CVSA provides support, education and research for families, patients, and professionals who live with Cyclic Vomiting Syndrome. There is a directory of local support groups available and they provide parent-to-parent matching. CVSA publishes a biannual newsletter, Code-V , the cost is included in the annual membership. They also provide pamphlets and new parent packets at no cost that include medical reference list, medication guidelines, and fact sheets. CVSA has an eighty minute educational video available for $15. The organization collects information on physicians who treat and study condition. In some cases this information is available to members. They also provide information and referral to other providers.

Definition
Cyclic vomiting syndrome (CVS) is a rare disorder characterized by recurring periods of vomiting in an otherwise normal child.
Description
Children in the pre-school or early school years are most susceptible to CVS, although it can appear anywhere from infancy to adulthood. This disorder was identified a century ago, but its cause is still unknown. Episodes can be triggered by emotional stress or infections, can last hours or days, and can return at any time. Abdominal pain is a frequent feature.
Causes and symptoms
The cause of CVS is still a mystery. Similarities to migraine suggest a common cause, but as yet no firm evidence has surfaced. Patients can usually identify some factor that precedes an attack. Vomiting can be protracted and lead to complications such as dehydration , chemical imbalances, and tearing, burning, and bleeding of the esophagus (swallowing tube). Between attacks, there is no sign of any illness.
Diagnosis
The most important and difficult aspect of CVS is to be sure there is not an acute and life-threatening event in progress. So many diseases can cause vomitingfrom bowel obstruction to epilepsythat an accurate and timely diagnosis is critical. Because there is no way to prove the diagnosis of CVS, the physician must instead disprove every other diagnosis. This can be tedious, expensive, exhausting, and involve almost every system in the body. The first episode may be diagnosed as a stomach flu when nothing more serious turns up. Only after several episodes and several fruitless searches for a cause will a physician normally consider the diagnosis of CVS.

Issenman MD, Professor of Pediatrics, McMaster University, Chief of Pediatric Gastroenterology, Children's Hospital, Hamilton Health Sciences, Hamilton, Ontario, Canada Introduction Cyclic Vomiting Syndrome was first described about 120 years ago by Dr. Samuel Gee, the erudite British physician. Interest in the syndrome was revived when Kathleen Adams, a parent of an affected child founded the Cyclic Vomiting Syndrome Association in 1993. She enlisted the support of pediatric gastroenterologists Drs. David Fleisher and B U.K. Li, who recognized that better treatment of the disorder would only occur if the syndrome could be scientifically defined for medical researchers. The result of this effort has been three scientific meetings on the Cyclic Vomiting Syndrome. The first international meeting was in 1994 and the latter two in Milwaukee in 1998 and 2000. Definition of Cyclic Vomiting Syndrome These meetings and some 70 publications since have defined Cyclic Vomiting Syndrome (CVS) as a condition characterized by recurrent, stereotypical bouts of intense vomiting interspersed with periods of completely normal health. The average child with CVS starts bouts of cyclic vomiting at 5.2 years of age and has been affected for 2.6 years before diagnosis. Females and males are equally affected (55:45).

Background In 1994, the 1st International Symposium on Cyclic Vomiting Syndrome was held. Researchers there proposed relatively strict guidelines for the diagnosis of Cyclic Vomiting: 1. A child has recurrent, severe episodes of vomiting which may last for hours or days but have intervals of complete normal health in between vomiting episodes. 2. No other known cause of the vomiting. The reason for having these criteria is that there are children who have vomiting for unknown reasons but do not have normal healthy intervals in between episodes. These children may be vomiting every day or every other day as opposed to children with Cyclic Vomiting who tend to go for weeks without vomiting episodes. This disorder is also known as abdominal migraine . At the 2nd International Scientific Symposium on CVS in 1998, one of the major conclusions of the meeting was the recognition of the close relationship between CVS and migraine. Some children with CVS may evolve to classic migraine. Stress may be an important precipitant of vomiting for some children and psychological assessment and treatment may be helpful for children and families. During treatment of an acute episode of vomiting, close attention to the amount of fluid that is replaced is important as too much fluid as this may stimulate some of the chemicals which will perpetuate vomiting. It is important to ensure a good supply of sugar during this process.

Learning About Cyclic Vomiting

Ed note: I recently received a copy of the Orphan Disease Update, a newsletter published by the National Organization for Rare Disorders (NORD). In a column entitled "New Support Groups", the name of one group jumped out at me - the Cyclic Vomiting Syndrome Association. Although there have been more than a few reports of cyclic vomiting in some individuals with congenital rubella syndrome, I had never heard of such a syndrome. I contacted Kathleen Adams at CVSA and she sent me a whole bunch of information. The following is a summary of their fact sheet. "Cyclic Vomiting Syndrome is a rare unexplained disorder of children and some adults which causes intense suffering in those affected and their families. The disease is characterized by recurrent episodes of severe vomiting, nausea and prostration with no apparent cause. Episodes are self-limited and tend to be similar to each other in symptoms and durations. Occurrences are often not predictable and usually end in a dramatically sudden way regardless of treatment. The child is often in good health and symptom-free between episodes. Dr. Samuel Gee was the first to describe CVS in the medical literature in 1882.(1) The onset of symptoms occurs in infancy through adulthood but most commonly age 3-7. The disorder can persist for months to decades and each episode may last from hours to 10 days or more (most commonly 1-4 days). Episodes may occur several times a year to several times a month. Males and females alike are affected without regard to family situation or world-wide location. There is often a family history of migraine and the child may have allergies, asthma and/or motion sickness. The disorder frequently fades out during adolescence and also may begin to manifest as migraine headaches.

Our quest for information was prompted by families and service providers who noticed sudden or ongoing changes in health or behaviors of their family members or clients with CRS. Since this report was published, the response has been incredible. We have been in contact with families and professionals around the world whose children or clients have developed additional maladies. Mary Tuder: I'm Mary Tuder; I've got a son, Tim, who's 17, who's had cyclical vomiting since he was six.
Tim Tuder: I think the first time I was at a friend's birthday party. I was outside, I felt a bit sick, so I went inside and sat on the couch, and I think I just started vomiting up on their couch, and that was how it all started.
Mary Tuder: He would lie in his bed, as white as a ghost, with big black sunken eyes, and I actually thought he was going to die. Nothing would stop his vomiting at all.
Tim Tuder: I just felt really, really bad, and because we didn't know what was going on, it was even worse.
Rae Fry: Tim Tuder is actually fairly lucky, because unlike most people, he was diagnosed with cyclic vomiting syndrome relatively early. The first time Tim was admitted to Princess Margaret Hospital in Perth, the gastroenterologist on duty was Dr David Forbes. He'd been curious about cyclic vomiting since his days as a trainee paediatrician, so he recognised the patterns of the illness.
