Cystic Hygroma in Adulthood
K T Chong, C L Ong ABSTRACT The majority of cystic hygromas present in childhood, usually below the age of two years( ). A case report of adult presentation is presented, with discussion of development, presentation, pathology and management of cystic hygroma. Keywords: lymphangioma, cystic hygroma, angioma INTRODUCTION Cystic hygroma presents most commonly in the paediatric age-group, with more than 90% with its first presentation below the age of two years( ). There are less than 70 reported cases in adulthood( ). Here, we present a recent case report, followed by discussion on cystic hygroma presenting in adulthood. CASE REPORT A 27-year-old Malay lady presented with a progressively enlarging right neck mass over the past four months. She denied any history of pain, discharge, voice changes, associated upper respiratory tract infection, or any obstructive symptoms caused by the mass like dyspnoea or dysphagia. Physical examination revealed a 10 x 10 cm soft, fluctuant, non-tender right cervical mass in the posterior triangle of the neck. No cervical or axillary lymphadenopathy was noted. Cranial nerves were intact. No neurological deficits were found in the right upper limb. Initial clinical impression was that of a cervical lipoma. However, aspiration of the mass revealed straw-coloured fluid, and cytology showed abundant mature lymphocytes with no malignant cells. CT scan of neck was requested, which showed a low-attentuated, well-defined cervical mass posterior to the sternocleidomastoid muscle, which extends inferiorly to the lateral aspect of the right clavicle (

ABSTRACT
Objective : To discuss the case of a 12-year-old female presenting with a mass in the right submandibular region consulting for chiropractic evaluation. Clinical features : A 12-year-old female, accompanied by her mother presented for chiropractic consultation regarding a diagnosis of cystic hygroma. Her mother stated that a mass had developed in her daughter's right submandibular muscle at approximately 5 years of age. Since the age of nine she had four surgical procedures in an effort to resolve the mass and the severe sinus drainage she experienced. All surgical procedures had failed to relieve her severe sinus drainage or contain the development of the mass. The mass was approximately the size of a golf ball on her first visit to the chiropractor. The subject has also suffered from daily morning neck pain and headaches for the past year. Intervention and Outcome
Conclusion
: A case report of a 12-year-old female with cystic hygroma is presented. Integral to her condition is the physical exam finding of significant left C2 lamina pedical point tenderness and associated paravertebral muscle spasms resulting in loss of vertebral motoricity of the cervical vertebral units. The care and treatment was approached with the hypothesis that reduction of the subluxation complex of the upper cervical region may result in improved sinus function and or lymphatic drainage. The successful resolution of the patient's cystic hygroma while under chiropractic care indicates the need for further study of the role of chiropractic care in the case of non-musculoskeletal conditions.

I am looking for survivors of Lymphangioma Cystic Hygroma, people whose children have had the surgery and the growth never returned. My son is 4 months old and just had the surgery, but the surgeon says it will never go away and he will always have this, are there any miracles out there. Last changed: November 14, 2002

Background This is a benign proliferation of lymph vessels. Unlike its cousin, the hemangioma, these tumors are rare. However, they are important to recognize because they may mimic many vascular tumors including angiosarcomas and Kaposi's sarcoma. These tumors may occur anywhere but usually present in the head and neck in children and infants. Visceral organs such as the lungs and gastrointestinal tract may also be involved. Radiological Diagnosis Omphalocele with Enteric Duplication Cyst Omphalocele (exomphalos) is a defect in the anterior abdominal wall in which there is extrusion of the abdominal contents into the base of umbilical cord. The herniated mass is covered by peritoneum. The umbilical cord inserts into the sac rather than in the anterior abdominal wall as in Gastroschisis [1]. The incidence of omphalocele is 2.5 in 10000 births[1]. Omphalocele is associated with other anomalies in 50 to 70% of cases. In prenatal studies at least 50% of fetuses have associated anomaly. Chromosomal anomalies occur in 40 to 60%. 50% of associated anomalies affect the heart. Omphalocele rarely may be associated with ectopic heart (Pentology of Cantrell) [2]. Other anomalies like cystic hygroma, micrognathia, hemivertebra, neural tube defects etc, occur in 10 to 30% of cases[2]. In the present case there was septated cystic lesion adjacent to neck suggestive of cystic hygroma (Fig.3). A thin walled, multiseptated cystic structure near the fetal head and neck and eccentrically situated with respect to the long axis of the fetus is characteristic of cystic hygroma[5].

Cystic Hygroma Discussion Introduction
This malformation is commonly localized in the nuchal region (75% of cases). An additional 20% are found in the axilla, while the remaining 5% are found in the mediastinum, retroperitoneum, abdominal viscera, groin, bones and scrotum. Pathogenesis
There are at least three theories to explain the origin of cystic hygroma.
The first theory suggests that an early jugular-lymphatic obstructive sequence could cause hydrops fetalis, pterigium colli and cystic hygroma. This obstruction impedes communication between the jugular lymphatic sacs and the internal jugular vein. Some believe cystic hygroma is caused by an abnormal embryonic sequestration of lymphatic tissue and its subsequent failure to join normal lymphatic channels, while a third theory suggests that abnormal budding of the lymphatics occurs between the sixth and ninth weeks of gestation. These then canalize to form lymph-filled cysts.
