spite of reassurances from friends and family members who usually can see nothing to justify such intense worry and anxiety. BDD often includes social phobias. Sufferers are shy and withdrawn in new situations and with unfamiliar people.
BDD affects about two percent of the people in the United States. It strikes males and females equally. Seventy percent of cases appear before age eighteen.

Rate . "The rate of body dysmorphic disorder (BDD) in inpatient psychiatric settings and the nature of the presenting complaints are unknown. Because of the shame and humiliation that BDD patients suffer, we hypothesized that, unless specifically screened for at the time of admission, BDD would be underdiagnosed in psychiatric inpatients. . . . . Sixteen (13.1%) of the 122 subjects were diagnosed with BDD. None of the subjects with BDD had been diagnosed with BDD by their treating physician during hospitalization. All 16 subjects reported that they would not raise the issue with their physician unless specifically asked due to feelings of shame" (Grant et al. 2001:517). Neuro-notes .
