Chondromalacia
Chondromalacia (pronounced KON-DRO-MAH-LAY-SHE-AH), also called chondromalacia patellae, refers to softening of the articular cartilage of the kneecap. The disorder occurs most often in young adults and may be caused by trauma, overuse, parts out of alignment, or muscle weakness. Instead of gliding smoothly across the lower end of the thigh bone, the kneecap rubs against it, thereby roughening the cartilage underneath the kneecap. The damage may range from a slight abnormality of the surface of the cartilage to a surface that has been worn away completely to the bone. Traumatic chondromalacia occurs when a blow to the knee cap tears off either a small piece of articular cartilage or a large fragment containing a piece of bone (osteochondral fracture).
What Are the Symptoms of Chondromalacia? How Is It Diagnosed?
The most frequent symptom of chondromalacia is a dull pain around or under the kneecap that worsens when walking down stairs or hills. A person may also feel pain when climbing stairs or during other activities when the knee bears weight as it is straightened. The disorder is common in runners and is also seen in skiers, cyclists, and soccer players. A patient's description of symptoms and a followup x ray usually help the doctor make a diagnosis. Although arthroscopy can confirm the diagnosis of chondromalacia, it is not performed unless the condition requires extensive treatment.

Degenerative Chondromalacia The term chondromalacia comes from two Greek words that essentially mean cartilage softening.1 Chondromalacia describes the changes that occur in cartilage covering the ends of bones as it breaks down and degenerates. This condition is not the same as osteoarthritis, and one does not necessarily lead to the other. Osteoarthritis results from long-term wear and tear that causes the lining of the joints to become rough and painful. Chondromalacia results from an injury to otherwise healthy cartilage or a response to abnormal pressure on the cartilage. Cartilage gets its strength from an interlocking mesh of collagen fibers and proteins. When viewed through a microscope, the cartilage in a joint with chondromalacia has fewer collagen fibers and has a lower protein content, which means the interlocking mesh has begun to unwind. The lubricating fluid inside the joint also shows elevated levels of some enzymes that can dissolve proteins. The causes of chondromalacia are different for different joints. A baseball pitcher can develop it in his elbow due to overuse, and someone who constantly works with his or her arms overhead, such as an electrician can develop it in the shoulder. However, the most commonly affected joint by far is the knee. Several factors contribute to the development of knee pain due to chondromalacia. Occupations involving repeated kneeling, squat-ting, and climbing stairs or ladders can lead to knee pain. Malalignment of the leg bones causes some people to develop this problem. For example, people who are "knock kneed," or have what is called valgus alignment, have increased stress on the area between the patella (kneecap) and femur (thigh bone). This abnormal pressure causes a breakdown of the cartilage and knee pain. People who have loose joints and are hyperflexible also have abnormal stress on their patella and can develop these changes. Any injury to the knee that bruises the cartilage can lead to softening. Even a mild injury can easily interrupt the blood supply the cartilage receives from the underlying bone and can lead to death of the cartilage cells. Aging may also play a part.

Articular cartilage is a smooth, tough tissue that allows for the smooth movement when your knee bends and also functions as a protective barrier for the underlying bone. There are a variety of reasons why chondromalacia may occur. A direct injury from sports activities or an injury, even general "wear and tear" of the cartilage over a period of time can cause irritation. When the kneecap is not aligned properly, it can prevent smooth movement when your knee bends. This causes the kneecap to rub against the thighbone and it begins to roughen the articular cartilage. SYMPTOMS A patient with chondromalacia may experience pain and swelling of their knee. Bending at the knees and climbing stairs tend to irritate the problem. A person may feel grinding in their knee when bending it.

More common among active adolescents and young adults it is associated with the softening and fibrillation of articular surface of the patella. The basic disorder is to a major extent caused by mechanical overload of patellfemoral joint.
Before starting on the detail causes, symptoms and management of this condition it is essential to understand terms as cartilage, bone, the difference between them, patella and so on. What is a cartilage? Cartilage is the material that covers the ends of the bones of any joint. This white, shiny material has a rubbery consistency and is smooth than a state of ice. The function of articular cartilage is to absorb shock and provide an extremely smooth bearing surface to facilitate motion. There is articular cartilage essentially everywhere that two bony surfaces move against one another, or articulate. In the knee, the cartilage covers the ends of the femur ( large bone of the thigh), the top of the tibia (large bone of the lower leg) and the back of the patella (fourth bone of the knee joint.), or kneecap. This material is about 1/4 of an inch thick in most large joints.
Our bones are not very flexible. They aren't supposed to bend. The places where two bones come together are called joints. Cartilage protects these bones from rubbing together and causing a lot of pain.

Chondromalacia patellae occurs in two distinct age-groups:
It can happen in the older age-group (in the s and beyond) when the articular cartilage breaks down as part of the wear-and-tear process that occurs with the rest of the body. The patella cartilage is one of the earliest places where cartilage breakdown occurs, and is slowly progressive, leading to degenerative arthritis (osteoarthritis) in the knee joint.
What are the symptoms?
The typical patient is a teenage girl complaining of pain in the front of her knee around the knee-cap. She may also have pain that is deep-seated that may radiate to the back of the knee. The pain comes and goes, but usually with squatting, kneeling, and negotiating steps, especially going down the stairs. She may be engaged in strenuous sports, but experiences pain with repeated bending of the knees. Although girls are more often affected, boys can have this problem too.

WHAT NOW?CHONDROMALACIA?CAN ANYBODY PROVIDE INF (lucy) lucy, chill. chondromalacia is the least of your worries. the kneecap pain goes away. it may take months. you had a lot done to your knee. you're a diligent, conscientious medical consumer. now that summer's here, go swimming, biking, and low-impact-exercise what's left of your meniscus. then ice that baby down - i'm a big fan of deep icing to reduce acute inflammation. my secret: i freeze a styrofoam cup of water. then i peel it down like an ice cream cone and do icy circles around my whole kneecap for 10 whole minutes without removing the icecube from the knee. (that's why you use a styro-cup - so you can hold it without your fingers going numb!) it's also good to tie a towell in a knot around your calf to catch the drips. you'll think i'm crazy the first few times. and, yes, your skin turns red, but it's not frostbite! trust me.

Description

The kneecap, or patella, rides along a trough or groove at the end of the thighbone (femur), much like a bobsled down a track. The patella is attached to the strong thigh muscles on one end, and is attached via the patellar tendon to the front of the shinbone (tibia) on the other end. Thus, when the thigh muscles contract, they pull on the patella, and the leg straightens. The undersurface of the patella has a thick layer of smooth cartilage, and rides nearly friction free. Damage to the cartilage undersurface of the patella is termed Chondromalacia Patella.
Common Causes
May be due to direct trauma, such as a fall on a flexed knee. Abnormal cartilage wear may develop due to chronic maltracking or subluxation of the patella (see patellar instability). Weight gain, or other increased load on the knee can cause symptoms. Also frequently seen in adolescent girls.
Symptoms
Pain in the front of the knee. Worsened with going up and down stairs, climbing, squatting or kneeling, and prolonged standing.

Runners Knee refers to pain "primarily" on the inside of the knee, although pain on both sides or beneath the kneecap can also occur. It is more commonly seen in runners as opposed to walkers due to high level of activity, particularly mileage. The key element in this injury is due to overpronation causing a repeated stress of the patella bone moving abnormally from side to side over the groove of the thigh bone (femur), gradually softening the cartilage under the kneecap. The result is the surface of the joint can become rough. Pain may be just a prelude to further destruction of the surface (traumatic arthritis). This injury can be confused with "Patella tendonitis" which is usually pain beneath the patella bone. In either case it is the "tracking" of either the bone or tendon which needs to be addressed.
Other causes of this injury can be:
