Chondromalacia Patellae
What is Chondromalacia patellae? Chondromalacia literally means "softening of the cartilage", and Patellae means "the knee-cap". So Chondromalacia patellae means "softening of the articular cartilage of the knee-cap." The articular cartilage is the cartilage lining under the knee-cap that articulates with the knee joint. Under normal circumstances, it is smooth and shiny, so that it glides smoothly along the articular groove of the femur as the knee bends. When it "softens", it may break down, causing irregularities along the undersurface of the patella. What causes Chondromalacia patellae? Chondromalacia patellae occurs in two distinct age-groups:
  • It can happen in the older age-group (in the 40’s and beyond) when the articular cartilage breaks down as part of the wear-and-tear process that occurs with the rest of the body. The patella cartilage is one of the earliest places where cartilage breakdown occurs, and is slowly progressive, leading to degenerative arthritis (osteoarthritis) in the knee joint.
  • It can occur frequently in teenagers (especially girls) when the articular cartilage "softens" in response to excessive and uneven pressure on the cartilage, due to structural changes in the legs with rapid growth, and muscle imbalance around the knee. During periods of rapid growth, especially in girls, any knee valgus (knock-knees) is accentuated, thereby increasing the

Chondromalacia
What Is Chondromalacia?
Chondromalacia (pronounced KON-DRO-MAH-LAY-SHE-AH), also called chondromalacia patellae, refers to softening of the articular cartilage of the kneecap. The disorder occurs most often in young adults and may be caused by trauma, overuse, parts out of alignment, or muscle weakness. Instead of gliding smoothly across the lower end of the thigh bone, the kneecap rubs against it, thereby roughening the cartilage underneath the kneecap. The damage may range from a slight abnormality of the surface of the cartilage to a surface that has been worn away completely to the bone. Traumatic chondromalacia occurs when a blow to the knee cap tears off either a small piece of articular cartilage or a large fragment containing a piece of bone (osteochondral fracture).

Anterior Knee Pain Syndrome (Chondromalacia of the Kneecap) Introduction Knee pain is a common problem among people active in sports which involve running. One of the most common causes of knee pain is chondromalacia or patellofemoral syndrome'. As many as one-half of all knee pain complaints other than injury may be due to this syndrome. This syndrome is caused by an irritation of the undersurface of the kneecap (or patella), which is normally smooth. The irritation can lead to a roughening of the kneecap undersurface, a condition called chondromalacia. The kneecap moves up and down in a shallow groove at the lower end of the thigh bone (femur). The kneecap is connected on top to the large muscles from the front of the upper leg (quadriceps muscles) and then is connected below to the top of the lower leg bone (tibia) by a tendon patellar tendon). Tendons are the fibrous bands of tissues that connect muscles to bone. Knee pain results from the pressure of the kneecap, which becomes irritated and roughened, on the shallow groove in the thigh bone. Patellofemoral pain syndrome and chondromalacia is a form of arthritis. However the more common form of knee arthritis is a degeneration of the cartilage in the knee joint where the upper leg (femur) and lower leg bone (tibia) join. What else can cause knee pain?

The patello-femoral joint refers to a specific part of the knee joint. Medically, the kneecap is known as the patella and the thigh bone is called the femur. The knee joint is composed of three bones - the femur (or thigh bone), the tibia (or shin bone), and the patella (or kneecap). The parts of the knee joint are subdivided into the tibio-femoral joint which refers to the joint space between the tibia and the femur; and the patello-femoral joint which is the joint space between the patella and the femur. Both of these joints (patello-femoral and tibio-femoral) form the knee joint. The patella is connected to the quadriceps tendon at the top of the patella. The quadriceps tendon attaches to the quadriceps muscle which attaches to the pelvis. The patellar tendon goes from the bottom of the patella to the front of the tibia known as the tibial tubercle. When the quadriceps muscle contracts (shortens), it pulls the patella which in turns pulls on the tibial tubercle, which causes the knee to straighten (go into extension). As the knee moves, the patella glides across the front of the knee joint in a shallow groove on the front of the femur which is known as the trochlear groove of the femur. A tracking problem refers to the fact that the patella stays in place in front of the knee, but it no longer remains centered in the front part of the femur known as the trochlear groove.

Chondromalacia
Chondromalacia is the degeneration of the cartilage in your knees. Generally it is described as a pain beneath or on the sides of your knee caps. There can also be swelling, in severe cases you can hear the bones grinding. This can be caused by a muscle imbalance (Weak quadriceps and strong hamstrings, etc.) Excessive pronation (arch collapsing too much, causing the knee cap to twist side ways) can also be a source of chondromalacia. There are things you can do to self treat . For example decreasing activity, icing for swelling, even aspirin. Biking should actually help chondromalacia, not hurt the condition. You should remember to stay off the large chain ring, avoid hills and adjust your saddle so that your knee is only slightly bent at the bottom of the pedal stroke. I'm not a doctor. (I don't even play one on T.V.) I'm a 37-year-old woman suffering from chondromalacia patella, an obstinate desire to keep playing competitive volleyball, and possibly even a mid-life crisis. Consult your doctor or sports medicine specialist if you are suffering from knee pain. ( Update 18-Feb-2002: I find it terribly ironic that I've been sidelined from volleyball since late 1999 for torn wrist ligaments that could only be partially repaired by surgery. Creaking around on bad knees is one thing; doing without the use of one hand is another! Until technology gives me a fully-repaired wrist, I'll be watching my daughter, the Wild Child, a chip off the amazon block, slam volleyballs down short chicks' throats, instead of playing myself. She even stuffs those silly hitters who try to dink over herjust like Mama! :-D That's my baby! PS: I'm now almost 40! Egads!)

Dear Alice, My doctor says I have chrondomalatia. What is that?
Dear Alice, Under the knee cap and covering the ends of the femur (thigh bone) and tibia (shin bone) is a sort of natural shock absorber made of cartilage. This shock absorber does not come with a lifetime guarantee; wear and tear over the years can result in a loss of mass of the shock absorbing cartilage. Once some of this cartilage has degenerated, the knee obviously cannot absorb or handle shock from running, for example, as well as it could before. This condition is referred to as "chondromalacia patella." You can do some things to prevent (further) knee injury and pain. Always wear good, supportive shoes that aren't worn out, especially when you exercise. Have a health care provider check your feet to make sure that they aren't contributing to a misalignment of your body that puts undue stress on one knee over the other. Or, if you get a chance to see a physical therapist, ask her/him to check the way you walk. If you ride a bike regularly, make sure that your seat is up high enough so that your knee is only slightly bent at the bottom of a pedal stroke. If you're a runner, avoid running on uneven surfaces as much as possible to lower your risk of falling and/or twisting your knee. If, after every time you engage in a certain sport, say racquetball or running, you experience a great deal of knee pain, you might want to take a break from that particular activity and try a few that won't aggravate your knees (hopefully). ALICE, I JUST WANTED TO SAY THANK YOU FOR YOUR PAGE. I HAVE HAD KNEEE PAIN FOR OVER TWO YEARS AND FINALLY GAVE IN AND WENT TO THE DOCTOR. I HAVE CHONDROMALACIA AND DID NOT KNOW WHAT IT MEANT. SO THANKS FOR THE INFO. I WAS A RUNNER AND I AM NOT READY TO GIVE THAT UP, SO I AM DOING MUCH RESEARCH NOW. AGAIN, THANKS FOR YOUR PAGE AND INFO. DEE
Dear Alice, I suffered from chondromalacia for years. Arthroscopic surgery a few years ago helped, but what did the trick was developing the muscles that hold my knee in place and allow them to track properly. Spinning classes 2 - 3x/week for the last two years *really* helped with this. I am also sure to stretch it well, including the ITB. Anyone with chondromalacia should see a good sports- and fitness-focused physical therapist, and with their blessing, try spinning!

    What is Chondromalacia?
    Definition: Chondromalacia is a softening or wearing away and cracking of the articular cartilage under the patella, resulting in pain and inflammation.
    Chondromalacia Patella is often used by physicians as a catch-all term to describe anterior knee pain that results from patellar malalignment. The strict definition, however, is simply the degeneration of the cartilage on the posterior aspect of the kneecap.
    The articular cartilage on the posterior aspect of the patella comes in constant contact with the articular surfaces of the femur during normal knee motion. (Fig. 1)

I had a case of patellar tendinitis in my knees (alternatively) and I ordered a knee wrap. I received it last night, and immediately felt a difference!     Chondromalacia Chondromalacia is a diagnosis referring to damage, and subsequent pain, to the cartilage under your kneecap. It can be caused by several factors, and produces discomfort with such activities as walking up and down stairs, kneeling, squatting, or getting up from a seated position. It can occasionally produce swelling in the knee and a sensation of giving way or "catching" ("locking"). It is a very common problem and is directly related to the amount of pressure between the kneecap and the femur bone beneath it. This pressure is normally 2-3 times your body weight when simply descending stairs; one can imagine the magnitude of the pressure when running! All the following suggestions and treatments are directed at decreasing the pressure between the kneecap and the underlying femur. The usual precipitating cause of pain in the patient with chondromalacia is either trauma (an injury such as a fall on the knee or a car accident) or a developmental abnormality (Malalignment) of the knee, which may predispose a patient to knee cap pain. Some patients suffer from a kneecap that repeatedly dislocates (usually towards the outside of the knee), which not only causes pain, but buckling of the leg and damage to the undersurface of the kneecap. There is a "normal" relationship between the thigh muscle, the patella, and the point of attachment of the patellar tendon. This is described by the Quadriceps Angle (Q-angle). In those people in which the Q-angle is larger than normal, there is a greater tendency for the kneecap to track abnormally in the groove on the front of the femur with knee bending, leading to increased pressure in certain parts of the kneecap-femur joint.
