Buerger's Disease Etiology and Pathophysiology:
Smoking is very closely related to Buerger's disease and smoking history is one of the criterion for diagnosing the disease. In general if the patient absolutely abandons smoking the course of the disease will be invariably benign, but if smoking continues any treatment will ultimately be futile. Though "passive smoking" has adverse effect on cardiovascular system, non smokers should never develop the disease. Active smokers can be indentified by measuring levels of continine, the major metabolite of nicotine in urine. Since all smokers do not develop the disease an immunopathogenesis is considered probable. It has been proposed in Japanese that presence of a gene linked to some HLA antigens might control the susceptibility to the disease. Socioeconomic conditions, work environment may also play in etiology as the disease is seen more in out door and manual workers. Hypercoagulable state has been observed in association with the disease. Hepatitis B Virus and rickettssiosis may contribute to pathogenesis, but this role is uncertain. Buergers disease is an inflammatry occlusive disease which involves all layers of medium sized and small arteries of the extremitiles. Involved superficial veins bear a close resemblance to those in the affected artery. Majority of the patients develop critical limb ischemia with trophic lesions are distal to ankle, the anklebrachial doppler index could be normal in early stage. Toe pressures can be measured and if it is less than 30 mm Hg, the healing of ulcers is unlikely. The disease though commences peripherally, may gradually extend proximately occluding the larger arteries.

Reaction to Having the disease: It was a relief in one way, because I had suffered from the symptoms for 6 yrs.and was told numerous reasons for the pain in my arms and legs. It was hard on my father,because his father had this disease. That was scary, because he lost one of his legs, because of it. Dealing with the disease: It seems as though you are always sick. Hardly a week goes by that there is notpain of some sort. Of course, having to give up my career in Accounting when the disease progressed was emotionally hard. Other info about the disease: It is very rare and hard to diagnose, once you are diagnosed, be very comfortablewith your dr. You will spend alot of time in their office and you need to feel as thoughthey understand your pain and are giving you the best care possible. Take any medications given to help your circulation, and by all means try not to smoke. It's hard, I was diagnosed in 1995 and am still fighting the nicotine habit. Buerger's disease (Thromboangiitis Obliterans) is an inflammatory disease of the small and medium sized arteries and veins of the extremities. It is often seen in men and appears to be directly related to smoking. There may also be an autoimmune element as well. Raynaud’s phenomenon, ulcers and pain are typically seen. Immediate termination of smoking is critical, but sadly, often difficult to do for these patients.

Can you tell me where the name comes from and whether or not it is caused by smoking? When you see my signature you will understand my interest. ANSWER: This disease is named for Dr. Leo Buerger who first described it in 1908. Buerger's disease is relatively rare in this country, but it does occur primarily to young smokers and is considered to be directly related to smoking. This condition, which is also called "thromboangiitis obliterans", consists of blockages in the small- and medium-sized arteries of the arms and legs and can lead to gangrene and, frequently, to a loss of a limb. The first symptom of Buerger's is often lack of blood circulation to a section of the lower leg and localized gangrene in the toes. A recent study of 26 people who had Buerger's disease over the last 15 years found that most sufferers are male and all smoked cigarettes. Eight of these patients lost a leg to the disease. Only 8 percent of the cases occur in women in other statistics. Oddly, Buerger's disease was widely diagnosed in the United States early in this century, but the number of cases have declined drastically. Many cases continue to be seen in other parts of the world. No one knows why the distribution of this condition has changed. Mr. M was a 38-year-old male who has been smoking about 10 cigarettes a day for over 15years. He had no symptoms in the past. He had sudden onset of severe pain in the left middle toe and noticed the toe to be discolored. He was admitted to a local hospital, treated with analgesics and IV Trental. It was noted that his left dorsalis pedis pulse was absent, the posterior tibial pulse was weak but palpable. Pulses in the other leg and upper extremities were normal. After about a week's treatment, since the patient did not improve a left lumbar sympathectomy was performed. On the 4th post operative day patient developed pain and discoloration of left 5th toe and was then referred to us for vascular evaluation. Patient was admitted to the hospital under our care and IV Trental was discontinued. He underwent angiogram which showed a high grade stenosis of the left popliteal artery behind the knee and showed "corkscrew" like collaterals suggestive of TAO.

ABI: Ankle Brachial-Index measurement. A method for testing for peripheral arterial disease through the use of a Doppler probe and a blood pressure cuff on both the arms and ankles. Amputation: Surgical removal of a limb or portion of a limb. Above knee, below knee, or partial foot are all varieties. Arteriogram: An x-ray used to determine specific arterial blockages in the body. The procedure involves inserting a small c atheter into the artery that injects dye. Artery: A pipeline (blood vessel) carrying oxygen-rich blood from the heart to the rest of the body. When diseased, the organ supplied may become damaged due to lack of oxygen and nutrients. Blood pressure: The force of blood pushing against blood vessel walls. C
Catheter: A tiny flexible inserted in a blood vessel to inject dye, assist with the removal of a blood clot, or inject medication. Critical Leg Ischemia: A severe obstruction of the arteries which seriously decreases blood flow to the extremities (hands, feet and legs) and has progressed to the point of severe pain and even skin ulcers or sores. Critical Limb Ischemia (CLI) is often present in individuals with severe peripheral arterial disease (PAD). D
Doppler: A diagnostic tool that uses low intensity ultrasound to detect blood flow velocity in arteries or veins.

I was first diagnosed as having Buergers disease in the early sixties. Little was known on the causal effects of this syndrome but increasing research established the causal link between Buergers and smoking. It is axiomatic that if you do not smoke you not get Buergers disease. The results of my smoking have been the bilateral amputation of both legs below the knee and the loss of some finger tips. There was the distinct possibility that both hands would require amputation but with the immediate cessation of smoking I regained almost full use of my hands Last changed: February 14, 2002

I reported aboard the USS BURTON ISLAND AGB-1 at Long Beach California,in February of 1955.I was aboard until November 29th 1956. I made two Northern Cruises during that period of time.
Lots of ice all around us.But that is what she was built for.
The Burton Island was launched April 30th 1946,designated AG-88 and commissioned December 28th 1946. Her first skipper was CDR G KETCHUM.Her first cruise was a southern cruise to ANTARCTICA,for the first ANTARCTIC Development Project in 1947. She steamed through the ice pack to the Bay Of Whales to unload supplies,and received passengers for the return voyage to San Pedro,California. Between April 1948 and December 1956 she participated in nineteen different cruises in Arctic and Alaskan waters. She was designated AGB-1 in March of 1949.In December 1966 was transferred from the US Navy to the US COAST GUARD, and designated WAGB_283.
This is how she looked after her new designation as a COAST GUARD ICE BREAKER.

We now have over 1,000 members and send over 500 newsletters. Initially we sent with the newsletter a list of people willing to be contacted so that new members and anybody wanting a chat could look in the list and find somebody geographically near them. However, the list got so long we couldn’t afford the postage or printing. So it was suggested that we divide the country into areas (see map). A kind volunteer from each region has agreed to be the contact person (CP). If you are willing to be contacted could you phone the CP responsible for your area and let them know. The CP will keep details of your name, phone number, the town where you live and the illness, which concerns you most. Then as people wish to talk they will phone the CP who will put them in contact with somebody nearby.
