Cold sores are caused by the herpes simplex virus type one (HSV 1). It is the most common form of the virus and causes 60 per cent of cold sores on the face and lips. Herpes simplex type two (HSV 2) usually infects the skin and mucous membranes of the genital area - hence it's known as genital herpes. About 80 per cent of adults have blood evidence of previous HSV1 infection, whereas only 25 per cent have had an HSV 2 infection, though this number is steadily rising because of the increase in sexually transmitted infections of all types.

Herpes Simplex (Cold Sores) WHAT IS HERPES SIMPLEX?
Herpes simplex is a viral infection that attacks the skin and nervous system, and usually produces small, irritating and sometimes painful fluid-filled blisters on or around the mouth and nose. WHAT CAUSES IT? Herpes simplex is caused by a virus, herpes simplex 1 (HSV 1). After the initial outbreak, the virus usually lies dormant in the skin or in nerve tissue until something triggers another eruption. Often the trigger is unknown, but in some people overexposure to sunlight, fever, physical or emotional stress, hormonal changes such as pregnancy or menstruation, or certain foods and drugs seem to reactivate the virus. WHAT ARE ITS COMMON SYMPTOMS?
The first symptoms of a herpes simplex infection usually include burning, tingling or itching sensations around the edges of the lips or nose; this is referred to as a prodromal stage. Within a few hours, small-reddened areas develop followed by the formation of small blisters filled with fluid. Several small blisters may merge to form a larger one. As the blisters enlarge, they rupture and leak sticky serum-like fluid, which rapidly crusts. In uncomplicated cases, the herpes attack usually runs its course in three weeks or less, and seldom leaves a scar. Between cold sore eruptions, the herpes simplex 1 virus lies dormant, presumably in the skin or in nearby nerve tissue.

Cold sores or fever blisters are quite common.
Approximately 30 t0 40% of the population will have a cold sore sometime in their life. In some individuals the cold sores continue to recur, especially following a fever, stress, or exposure to the sun.
The herpes simplex virus causes cold sores. This is in contrast to canker sores, which are a different type of infection, and their cause is unknown.
There are other herpes infections such as herpes zoster, that causes chicken pox and shingles.
The FDA recently approved the drug valtrex to treat cold sores. However, studies have shown that the sores in people who were treated with valtrex healed only one day sooner than those who did not receive any treatment. This is a prescription drug.

    Researchers from the U of U School of Medicine and Eccles Institute of Human Genetics have traced susceptibility to cold sores, also known as fever blisters, to a region of six genes on human chromosome 21. Herpes simplex virus 1 (HSV-1) causes the sores, but while most people are infected with this virus, only some get frequent cold sores.
    A mathematical analysis computed the odds of linkage from one of the six genes to cold sores at 2,500:1-strong enough to indicate a correlation, said John D. Kriesel, M.D., assistant research professor of internal medicine.

Also, sometimes people were infected when they were young, didn't know it (either never broke out or had a little "scab") and then, for some reason or other, break out as an adult. The way to definitively find out is to have a doctor look at the sore and do a test (many doctors will simply diagnose by looking), but there are some pretty characteristic attributes of herpetic cold-sores:

When symptoms do occur, they may include fever, runny nose, and painful lesions (fever blisters or cold sores) on the lips or in the mouth.     When the cold sets in, the body reacts with bumps and quakes.
    By Kate Robertson Standing atop Haleakala Crater at sunrise is enough to make you forget you're in Hawai'i. The cold damp air penetrates your lightweight jacket and aloha shirt in a matter of moments. If you close your eyes, you can imagine yourself perched on the Golden Gate Bridge with the dense San Francisco fog and brisk sea air cutting to the bone. Cold is cold, even if you are just a couple of hours' drive from Ka'anapali Beach. In fact, were you clad for the beach, in nothing but a bathing suit, your body would begin to feel cold as soon as the air temperature dropped below 80 degrees. So unless you are properly bundled, the occasional 45-degree breeze at the crater rim can bite. Your skin feels it first. As the nerves make the call to the thermostat in your brain, your pores stop all sweat production; evaporation of moisture adds to the cold. At this time, the blood vessels in your skin constrict to decrease the flow of warm blood to the outer surface of the body, where it would cool quickly. The hypothalamus in the brain receives the call and kicks your body into action. First, it plays defense by shifting the flow of blood to vital organs to keep them warm. That means circulation decreases in those body parts that aren't essential to life such as your fingers and toes. As your blood warms your heart and lungs, your face pales and your hands and feet get cold.
