Abstract
Buerger's disease (thromboangiitis obliterans) is a rare peripheral vascular disease that usually affects young male smokers. It is characterized by multiple occlusions distal to the knee and elbow. Buerger's disease is a peripheral arterial occlusive disease that is becoming rare in Western countries but is more common in Asia. To evaluate the role of sympathectomy in Buerger's disease, we carried out this study and investigated the changes in the prevalence and characteristics of Buerger's disease at a major surgical institution in Iran. Among 100 sympathectomized patients (84 % male and 16 % female) as a whole 266 organs were involved and solitary involvement of only upper limb was the rarity (in 8.7 %).

Pain may be very intense in the affected regions. An angiogram demonstrating lack of blood flow to vessels of the hand ( figure below A normal angiogram of the hand ( figure below Despite the severity of ischemia (lack of blood flow) to the distal extremities that occurs in Buerger’s, the disease does not involve other organs, unlike many other forms of vasculitis. Even as ulcers and gangrene develop in the digits, organs such as the lung, kidneys, brain, and gastrointestinal (GI) tract remain unaffected. The reasons for the confinement to the extremities and sparing of other organs are not known. (top of page) What Causes Buerger's Disease? Pictured below , are a patient's fingertips that have developed gangrene. This is a very painful condition which sometimes requires amputation of the affected area.

Buerger's Disease Buerger's disease is a circulatory disease that is brought on by chronic inflammation of the blood vessels in the extremities. Also called thromboangiitis obliterans , Buerger's disease is most common with people who smoke . It normally affects the foot or lower leg, but it may occur in the hand, arm, or thigh as well. Early signs of Buerger’s disease are a tingling sensation like "pin pricks" and a burning sensation in the fingers and toes. It can lead to ulceration and gangrene; in severe cases, amputation may be required. There are other disorders associated with circulatory problems. When plaque or fatty deposits form along the walls of the arteries, it causes them to harden and constrict. Hypertension , or high blood pressure, results because the blood exerts greater force against the walls of the narrowed and/or more rigid blood vessels. Hand And Foot Pain A 31-year-old white male is seen in Urgent Care for the chief complaint of hand and foot pain beginning abruptly three days ago, and progressively worsening since then. Prior to the onset of pain, he noted several days of intermittent cough, low grade fever, and chills. Pain is described as diffuse, very severe, and sharp, and confined to the skin of the hands and feet. The patient denies any other symptoms. Of note, he remembers eating recently at Wendy's, a fast-food restaurant, where he ordered a Bacon Double Cheeseburger, but received a plain Double Cheeseburger instead (without bacon). No previous episodes are reported. Social history reveals that the patient is single, heterosexual, and has smoked two packs of cigarettes per day for fifteen years. He now presents for further evaluation and management.

Kroeger (D) Buerger’s Disease - last month's - update in red Buergers’ Disease (Thrombangitis obliterans, Winiwarter Buerger) was first described by Buerger in 1908 as a spontaneous acral gangrene in young men without prior vascular disease. Till today the pathologic mechanisms of the disease are not completely known. Buerger’s disease has to be differentiated from atherosclerotic peripheral obliterate disease. The diagnosis Buergers’ disease is made if certain criteria are fulfilled and no other reason initiating the vascular occlusion can be proved. Although there are some specific histological findings a histologic investigation is not required for the diagnosis. A lot of smaller studies have investigated possible pathogenic mechanisms. An infection with Reckettsia was discussed as well as allergic, immunologic, hematologic or hereditary disorders. Psychosomatic conflicts were hypothesised to trigger the onset of Buerger’s disease. Most of this study results were never repeated or independently proved in larger populations. Effective surgical or pharmaceutical therapeutic options are not available yet. Usually a pharmaceutical therapy with anticoagulation, dilution of the blood, vasodilatation and wound care is done. Analgetic therapy using opioids is often necessary. But because a causal therapy is not known a lot of the patients suffering from Buerger’s disease get amputated. Even if an amputation can be avoided severe disturbances of the peripheral perfusion persists.

This is a phase 4 study following the FDA accelerated approval of HGH for wasting, to confirm HGH's benefits. This was a double-blind, placebo controlled, multi-center study. Patients with 10% weight loss, a body mass index <20, or weight <90% of the ideal body weight were randomized to 12 weeks of treatment with HGH, 6 mg daily (DD), 6 mg on alternate days (AD), or placebo.
The primary endpoint of the study was to confirm the clinical efficacy of HGH compared with placebo, based on exercise function change as assessed by cycle ergometer work output from baseline to week 12. The secondary endpoint was the change in lean body mass (was LBM increased) as measured by bioimpedance spectroscopy (BIS). LBM and total body and trunk fat mass were assessed bt DEXA (dual energy X-ray absorptionetry). Quality of life questionaires were used to assess patient's response to treatment. Below is a review of the adverse events profile for the patients in the study.
RESULTS
A total of 757 patients (247 placebo, 253 DD, 257 AD) started the study and 646 completed 12 weeks. In the evaluable population (n=555) patients were mostly white (77%) and male (91%). Average age was 41. All patients were on ART with 88% on HAART. 8% Black. 10% Hispanic.

Hello,
I am 38, Male usually healthy. In 1992 I was Dig. with Buerger dis. After smoke quiting all symptoms declines and I had normal life. I also went back on smoking for some years with NO! evident symptoms of the Buerger Dis. Last Feb, I had a chest pain that lasted about 10 days, on the 10th day I had a frighting episode of arithmia, all chekups where O.k. I head an Echo, a stress ECG, and a holter (minor ectopinc activity - 2 pe's and 7 sve's ). The major holter finding was that while sleeping I had polse of 43. and I had 2 incidents of tachicardia, one of them induced by a cigarete and showed also some T segment changes on the HOlter ECG.

BUERGER' S DISEASE
(THROMBOANGIITIS OBLITERANS) Inflammatory Diseases Home Page . The lower extremities are involved in most cases, and the upper extremities in 40-50% of cases . The occlusions are made by inflammatory thrombi, which differ from normal thrombi in that they include many inflammatory cells, with neutrophil polymorphs being the most prevalent . Male, heavy tobacco users, 25-50 years old are mostly affected PATHOLOGY There are three phases in this thrombocytic disease. The acute phase is characterised by inflammation. Firstly, platelet adhesiveness is increased and the lumen is obstructed by a highly cellular thrombus, which contains a dense mass of neutrophil, lymphocytes and giant cells. The affected areas are interspersed along the blood vessel. Mononuclear leukocytes and lymphocytes remain in the lumen, forming foamy giant cells . At the periphery of the thrombus, microabscesses that contain polymorphs and multinucleated giant cells may form . The inflammatory cells then migrate into the surrounding vessel wall, involving the intima, media and adventitia. The inflammatory reaction spreads to the vessel coats, adjacent vessels and nerves. In the subacute phase , inflammatory cells are less involved. Perivascular fibrosis, recanalization of the thrombus and vascularization of the blood vessel wall is characteristic
