Advocates Lift Up Childhood Depression Day
Advocates for children with mental health problems on Childhood Depression Awareness Day, at a May 8 press conference emphasized the importance of "aggressively and completely" treating childhood depression and called attention to the need for parity in mental health services. The press conference coincided with an art exhibit and quilt featuring artworks created by children with mental illnesses, displayed in the United Methodist Building, which dramatized their struggle with psychiatric and emotional disorders. The wives of US Senate leaders, religious leaders, mental health advocates, and parents of children with mental illnesses joined to increase public awareness of the need to diagnose and treat childhood depression. The observance is part of the National Mental Health Month, held each May. "It is frightening that an illness rages our young people so silently that it requires a day of awareness," said Jane Hull Harvey, GBCS assistant general secretary. "Yet, with today’s action comes the hope that awareness will be followed by diagnosis and treatment, support systems and research." Harvey noted that there was a "special appeal" in her voice because her son, Stephen, battled depression and committed suicide more than 10 years ago. According to the Center for Mental Health Services, one in 33 children and one in eight adolescents suffer from depression. Suicide is the third leading cause of death for children aged five to 14 and the sixth leading cause of death for teens and young adults aged 15 to 24.

Richfield resident Ann knew her daughter Emmaline was different from other children almost from the beginning. But she didn't know why. But now that Emmaline has been diagnosed with bipolar disorder, and has appeared in an HBO program about the illness, the family has committed itself to educating the public about the disorder. The cable program, "What Matters," will be rebroadcast twice this month [Dec. 1999]. "This is really important to me, to get rid of the stigma of mental illness," Ann said. "The more people know the less they'll be afraid of it." At just three months old, Emmaline was already a very active and driven child. "She was always really, really active and intense," Ann said. "I thought she was going to be my emergency room baby." As Emmaline grew, she was constantly getting into things and became prone to overstimulation. She became combative and would continually seek out arguments. "You'd say,'the sky is blue' and she'd say,'no, it isn't'" Ann recalled. "One day we were at Como Zoo and I said, 'look at the polar bear' and Emmaline said, 'that's not a polar bear'. She just wanted to start an argument." I think she began to argue before she could talk.

Childhood Depression and Adult Personality Disorder: "Alternative Pathways of Continuity" Stephanie Kasen, PhD; Patricia Cohen, PhD; Andrew E. Skodol, MD; Jeffrey G. Johnson, PhD; Elizabeth Smailes, MPhil; Judith S. Brook, EdD; Archives of General Psychology Background:
This study extends previous findings of the risks posed by childhood major depressive disorder and other psychopathological features for later personality disorder (PD) in a random sample of 551 youths. Methods: Self-reports and mother reports were used to evaluate DSM-III-R (Axes I and II) psychiatric disorders at mean ages of 12.7, 15.2, and 21.1 years. Logistic regression was used to examine the independent effects of major depressive disorder in childhood or adolescence on 10 PDs in young adulthood. Results: Odds of dependent, antisocial, passive-aggressive, and histrionic PDs increased by more than 13, 10, 7, and 3 times, respectively, given prior major depressive disorder. Those effects were independent of age, sex, disadvantaged socioeconomic status, a history of child maltreatment, nonintact family status, parental conflict, preexisting PD in adolescence, and other childhood or adolescent Axis I psychopathological features, including disruptive and anxiety disorders. In addition, odds of schizoid and narcissistic PD increased by almost 6 times and odds of antisocial PD increased by almost 5 times given a prior disruptive disorder, and odds of paranoid PD increased by 4 times given a prior anxiety disorder.

When we think of children and depression , the picture that most likely pops into mind is that of a lonely teenager, diary in hand, languishing in the isolation of her bedroom; or we envision a misfit adolescent, sulking in a corner with only his Game Boy and comic books for company. But depression isn't just a teenage or adolescent problem. The "blues" can strike as early as kindergarten and can progress into an extremely serious illness if left untreated. According to Ellie Epstein, an employee assistance program counselor with Methodist Hospital in Omaha, Neb., childhood depression is more common than you would think. "There are many things that can contribute to a young child's bout with depression," says Epstein. "A stressful life event is usually found to be the trigger in children ages 6 through 9. Since depression has very little to do with biology at this age, the causes are usually found to be environmental; something that has occurred either inside or outside the family unit."

The past month he has been demonstrating severe mood swings, outbursts, fussiness during sleep, wetting himself, inappropriate language, and desires to hurt others sometimes. His psychiatrist who prescribes his Ritalin for the ADHD has recommended him also taking 10 mg of Prozac. He was taking 10 mg 3x daily of ritalin and that was decreased to 5 mg 3x daily. Is Prozac truly safe for a 6-year-old? Is it often prescribed for children? Does his symptoms truly seem to be depression? There is a family history of depression. What side effects are most common or should be looked for?

Some children may express their pain in suicidal thoughts, plans or gestures, which need immediate professional attention. Somatic or physical complaints include fatigue and loss of energy, changes in appetite and sleep patterns, psychomotor agitation, a feeling of restlessness, or psychomotor retardation, a feeling of being slowed down (DSM-IV, 1994). The origins of depression have been postulated to be biological, psychological, and environmental. Children have been found to be at greater risk for depression if a parent or relative has been diagnosed as depressed (Weissman et al., 1987). Environmental stressors in childhood include the loss of a loved one or friend, a sibling leaving home, loss of a pet, moving to a new school, parental fighting, and divorce (Seligman, 1998). Treatment for depression in children includes psychotherapy in an individual, group, or family format. Medication may also be indicated upon evaluation by a physician. Psychotherapeutic approaches include cognitive-behavioral interventions, psychoeducational interventions such as social skills or relaxation training, and psychodynamic play therapy.
