Definition
Cri du chat syndrome occurs when a piece of chromosomal material is missing from a particular region on chromosome 5. Individuals with this syndrome have unusual facial features, poor muscle tone(hypotonia), small head size (microcephaly), and mental retardation . A classic feature of the syndrome is the cat-like cry made by infants with this disorder.
Description
Dr. Jerome Lejeune first described cri du chat syndrome in 1963. The syndrome is named for the cat-like cry made by infants with this genetic disorder. Cri du chat means "cry of the cat" in French. This unusual cry is caused by abnormal development of the larynx (organ in the throat responsible for voice production). Cri du chat syndrome is also called "5p minus syndrome" because it is caused by a deletion, or removal, of genetic material from chromosome 5. The deletion that causes cri du chat syndrome occurs on the short or "p" arm of chromosome 5. This deleted genetic material is vital for normal development. Absence of this material results in the features associated with cri du chat syndrome. A high-pitched mewing cry during infancy is a classic feature of cri du chat. Infants with cri du chat also typically have low birth weight, slow growth, a small head (microcephaly) and poor muscle tone (hypotonia). Infants with cri du chat may have congenital heart defects. Individuals with cri du chat syndrome have language difficulties, delayed motor skill development, and mental retardation. Behavioral problems may also develop as the child matures.

The cri du chat syndrome is caused by the deletion of information on chromosome 5. The cause of this rare chromosomal deletion is not known, but it is expected that the majority of cases are due to spontaneous loss of a piece of chromosome 5 during development of an egg or sperm. A minority of cases result from one parent carrying a rearrangement of chromosome 5 called a translocation. Between 1 in 20,000 and 1 in 50,000 babies are affected. This disease may account for up to 1% of individuals with severe mental retardation. Infants with cri du chat syndrome commonly have a distinctive cat-like cry. They also have an extensive grouping of abnormalities with severe mental retardation being the most important.

Children with cri du chat syndrome can lead happy, fulfilling lives as valued members of their families and communities. Many aspects of the condition described below will worry new parents a great deal but they must remember that not every child has every one of these, many of these are minor problems which can be dealt with and you will overcome your initial shock and fear of the future as you find ways to help your child develop and learn. As parents ourselves, our major concern is for other parents. We want them to know what we wished we had known when our babies were born. Not knowing caused us to experience more pain and confusion than necessary. We were often angry and frustrated as we tried to find out how to help our children. We want to give you the information you need and offer all parents and carers the opportunity to share what they have learned about living with cri du chat syndrome. We understand that knowing what can go wrong might make you want to run away but it can also prepare you in advance. That knowing what to expect helps you to anticipate and plan so the future is not so scary. And best of all, knowing what is possible gives you goals to work towards and hope for the future. Knowledge can be frightening but it can also be empowering.

What is Cri du Chat Syndrome? Cri du Chat (KREE doo SHAH) Syndrome is a chromosomal disorder where the short arm of the fifth chromosome is lost from the first cell of life. All of our genes are attached to chromosomes. So when a piece of a chromosome is lost, so is a piece of the genetic code that makes us who we are. In approximately 90 percent of cases, including Vincent's, both parents are chromosomally normal and the deletion occurs spontaneously for reasons that geneticists cannot yet explain. The syndrome is rare. Estimates of its occurrence range from one in 30,000 to one in 50,000 live births. Undoubtedly this accounts for the relative paucity of information on Cri du Chat, as compared to more commonly known chromosomal disorders such as Down's Syndrome. Like most chromosomal disorders, Cri du Chat presents an array of developmental challenges intellectual as well as physical. Mental retardation ranges from mild to severe, with most cases falling into the moderate range. Communication is the most significant challenge for persons with this syndrome. Some form of sign language is a must. Physical development is impaired primarily by low muscle tone, which underscores the importance of early intervention in the form of physical therapy. It is considered exceptional for a child with this syndrome to walk before 3 years old. Some people with the syndrome are never able to walk at all.

Treatment: No specific treatment is available for this syndrome. The mental retardation must be addressed, and counseling is recommended for the parents. If a child is found to have this syndrome, his parents should have genetic counseling and a karyotype test to make sure that they do not have rearranged chromosomes that predispose to the child missing part of chromosome 5.
