What is chondromalacia Patella?
The term chondromalacia patella refers specifically to a pathological condition resulting in structural changes in the cartilage surface of the kneecap. These changes may be as mild as softening and discoloration of the normally firm, white glistening surface. if the process continues, fissuring and fraying may occur with the more advanced stages resulting in full thickness cartilage loss with exposure of the underlying bone. The process may be localized to one small area or it may involve the entire surface of the patella. Because of these variations in the cartilage changes, the symptoms experienced by patients with chondromalacia may vary. The patella aids the quadriceps muscle in extending the knee by improving it's mechanical advantage. The articular surface of the patella is the thickest in the body (four to six millimeters). Walking on level ground exerts a patellofemoral force equivalent to one-half the body weight, climbing stairs between 3-8 times the body weight, and rising from a full squat can generate forces eight times the body weight.
What causes chondromalacia
Although chondromalacia may develop following an acute injury to the knee (direct blow, fracture, or dislocation of the patella), more commonly, it has an insidious onset not specifically related to any one traumatic event. Any anatomical variation which results in "mal tracking" of the patella in the femoral groove during knee motion, may expose the cartilage to larger loads than it can withstand resulting in abnormal wear. Predisposing factors include anatomical variations in patellofemoral groove, tight supporting structures, rotational malalignment of lower extremity (traumatic or developmental), or altered test alignments.

Knee Problem: Chondromalacia Patella
This disease is also known as softening of the cartilage of your knee cap or "anterior knee pain". This is a big medical word for a condition where the shiny cartilage surface of your patella (knee cap) is softened due to many factors including abnormal pressure across the joint surface or hormonal changes in your body. This pain is usually located over the front of your knee and is described as a deep aching pain. It is sometimes associated with swelling and is usually worse when your knee is bent for long periods of time such sitting in a car or bus. The pain is also worse with such activities as squatting, kneeling or stair climbing (either up or down stairs). The pain is sometimes improved with rest and the use of pain medication. It is sometimes associated with mild or moderate swelling of the knee and some people report a grinding feeling in their kneecap. It is more common in younger females especially after a growth spurt where the knee must carry more weight.

Chondromalacia Patella: Treatment
Most orthopedic surgeons agree that chondromalacia should first be treated conservatively. This means without surgery. However, each person is different and your doctor may feel you need surgical treatment immediately. If you are told that you will require conservative care you can remember the plan easily by remembering the words:

Chondromalacia (pronounced KON-DRO-MAH-LAY-SHE-AH), also called chondromalacia patellae, refers to softening of the articular cartilage of the kneecap. The disorder occurs most often in young adults and may be caused by several things, including:

Instead of gliding smoothly across the lower end of the thighbone, the kneecap rubs against it, thereby roughening the cartilage underneath the kneecap. The damage may range from a slight abnormality of the surface of the cartilage to a surface that has been worn away completely to the bone. Traumatic chondromalacia occurs when a blow to the kneecap tears off either a small piece of articular cartilage or a large fragment containing a piece of bone (osteochondral fracture). Chondromalacia is a condition wherein the cartilage on the back of the patella (knee cap) is irritated and painful because it rubs against the medial femoral condyle. What are the common symptoms/complaints? Patients complain of pain felt under the knee cap on the medial side of the knee particularly while going up or down stairs and hills. Also pain and stiffness after prolonged sitting. How is it caused? During a normal gait cycle, the femur and the tibia rotate in unison (i.e. internally during pronation and externally during supination). However, Chondromalacia occurs when a person over-pronates because the tibia is locked into the Talus and therefore continues to rotate internally past the end of the contact phase (where supination starts at the beginning of midstance in a normal gait) while the femur receives its orders from the brain and begins to rotate externally at midstance. The resulting counter rotation of the femur and the tibia causes the patella to rub against the medial femoral condyle instead of moving smoothly up and down in its normal track, which causes the pain felt by the patient and the damage to the cartilage.   • Back to Encyclopedia Index Overview First used by Aleman in 1917, chondromalacia patella was the name given to a degeneration of the patellar articular cartilage (Kipnis and Scuderi, 1995). True chondromalacia patella involves the degeneration of the articular facets of the patella, with resulting ragged fronded edges, (Welsh and Hutton, 1990), and is a very common injury amongst both men and women of our sporting population (Williams, 1990). Along with osteoarthritis, chondromalacia patella is a type of cartilage damage resulting in anterior knee pain. It is also named as a cause of the symptoms of patellofemoral pain syndrome (Reid, 1992). When compared to the number of patients who are seen with patellofemoral pain, and those with actual changes to the articular cartilage of the patella, chondromalacia patella is a rare condition though (Nofthall, 1990). The knee is involved in around 10 % of all sporting injuries, (Turner and Avil, 1995). While treatment of knee complaints is not in the realm of podiatry practice, knee pathology can exert an influence on the overall lower limb’s functioning (Turner and Avil, 1995). This may result in biomechanical compensatory mechanisms in the foot, where podiatry can have a positive effect on such a scenario.

    PATELLAE Chondromalacia
    The key message with chondromalacia is that one must understand that this is not a disease. It is a word which means 'joint cartilage softening' and this diagnosis can only be made during surgery when the cartilage is probed or maybe on MRI where the cartilage may be seen to be abnormal. Too often patients are told they have 'chondromalacia' by a PT or medic just on the basis of a combination of symptoms - anterior knee pain (pain in the front of the knee), crunchy noises, pain on going down stairs. This fills them with unnecessary fear and they may start a totally inappropriate rehabilitation programme or, worse still, stop looking for the cause of their problem. A number of conditions of the knee can cause joint surface softening such as abnormal position or tracking of the patella or a plica. Arthritis Surgeons will grade the joint surface degeneration for the record. Although there is more than one classification, the principles are:
