printer friendly CYSTIC HYGROMA home more about us in your area conditions information ... how you can help search this site Cystic Hygromas are non-malignant malformations of lymphatic vessels, usually occurring around the head and neck region. They are usually evident by 3 years of age. They may be single or multiple. If they are large they may be detected on antenatal ultrasound scans. They are usually slow-growing and may spontaneously resolve or reduce in size. Rapid growth does occasionally occur if there is internal bleeding into the cysts or if there is a complicating infection. Large cysts present at birth may necessitate a tracheostomy if there is pressure on the airway, though this is rare. Small cysts may not require any specific treatment except to wait and see how they develop. Larger cysts which continue to grow may need to be actively treated. Laser treatment to cystic hygromas in the mouth can help to reduce their bulk to help eating. Surgical reduction of these lesions in either the mouth or neck may also be helpful. An alternative treatment has been to use an attenuated bacteria (OK432) as an injection into the cysts, which causes a local sclerosing reaction which may help to reduce the size of the lesions. No hereditary association has been identified Pre-natal diagnosis
In some cases the condition has been diagnosed through ultrasound scanning while in others it has remained undetected. This is because there is a wide variation in the extent of the condition which ranges from a small growth on the neck to a very extensive mass of growths.

Differential Diagnosis
The differential diagnosis for neck masses includes encephalocele, meningomyelocele, and teratoma. Teratomas are uncommon and are generally solid. Encephaloceles have associated calvarial abnormalities and contain brain tissue. Meningomyeloceles are uncommon in the cervical region and have associated spine abnormalities.

Discussion
Cystic hygromas, which usually occur in the neck, are most likely due to abnormal lymphatic development. Approximately half of these fetuses have monosomy X (Turner syndrome), while 10-15% of cases have trisomy 18, 21 or 13. Cystic hygromas are midline, posterior, septated, cystic masses without associated bone defects. They frequently contain a characteristic appearing midline nuchal ligament.

A 4-day-old male neonate weighing 2.8 kg presented with massive bilateral swelling of the neck, with involvement of the tongue and larynx resulting in airway obstruction (Fig. 1) . The tongue was so large that the child could not close his mouth. Emergency sub-total excision was done. Post-operatively, the child could not be extubated and required ventilatory support for 4 days. Eventually, he required tracheostomy, which needed to be kept for almost a month. The remaining part of the lymphangioma was excised at the age of six months. A cystic hygroma of the neck is soft, watery and easily transilluminated; there is no other lesion that is quite so characterstic. It consists of thin-walled cysts with flat endothelium. During surgery, it often seems that the mass has the consistency of water-filled soap bubbles and there is no end to the infiltration of normal tissue. In spite of the extensive invasion of tissue planes, these are benign lesions. About 75% of cystic hygromas are in the neck, 20% in the axilla

The majority of female fetuses with cystic hygroma probably have Turner's syndrome. This, the ultrasound detection of a cystic pouch at the back of the neck is highly suggested of monosmy x. However, nuchal hygromas are also part of other malformation syndromes, such trisomy 21, Noona's syndrome and Robert's syndrome. In addtion, a few familair observations of cystic hygromas suggest that this condition may also be inherited as an autosomal recessive trait.

    I was born with a cyst on my left shoulder and was diagnosed as Cystic Hygroma. Are there any long term health concerns that I should be aware of? I am 27 years old now and have had no reoccurance nor do I have any health problems. I read that this condition is a lymphatic malformation-is there a risk of other organs being deformed as well?

    I had an ultrasound at 11 weeks because of a cyst found on my right ovary.
    I was told that they suspect a cystic hygroma because of the size of the
    baby's neck. The doctor referred me to another doctor (for another
    ultrasound), but unfortunately I have to wait a week before I can see that
    doctor because of the holidays. Is it possible that this was a "bad"
    ultrasound and that there really isn't a cystic hygroma? Could the cystic
    hygroma be related to the fact that I had strep throat at the beg. of my pregnancy? Thank you. Dear Christine: Cystic hygroma can be misdiagnosed on ultrasound study and your physician has referred you to, I assume, a physician who is expert in interpreting ultrasound images for just this reason. Cystic hygroma is not related to a maternal infection in early pregnancy.

Cervical cystic hygroma was first described clinically in 1828 by Redenbacker and pathologically in 1843 by Wernher. They are frequently seen in the neck (75%) and axilla (20%). Most mediastinal cystic hygromas are extensions of cervical lesions, and cystic hygroma confined solely to the mediastinum is rarely encountered. We report a case of a female child with a purely extensive mediastinal cystic hygroma, outlining aspects of its unusual clinical features and management.     Recurrent Cystic Hygroma
    Vijay.R a P.Rajan b Cystic hygroma is a lymphatic malformation which usually presents in infancy and childhood. When diagnosed by prenatal ultrasound ,it is usually associated with chromosomal anomalies. Although an occasional infant may present with respiratory distress,the major problem is the cosmetic disfigurement they produce. Recurrence following surgical excision is not rare. Here,we present one such case. Miss S is a seventeen year old girl who was referred to us with the complaint of a swelling in the right side of her neck for the past six months. She gave a history of previous surgery six years back. After temporary regression, it gradually increased to its present size. On examination, scar of previous surgery was present. There is a diffuse swelling 20 X 15 cms ,occupying the posterior triangle of the neck extending to the submandibular and parotid regions, which was smooth, fluctuant, non transilluminant,adherent to the skin and deep to the sternocleidomastoid. A provisional diagnosis of cystic hygroma was made and a CT Scan obtained to exclude mediastinal extension.
