If your player has knee pain, does it increase when he walks down stairs or down the hill? If so, he may have a medical condition called chondromalacia. Normally, a fibrous band of cartilage cushions the kneecap (patella). This tough band protects the surfaces of the bones that make up the knee, enabling the kneecap to slide along the other bones when the joint moves. The cartilage is put to use every time the leg moves; to give the kneecap a nice "ride" over the other bones, the cartilage must be smooth and just the right thickness. Chondromalacia occurs when the cartilage is thinned out or has an abnormality on its surface, causing the kneecap to ride directly on the other bones. Lack of the cartilage cushion causes knee pain. Chondromalacia is common in soccer players, skaters, skiers, and bicyclists. It can be caused by poor muscle alignment, muscle weakness, and overuse. It may also be the result of a specific injury that caused the cartilage to wear out, or which dislodged a bone fragment that made the cartilage rub. Because chondromalacia causes discomfort and restricts movement, it usually requires medical evaluation to determine its cause and corresponding treatment. In addition to allowing the cartilage to heal, treatment normally involves strengthening the quadriceps through low or no-impact exercise, such as swimming or riding a stationary bike. The type of exercise is determined by the range of motion of the joint and how far it can be bent.

Date: Thursday, January 16, 2003, @ 4:01 p.m. After years of bodybuilding and powerlifting I have finally seen the light. I have been Olifting for 3 months now and love it. Unfortunatly, my knees are showing the signs of chondromalacia. To compound the problem I am flat footed. I have already begun searching for a good set of orthotics for my shoes. I was wondering if any one else has had similiar knee problems and how did you rehabilitate your knees?

Both knees were surgically "repaired" within about a six month period. Since then, there are certain positions which tend to aggravate it more, but it has not gotten in the way of getting stronger. Chondromalacia (pronounced KON-DRO-MAH-LAY-SHE-AH), also called chondromalacia patellae, refers to softening of the articular cartilage of the kneecap. The disorder occurs most often in young adults and may be caused by trauma, overuse, parts out of alignment, or muscle weakness. Instead of gliding smoothly across the lower end of the thigh bone, the kneecap rubs against it, thereby roughening the cartilage underneath the kneecap. The damage may range from a slight abnormality of the surface of the cartilage to a surface that has been worn away completely to the bone. Traumatic chondromalacia occurs when a blow to the knee cap tears off either a small piece of articular cartilage or a large fragment containing a piece of bone (osteochondral fracture). The main symptoms of runner's knee (chondromalacia) are aching around the knee cap. This ache may be worse on climbing stairs or cycling. Runners may experience knee pain after running a certain distance when their knee seizes up and forces them to stop. Cause
Chondromalacia is caused by disproportionate development of some muscles around the knee. These muscles then pull the knee cap in the wrong direction. The back of the knee cap then starts rubbing against other parts of the knee joint and ache develops. Some patients have a predisposition to knee pain because the knee cap is small or anatomically misplaced. Treatment
This condition often gets better on its own in time, but can persist for years. You can speed up the recovery process if you follow this advice. Modify your exercise to e.g. cycling in low gears, gentle jogging, swimming etc. Do the following; Knee exercises:
  • Sitting on a bed, keep your leg flat and bend your toes towards your head as much as possible, keep this position for ten seconds and release. Repeat this ten times.

    Chondromalacia Chondromalacia is an outdated term. But from the 1930s until a decade ago or so the term "chondromalacia patellae" was synonymous with pain pertaining to the patella. It is a rather dated term and most orthopedists are abandoning it (or should be). Nevertheless it still deserves discussion. For one, no term has been universally accepted as a replacement for "chondromalacia". Secondly, in the United States it is still a recognized insurance diagnosis code (717.7). Historical Review: The concept that cartilage changes are abnormal and potentially painful is hardly new. Hunter’s admonition that ‘From Hippocrates to the present age it is universally allowed that ulcerated cartilage is a troublesome thing and that when once destroyed it is not repaired" will attest to that. Konig and Aleman were two of the first to use the term chondromalacia, though Aleman himself referred back to a 1906 text when using the term. They noted softening (malacia) of the cartilage (chondro-) and extrapolated a correlation with patellar pain. All of these investigators attributed the cartilage lesions to trauma . The concept that articular lesions could be secondary to malalignment is relatively recent.

It is caused by direct trauma or chronic injury to the knee joint. Although it is often referred to and treated as a discernible ailment, chondromalacia patellae is best thought of as a symptom. The patella contains the thickest layer of cartilage in the body. It has five facets or ridges: superior; inferior; lateral; medial; and odd.     Chondromalacia is probably the single most common condition suffered by our kneecaps, sometimes causing pain and sometimes not. "Creaky" kneecaps are generally afflicted with chondromalacia, but medical studies have shown that people with such noisy kneecaps rarely notice anything abnormal about them until they actually become painful. Congenital (inborn) problems with a person's knees such as patellar malalignment (a natural imbalance of patellar posture) can cause chondromalacia by early adulthood. Malalignment affects patellar movement patterns and causes uneven stress to be placed on the patellar joint surface, perhaps similar to the abnormal pressure and friction that produces the uneven wear seen on automobile tires that are out of alignment. Over time, this may lead to advanced patellofemoral arthritis, occasionally occurring as early as the fourth decade of life. Chronic patellar overloading caused by obesity and/or many years of repetitive knee bending/squatting also seems to be associated with premature patellar cartilage breakdown, even in knees with normal patellar alignment. Chondromalacia patella (which can be considered an early stage of localized, knee joint arthritis) will sometimes remain clinically silent (cause no symptoms) for many years, whereas in other cases it becomes annoying and obvious early on. Patellar pain, combined with crepitation (a sensation of grinding, catching or creaking as one moves the knee), is typical. Sometimes inflammation causes fluid and swelling to develop in the knee. Activities such as stair climbing, squatting and kneeling often become uncomfortable, and in rare cases, almost impossible.

During a normal gait cycle, the femur and the tibia rotate in unison (i.e. internally during pronation and externally during supination). The resulting counter rotation of the femur and the tibia causes the patella to rub against the medial femoral condyle instead of moving smoothly up and down in its normal track, which causes the pain felt by the patient and the damage to the cartilage.

    *For Medical Professionals only, registration required Chondromalacia patellae, a syndrome of pain and crepitus over the anterior aspect of the knee resulting from loss of cartilage over one or more portions of the patella. This disorder occurs in adolescents and young adults. The typical site of involvement is the medial facet of the patella, particularly about the ridge that separates the medial and odd facets. Swelling and oedema of the cartilaginous surface are present. The changes are difficult to observe on images, and histologic evaluation is needed for diagnosis. MR imaging is perhaps the best noninvasive diagnostic technique for assessment of cartilage loss, but arthrography, particularly in association with CT scanning, can be valuable ( Fig.1
