Corticobasal Degeneration
Corticobasal degeneration is a progressive neurological disorder characterized by nerve cell loss and atrophy (shrinkage) of multiple areas of the brain including the cerebral cortex and the basal ganglia. Corticobasal degeneration progresses gradually. Initial symptoms, which typically begin at or around age 60, may first appear on one side of the body (unilateral), but eventually affect both sides as the disease progresses. Symptoms include signs of parkinsonism such as poor coordination, akinesia (an absence of movements), rigidity (a resistance to imposed movement), and disequilibrium (impaired balance); and limb dystonia (abnormal muscle postures). Other symptoms such as cognitive and visual-spatial impairments, apraxia (loss of the ability to make familiar, purposeful movements), hesitant and halting speech, myoclonus, and dysphagia (difficulty swallowing) may also occur. The patient is unable to walk. Symptoms vary among patients.

It is an heterogeneous disease (i.e. composed of different elements), and is characterised by a combination of cognitive impairment and movement disorder. In a review by Rinne in 1994 the authors found only 40 cases. However, CBD is difficult to diagnose. Studies have shown that a clinical diagnosis may be different from neuropathology, i.e. a diagnosis made after examining brain tissue damage which would be revealed at autopsy. Symptoms of the disease can be cognitive (relating to comprehension, judgement, memory and reason) or motor. A range of cognitive skills, including memory and concentration, are affected by CBD. Motor symptoms (problems with co-ordination of movement) are asymmetrical - i.e. they are on one side or the other. They are often described as a stiff hand or arm, or difficulty with writing. It is a difficult disease to diagnose with accuracy and so CBD is sometimes diagnosed as something else, most commonly Parkinson's disease, vascular dementia, Alzheimer's disease or Pick's disease.

Abstract
Objective:
To investigate whether atrophy of the Corpus Callosum is associated with cognitive impairment and Cerebral Cortical Hypometabolism in Corticobasal Degeneration. Design:
Prospective clinicoradiological correlation with magnetic resonance imaging and positron emission tomography in a university hospital. Patients:
Eight right-handed patients with clinically diagnosed Corticobasal Degeneration (mean±SD age, 64±8 years). Main Outcome Measures:
Midsagittal Corpus Callosum area-skull area ratio (on T -weighted magnetic resonance images), the sum of the scaled scores of the 6 subtests on the Wechsler Adult Intelligence Scale-Revised (Digit Span, Arithmetic, Picture Arrangement, Object Assembly, Block Design, and Digit Symbol), and Cerebral metabolic rate of glucose (measured with positron emission tomography by using fludeoxyglucose F 18 as a tracer). Results:
Compared with 36 age-matched right handed control subjects, the patients had significantly decreased Callosal area - skull area ratio. The reduction in this ratio was greatest in the middle half of the Corpus Callosum.

    Tau-Related Neurodegenerative Diseases:
    Corticobasal Degeneration Corticobasal Degeneration (CBD) is clinically and pathologically similar to PSP . Age at onset is typically in the 7th decade, and the predominant symptoms are motor dysfunctions, including rigidity, ataxia, dysarthria, Parkinson’s-like signs, and in some cases, supranuclear gaze palsy. These symptoms and the underlying pathology frequently are asymmetric. CBD also exhibits clinical signs of cortical involvement: constructional apraxia (inability to perform complex movements), “alien hand” sign, and frontal dementia. Neuropathology involves the basal ganglia, corticospinal tracts, and parietal and frontal cortices. In the cortex white matter is as much involved as gray. Two types of neuronal pathology are observed: ballooned achromatic neurons that lack tau inclusions, and neurons with tau inclusions that resemble Pick bodies. As with PSP, glial involvement is massive, and includes coiled bodies and glial threads in the white matter. The astrocytic pathology consists of astrocytic plaques , rather than the tufted astrocytes of PSP.
