When the wrist is poorly positioned, there isn't enough space for the median nerve to travel to the hand. Carpal tunnel syndrome is caused by pressure exerted on the median nerve at the point where it passes through the wrist. The median nerve supplies sensation to the thumb-side of the palm, and to the thumb, index finger, middle finger, and the thumb-side of the ring finger. It also supplies movement to part of the hand. The nerve enters the hand between the wrist bones (called the carpal bones) and the tough membrane that holds the bones together (the transverse carpal ligament). This space is called the carpal tunnel. Since the passageway is rigid, any swelling in this area can cause compression of the nerve (this is also called entrapment of the nerve).

That carpal tunnel syndrome occurs in New Orleans comes as no surprise to anyone, that we see it in our peripheral nerve clinic is somewhat more surprising. Many might think that the diagnosis and treatment of carpal tunnel is so straightforward that there is seldom need to refer it. In general, this is true, yet there are atypical or problematic cases which we see in our peripheral nerve clinic.
In those patients with typical symptomatology, numbness and paresthesias in the median distribution, irritability of the nerve as demonstrated by a Tinel's sign at the wrist or a positive Phalen's maneuver are usually relieved of their symptoms with a carpal cunnel release (CTR). This is a successful operation with patient satisfaction at LSU being 89% in 376 operations. Resolution or reversal of the symptoms vary with their nature and severity. Numbness improved in 56% and weakness to a lesser degree of 42%.
It is our belief that some of the failures of CTR we have see are due in part to prolonged immobilization or splinting. Consequently early mobilization and use of the hand is actively encouraged and occupational therapy sometimes employed to achieve this goal.

The standard surgical technique for median nerve decompression that is, carpal tunnel release has been a very successful operation restoring most patients to their prior activities.
One of the major draw backs to standard, open carpal tunnel release was the slowness of recovery in the palm side surgical scar. Often, the 2 inch palm side scar would remain sensitive to direct pressure for approximately six to eight weeks. in the working patient, this scar sensitivity could preclude return to normal work activities. Recently, orthopedic technology has advanced to the point where a standard arthroscope has been specially modified to be of use in carpal tunnel surgery. To employ the endoscopic technique in carpal tunnel release, a small 1/2 inch horizontal incision is made at the wrist and the arthroscope is introduced underneath the transverse carpal ligament. A cannula is inserted into the hand, just under the offending ligament. The surgeon looks through a scope and uses a small knife to simply cut the ligament, thus freeing the nerve. A special blade attached to the tip of the arthroscope is then utilized to incise the transverse carpal ligament from the inside of the carpal tunnel. In effect, by cutting through the transverse carpal ligament, the compression on the median nerve is alleviated and the nerve is thus allowed to go on the heal. With nerve healing, the painful wrist syndrome is corrected.

Carpal tunnel syndrome is compression of the median nerve in the wrist. Nerves are soft structures and they can become compressed at various sites in the body. The median nerve is one of many that control the muscles of the arm and hand. It also relays sensation from the skin on the palmar surface of the thumb, index and middle fingers, and also half of the ring finger. It runs from the elbow through the forearm to enter the wrist and hand on the same side as the palm. Although the nerve can be damaged anywhere along its course, it is most commonly compressed as it enters the wrist. Here, the nerve lies in a tunnel; the floor of this is made up of the bones and tendons of the wrist, while the ceiling is a band made of tendon.     The old school formula of platform gaming has become nearly as antiquated as the 8-bit graphics that used to accompany such games. The basic recipe was numerous levels, followed by a boss, followed by more levels and harder bosses, occasionally spiced up by an unexpected visit from an earlier boss, culminating in an epic battle against some big baddie that filled up nearly the entire screen with his overwhelming, er, badness. Most modern games have largely forsaken that formula, but the good folks at Smilebit saw fit to resurrect the good ol’ days of gaming with their latest Xbox release, Gun Valkyrie. In Gun Valkyrie (let’s call it GV for short), you take control of one of two futuristic jet-pack wearing and massive gun carrying commandos, all decked out in the latest technology that the early 1900s have to offer. Yes, this game takes place in the late Victorian era, when horse-drawn buggies were all the rage and readers eagerly anticipated the printing of the next Sherlock Holmes adventure. In the game's alternate past, the passing of Halley's comet graces Earth with a new type of energy source, allowing for all sorts of advances like highly efficient land transportation, space exploration and colonisation, and really big guns. Dr. Hebble Gate is the man behind all the advancements, and wouldn't you know, he's gone missing. What a coincidence, at the same time one of the British Empire's colonised planets, Tir na Nog ("Land of Youth" in Gaelic), suddenly stops responding to calls, and it's up to you to find out what’s happened.
