Definition
Cyclic vomiting syndrome (CVS) is a rare disorder characterized by recurring periods of vomiting in an otherwise normal child. Description
Children in the pre-school or early school years are most susceptible to CVS, although it can appear anywhere from infancy to adulthood. This disorder was identified a century ago, but its cause is still unknown. Episodes can be triggered by emotional stress or infections, can last hours or days, and can return at any time. Abdominal pain is a frequent feature. Causes and symptoms The cause of CVS is still a mystery. Similarities to migraine suggest a common cause, but as yet no firm evidence has surfaced. Patients can usually identify some factor that precedes an attack. Vomiting can be protracted and lead to complications such as dehydration, chemical imbalances, and tearing, burning, and bleeding of the esophagus (swallowing tube). Between attacks, there is no sign of any illness. Diagnosis The most important and difficult aspect of CVS is to be sure there is not an acute and life-threatening event in progress. So many diseases can cause vomitingfrom bowel obstruction to epilepsythat an accurate and timely diagnosis is critical. Because there is no way to prove the diagnosis of CVS, the physician must instead disprove every other diagnosis. This can be tedious, expensive, exhausting, and involve almost every system in the body. The first episode may be diagnosed as a stomach flu when nothing more serious turns up. Only after several episodes and several fruitless searches for a cause will a physician normally consider the diagnosis of CVS.

Research, much of it developed within the past five years, suggests the disorder may be genetic and caused by some sort of neurologic malfunction or flawed stress-hormone activity. The ailment usually starts in toddlers and young school-age children, both girls and boys. It often tapers off by adolescence, though about one-third of children go on to develop migraines. Fleisher said at least 20 percent of cases begin in adolescence or adulthood.

Li BUK, Murray RD, Heitlinger LA, et al. Is cyclic vomiting syndrome related to migraine? J Pediatr 1999;134:567-72. Some children present with recurrent, severe vomiting with periods of wellness between episodes. If there is not apparent cause for the vomiting, they are diagnosed with cyclic vomiting syndrome. Li et al . reviewed their experience with 214 children diagnosed with cyclic vomiting syndrome over a 12-yr period and, for comparison purposes, divided them into two groups based on whether there was or was not a family history and/or patient development of migraine headaches. Most of the patients were in the group with an association to migraine headaches (n = 176), but there were no demographic differences between the two groups. Among the associated symptoms, abdominal pain was found more commonly in the migraine-associated group ( p = 0.015). There was also more photophobia ( p = 0.022) and withdrawal (

Hi, have the doctor's suggested Cyclic Vomiting syndrome? My daughter was diagnosed with it about 4 years ago. She would vomit everyday for about 3 weeks and stop for about one. She would also run a low grade fever before the cycle started. We have been to hell and back with this situation. She also suffers a lot with migraines now. I have a son with even a worse migraine problem. If you read material on CVS, you will find they mention treatment with a drug I can't remember the name of now, but it was unfamiliar. I began to search it out and found out it is Dramamine. Hope any of this helps. Feel free to email me if you would like to talk more. My daughter's stomach is extremely sensitive now and can hardly take any medications. Tina Next Article

b. ondansetron
c. antimigraine compounds (5HT agonists such as sumatriptan)
d. antiemetics such as 5HT3 antagonists (ondansetron)
e. phenothiazines
f. placebo 2. Clinical characteristics of CVS include a. total absence of symptoms in between episodes
b. a short 30 minutes prodrome with nausea and pallor
c. rapid resolution (within 4 hours) from onset to end of vomiting
d. 4 emeses per hour, and 2 episodes per week
e. onset during early morning (2-4 AM) and 6-8 AM is characteristic True or False 3. In about 1/3 of children with CVS, the vomiting episodes resolve as they mature and are replaced by episodes of migraine headaches. 4. Despite the high frequency of emeses, affected children remain very active during episodes. 5. The episodes usually end abruptly, resolution of symptoms is usually preceded by an end-of-episode sleep.

I learned the most about my life when I thought I was going to lose it. I learned about myself, who my real friends are and what is really important.
Last November I was sick again with my chronic illness: cyclic vomiting syndrome.* I had dealt with it often, but this time was different; it was worse. After lying on the couch for a few days because I didn't have the energy or coordination to walk, I started to believe I was dying. I couldn't keep my eyes open, feel my body, distinguish time or remember the past few days. I hadn't been able to eat or drink anything for days. Finally, I used all my energy and whispered, "I love you, Mom," and passed out.
I was surprised to wake up in the hospital, and terrified because I didn't remember going there. I had already been hospitalized for two days. I couldn't eat and could barely drink; I lived on ice for a week. Some of my friends and family called or sent flowers and candy.
I stayed in the cancer and terminal illness section of the hospital for four days. I visited with two boys my age who had been there for months. They knew they were there for the rest of their lives, which probably wouldn't be much longer. One of them didn't even have any contact with his family. I wanted to cry, but realizing they probably didn't want my pity, I didn't. Being there and seeing them made me realize how valuable family and life are, and how unimportant other things are.
When I was discharged I spent two days in bed at home. I forced myself to return to school, but with a very different attitude. I quit being so hard on myself and trying to please everyone else. Now, I just want to make myself happy. I may not be quiet and make 110% in every class anymore, but that's not the best part of life. I still work hard, but I don't take on more projects than I can handle and instead do more for myself. I don't concentrate on the quantity of friends I have; instead I look for quality. I try harder to get along with my family. Even though it's very difficult, I know it's worth it.
