NORMAL URINATION:
The ability to hold urine and maintain continence is dependent on normal function of the lower urinary tract, the kidneys, and the nervous system. Additionally, the person must possess the physical and psychological ability to recognize and appropriately respond to the urge to urinate.
The process of urination involves two phases: 1) the filling and storage phase, and 2) the emptying phase. Normally during the filling and storage phase, the bladder begins to fill with urine from the kidneys. The bladder stretches to accommodate the increasing amounts of urine. The first sensation of the need to urinate occurs when approximately 200 milliliters of urine is stored. The healthy nervous system will respond to this stretching sensation by alerting you to the need to urinate while also allowing the bladder to continue to fill. The average person can hold approximately 350 to 550 milliliters of urine. The ability to fill and store urine properly requires a functional sphincter (muscle controlling output of urine from bladder) and a stable bladder wall muscle (detrusor).
The emptying phase requires the ability of the detrusor muscle to appropriately contract to force urine out of the bladder. Additionally, the body must also be able to simultaneously relax the sphincter to allow the urine to pass out of the body.

Cystourethrocele - is a combined "cystocele" and "urethrocele" in other words there is a loss of support for the whole anterior vaginal wall. The main supportive layer known as the pubocervical fascia is no longer supporting the bladder or urethra appropriately. Cystourethrocele - as seen on vagina inspection Vaginal and Uterine Support (side view) - Note the upper pubocervical fascia supports the bladder and urethra, the lower rectovaginal fascia supports the rectum. Specifically the rectovaginal fascia keeps the rectum from protruding into the vagina. Both of the pubocervical and rectovaginal fascia attach to the uterus which in turn attaches to the uterosacral ligaments. There is a continuous supportive structure (fascia) from the opening of the vagina to the uterosacral ligaments. Anterior Vagina Normal Support - (aerial view) The bladder and urethra sit on the pubocervical fascia. The pubocervical fascia is attached laterally to the arcus tendineus on both sides.

CYSTOCELE
CYSTOCELE AND/OR UTERINE PROLAPSE A cystocele or uterine prolapse is not life-threatening. It does not cause or become cancerous. Attempted correction of these situations is actually elective. If a woman notices these symptoms, she should see her physician for evaluation. Treatment may involve trying to keep the bowel contents soft so there is less bearing down at the time of a bowel movement. Certain exercises to strengthen the muscles of the area called Kegel exercises may be prescribed. Surgical correction is often available. A pessary is a device that is placed in the vagina to help hold up the bladder and uterus. Your doctor may discuss this with you and fit you with a pessary. When a pessary is in place, the patient should not be able to feel it. If you have further questions concerning cystocele and/or uterine prolapse, please contact our office to make an appointment. Protective effect of suburethral slings on postoperative cystocele recurrence after reconstructive pelvic operation

OBJECTIVE: The purpose of this study was to evaluate the independent effect of suburethral sling placement on the risk of cystocele recurrence after pelvic reconstructive operation.
Study Design: One hundred forty-eight women with cystoceles to or beyond the hymenal ring underwent pelvic reconstructive operation, with or without incontinence procedures, and were evaluated at 12 and 52 weeks after operation with a standardized pelvic examination. Rates of recurrent prolapse, at all sites, were statistically compared between subjects with and without suburethral slings. A multiple regression analysis was used to determine the independent effect of sling placement on the risk of recurrent cystoceles.
RESULTS Suburethral sling placement was associated with a 54.8% reduction in the mean rate of postoperative cystocele recurrence (P =.004). This protective effect was observed as early as 12 weeks and remained significant at 1-year follow up (42% vs 19%). A markedly reduced risk of cystocele recurrence was observed when women with sling procedures were compared with all other women, with those women who underwent other incontinence operations, and even with those women who had undergone prolapse repair with no incontinence procedure. The protective effect of the sling procedure remained highly significant (odds ratio, 0.29; P =.0003), even after controlling for potentially confounding variables in a multiple logistic regression model.

Cystocele is the name for a hernia-like disorder in women that occurs when the wall between the bladder and the vagina weakens, causing the bladder to drop or sag into the vagina. What are the results of a cystocele?
In addition to discomfort, the resulting dropped bladder can cause two kinds of problems to occur, including the following:
