One of the most common types of back surgery is disk removal (diskectomy), the removal of an intervertebral disk, the flexible plate that connects any two adjacent vertebrae in the spine. Intervertebral disks act as shock absorbers, protecting the brain and spinal cord from the impact produced by the body's movements. Purpose About 150,000 Americans undergo disk removal each year in the United States. Removing the invertebral disk is performed to treat back pain that has lasted at least six weeks as a result of an abnormal disk and that has not responded to conservative treatment. Surgery is also performed if there is pressure on the lumbosacral nerve roots that causes weakness or bowel or bladder disfunction. As a person ages, the disks between the vertebrae degenerate and dry out, and the fibers holding them in place tear. Eventually, the disk can form a blister-like bulge, compressing nerves in the spine and causing pain. This is called a "prolapsed" (or herniated) disk. If such a disk causes muscle weakness or interferes with bladder or bowel function because it is pressing on a nerve root, immediate surgery to remove the disk may be needed. The aim of the surgery is to try to relieve all pressure on nerve roots by removing the pulpy material from the disk, or the disk itself. If it is necessary to remove material from several nearby vertebrae, the spine may become unsteady. In this case, the surgeon will perform a spinal fusion, removing all the disks between two or more vertebrae and roughening the bones so that the vertebrae heal together. Bone strips taken from the patient's leg or hip may be used to help hold the vertebrae together. Spinal fusion decreases pain but it also decreases spinal mobility.

Can physical therapy relieve a bulging lumbar disc? Discs are the cushions or shock absorbers located between each vertebra. The outer layer of the disc is made of fibrocartilage laid down in many layers to give the disc strength. The inner portion of the disc is made of a gel-like substance, which is mostly water. All discs will experience wear and tear as we get older. The lumbar discs are located in the lower portion of the spine and absorb most of the weight of the body. They are the most prone to wear-and-tear and injury. The discs can be injured by trauma, frequent repetitive movements like bending and twisting and prolonged static postures like sitting in a car or at a desk. The inner portion of the disc can push out on the injured part of the outer disc and cause the disc to swell or bulge. Bulging discs can cause local back pain and can even refer pain to one or both legs if the bulge is big enough. A bulging disc can also produce symptoms of numbness or weakness in the leg. An MRI or CAT scan can show the presence of a bulging disc. Most disc problems get better with a combination of time, rest and the proper treatment. The vast majority of bulging discs do not need surgery. This is where physical therapy can help.

Not that long ago, Bonnie Peebles was delivering the first of her two children and beginning the happiest time of her life. Her joy quickly faded as both children were diagnosed with Cystic Fibrosis, an often fatal inherited disease affecting the pancreas, respiratory system and apocrine glands.
The downward spiral dictating her life continued after Peebles received a diagnosis of Crohn's Disease, an intestinal disorder. To combat her disease, doctors placed Peebles indefinately on high dose steroids which caused her to gain 100 pounds. A large disk then slipped out of place forcing her to use a walker. After time, she developed disuse atrophy of the muscles requiring her to use a wheelchair.
Ultimately she was confined to an electric wheelchair requiring the use of braces to keep herself vertical. At the lowest point of her life, crippled and unable to care for herself, she met David Duncan MD, a family practice physician at Columbia Doctors Hospital and ColumbiaSpecialty Hospital, and finally began the road to recovery.
"I hurt all the time and I could not get out of the electric wheelchair," said Peebles. "My lower back and legs hurt and I did not think it would improve." "I just thought the pain would continue to get worse and go on forever."

It seems so long since my diagnosis of Lupus in January of 1995, yet at times it seems like yesterday. I can clearly remember how I felt when I heard I had Lupus....the emotions, the fear...the UNKNOWN. But when I think of all that I have been through since then, and am still going through, I am in awe that it HAS only been 4 years. So many changes have taken place, both good and bad, but the changes in who I have become are probably the most dramatic.
Let me start by telling you a little about myself. I was born April 13, 1972 in a town called Bayshore in Long Island, New York. My parents separated when I was 4, divorced when I was 9. I had wonderful grandparents that I lived with for many years that helped raise me and instill values that I am influenced by to this day.
I had a rather uneventful childhood....full of the regular illnesses typical children tend to succumb to...chickenpox, frequent step infection, eye infections..the usual. But looking back, and hindsight is 20/20, I had many allergies and pains that aren't typical with children.
In 1984, my mother and I moved to Maryland to be with the man who she soon would marry. I have lived here ever since. Currently, I live with my fiance of 6 years, Richard, and although there are no immediate plans to marry, we are happy.
