  • A 42 year old obese women who does not smoke, presents with diastolic hypertension and menstrual irregulatitoes. PE shows a full, plethoric appearing face, increased facial hair, truncal obesity, and purple stria around the abdomen with scattered echymosis over the entire body. Labs show an HgB of 18 (12-16), a WBC of 18,000 (4500-11000). The leukocyte differential shows and absolute neutrophillic leukocytosis and absolute lymphopenia and eosinopenia. CXR is normal.

Petroid Sinus Venous Sampling
In this test, catheters are threaded from the groin area up to near the pituitary gland to better determine where a suspected tumor may be. You are allowed water only from midnight the night before (nothing else to eat or drink). You will be given a light sedative, but will be awake during the procedure. You will be taken to the Radiology Department where the procedure will take place. The radiologist will place some local anesthetic into the groin on each side over the main vein that drains blood from each leg. Then a fine bore catheter will be passed up the vein, past the heart and into the major vein in the neck (the jugular vein). From there it is passed into a smaller vein that drains blood directly from the pituitary gland, known as the inferior petrosal sinus. The procedure is repeated for the other side. X-ray screening guides the radiologist to know where the catheters are positioned. A small butterfly needle is inserted into an arm vein.
Once the catheters are in place, blood samples will be taken from the right and left petrosal sinus, and an arm vein at exactly the same time. After two baseline samples, a hormone called CRH is injected into the arm vein. This increases ACTH when a pituitary gland

1) Cell Types
Chromophobes (50%) - Corticicotrophes (ACTH, b -LPH, MSH, all from pro-opiocortin) Acidophils (40%) - Somatotrophes (GH), Mammotrophes (PL) Basophils (10%) - Thyrotropes (TSH), Gonadotropes (LH, FSH) - share similar alpha subunits with different beta subunits for specificity.

MICROADENOMAS include Cushings disease (small corticotrophe tumour) and most prolactinomas.
a) Prolactinomas
In women cause menstrual irregularities, oligo-amennorrhoea, galactorrhoea, and occasionally hirsutism, and are usually microadenomas. In men symptoms are few (impotence) until large tumours cause signs. Treat with Bromocryptine. NB Prolactin release is controlled by Prolactin Release Inhibitory Factor, the only anterior pituitary hormone with an inhibitory releasing factor.
b) Acromegaly
Large hands, feet, tongue, protruding jaw, redundant facial skin cause characteristic facies. Cardiomegaly, cardiomyopathy, diabetes, hypertension, prolactin secretion, blindness, and occasionally a peripheral myopathy may occur. Occasionally burns out of its own accord, or else is treated with DXRT or surgery.
c) Cushings disease
Cushings disease is pituitary dependent inc ACTH (70% of cases of Cushings syndrome). Secretion of CRF, by cells in the median eminence of the hypothalamus, into the pituitary portal system, causes ACTH release in response to stress and diurnal rhythmn. ACTH is a 39 AA polypeptide with half-life of 15 minutes and MW of 49,000. Via a surface receptor on the zona reticularis, c-AMP is increased, causing inc uptake of cholesterol and inc glucocorticoid synthesis. Cushings sydrome is the clinical syndrome with features of prolonged high glucocorticoid levels, ie:

Cushing's syndrome
A hormonal disorder caused by prolonged exposure of the body's tissues to high levels of the hormone cortisol. Sometimes called " hypercortisolism ," it is relatively rare and most commonly affects adults aged 20 to 50. Anestimated 10-15 of every million people are affected each year.
