Cor pulmonale is part of the "Pickwickian syndrome" or sometimes called "Obesity Hypoventilation syndrome". It is a rare symptom of severe obstructive sleep apena. It is usually terminal with a 1-5 year outlook possibly as much as 10 years. There are more ways of treating it then every before. Often with control of the OSA and weight loss it is possible for the enlargement of the heart to shrink. Therefore giving the patient a better quality of life. Dictionary definition: Cor pulmonale: an abnormal condition of the heart. It is marked by swelling of the right lower chamger (ventricle) of the heart. This results from high blood pressure (hypertension) of the lung circulation. Long-term cor pulmonale increases the size of the right ventricle because it cannot adjust to a rise in pressure as easily as the lift ventricle. In some patients, however, the disease also increases the size of the left ventricle. Some of the diseases linked to cor pulmonale are cystic fibrosis, myasthenia gravis, heart disease, and swelling of the lung arteries (pulmonary arteritis). Chronis obstructive pulmonary disease and emphysema are others. Cor pulmonale accounts for about 25% of all types of heart failure. The disease affects middle-aged and elderly men more than women. It may occure in children linked with cystic fiborisis, hemosiderois, scleroderma, nervous system diseases affecting lung muscles, or disorders of the breathing control center in the brain.

Cor pulmonale is also called right-sided heart failure, and is characterized by enlargement of the right ventricle. Treatment targets the underlying illness and may include supplemental oxygen, a low-salt diet or calcium channel blockers.

It is a sensitive issue as well for it correlates well with COPD and that a root cause is smoking. A present and impending danger to the health of today's society. Before getting any further, let us first set its definition: Cor Pulmonale refers to disease of the right ventricle that results from pulmonary hypertension secondary to a pulmonary disease. It is characterized by right ventricular dilatation, hypertrophy, and, eventually, failure. For its Etiology: Cor Pulmonale can occur in any condition that causes pulmonary hypertension. Acute cor pulmonale is most often due to extensive pulmonary embolization. Chronic cor pulmonale occurs most often in patients with COPD; stress in these patients may induce acute but reversible exacerbations of the cor pulmonale. Indeed, in this occasion we are going to dwell more on the latter problem. It best suits the theme as both cardio and pulmo problems are correlated. It is also a pertinent and sensitive issue of today as COPD is characterized by chronic bronchitis and emphysema which are mostly blamed on the industrialized vice of smoking. A malady inherited since the era of Christopher Columbus. Smoking is no longer just an individualized problem for it has become a social issue as well, that, in reference to the proposition on passive smoking.

In fact, about 85% of patients diagnosed with cor pulmonale have COPD. Chronic bronchitis and emphysema are types of COPD. High blood pressure in the blood vessels of the lungs (pulmonary hypertension) causes the enlargement of the right ventricle. In addition to COPD, cor pulmonale may also be caused by lung diseases, such as cystic fibrosis, pulmonary embolism, and pneumoconiosis. Loss of lung tissue after lung surgery or certain chest-wall disturbances can produce cor pulmonale, as can neuromuscular diseases, such as muscular dystrophy. A large pulmonary thromboembolism (blood clot) may lead to acute cor pulmonale. Causes and symptoms Any respiratory disease or malfunction that affects the circulatory system of the lungs may lead to cor pulmonale. These circulatory changes cause the right ventricle to compensate for the extra work required to pump blood through the lungs. The right ventricle has thin walls and is crescent-shaped. The resulting pressure causes the right ventricle to dilate and bulge, eventually leading to its failure.

Overview Symptoms Treatment Prevention Definition: Failure of the right side of the heart caused by prolonged high blood pressure in the pulmonary artery and right ventricle of the heart.
Alternative Names: Right-sided heart failure
Causes, incidence, and risk factors: Normally, the left side of the heart produces a higher level of blood pressure in order to pump blood to the body; the right side pumps blood through the lungs under much lower pressure. Any condition that leads to prolonged high blood pressure in the arteries or veins of the lungs (called pulmonary hypertension) will be poorly tolerated by the right ventricle of the heart. When this right ventricle fails or is unable to properly pump against these abnormally high pressures, this is called cor pulmonale. Almost any chronic lung disease or condition causing prolonged low blood oxygen can lead to cor pulmonale. A few of these causes include:

  • Shortness or breath
  • Symptoms of the disease that causes low oxygen (wheezing, coughing)
  • Swelling of the feet or ankles
Signs and Tests:
  • Bluish color to the skin (cyanosis)
  • Distension of the neck veins
  • Abnormal fluid collection in the abdomen, enlargement of the liver
  • Abnormal heart sounds
  • Abnormal electrocardiogram (EKG)
  • Echocardiogram
  • Chest x-ray
  • CAT scan of the chest
  • Pulmonary function tests
Treatment: Treatment is often directed at the underlying illness. Supplemental oxygen may be prescribed to increase the level of oxygen in the blood. A diet low in salt is often prescribed. Diuretics may be given to remove excess fluid from the body. Calcium channel blockers or intravenous prostacyclin may be used in primary pulmonary hypertension. Surgery may be used to reverse heart defects that cause cor pulmonale.
