Background: Dowling first delineated this genodermatosis as a distinct entity in 1938. In 1954, Degos and Ossipowski described a patient with a similar case. Few patients with reticulate pigmented anomaly, also known as Dowling-Degos disease (DDD), have been reported. DDD is characterized by flexural pigmented reticulate macules and sometimes comedolike papules on the back and/or the neck (dark dot follicles). Some patients have pitted perioral scars. Pruritus of affected flexural areas may be the only symptom. In both male and female patients, pigmented reticulate macules may also be evident on the genitalia, where they may be seen alone. The pigmented eruption on the male external genitalia may be a cutaneous marker of underlying testicular carcinoma (Schwartz, 1980), although the association is probably fortuitous. The disease is slowly progressive. It is characterized by pigmented filiform epidermal papules closely resembling an adenoid seborrheic keratosis, but similar proliferations also develop around the variably dilated pilosebaceous follicles.

We report the first case of reticulate pigmented anomaly of the flexures (Dowling-Degos disease) associated with hidradenitis suppurativa. Our patient also showed a squamous cell carcinoma of the groin, which is a rare complication of hidradenitis suppurativa. The relationship between Dowling-Degos disease and hidradenitis is discussed and the literature on squamous cell carcinoma and hidradenitis suppurativa is reviewed. A patient with hidradenitis suppurativa (HS) is described in association with Dowling Degos disease and perianal squamous cell carcinomas. As the initial treatment for HS failed to give any satisfactory relief he sought no further medical help for the subsequent 40 years. This patient illustrates the management difficulties in HS and the need for regular surveillance to exclude the development of anogenital squamous cell carcinoma. Two patients with penile keloids: a review of the literature.

Department of Plastic and Reconstructive Surgery, Marmara University School of Medicine, Altunizade-Istanbul, Turkiye. Keloid formation on the penis is exceptionally rare even though the penis is frequently subjected to surgical manipulations such as circumcision. Review of the literature revealed a few reports of patients with penile keloids. A 13-year-old Caucasian boy who had a pronounced penile keloid after circumcision and a 56-year-old Caucasian male who had keloid formation on the dorsum of the shaft of the penis after several attacks of penoscrotal hidradenitis suppurativa are presented. They comprise the fifth and the sixth cases of keloid formation in the literature to our knowledge. Intralesional steroid injection and surgical excision are the treatment modalities for this malady. Surgical excision appears to be the choice of treatment for larger lesions that are disabling. Recurrence is still possible, of course.

Reticulated pigmented anomaly of the flexures. Dowling-Degos disease in association with acne inversa
Zusammenfassung Abstract Reticulated pigmented anomaly of the flexures (Dowling-Degos disease) is a rare autosomal-dominant genodermatosis with variable penetrance that is characterized by reticulated hyperpigmentation of the flexures. Acne inversa has been previously described in the literature under several synonyms, such as acne triad, acne tetrad and hidradenitis suppurativa. Acne inversa is an inflammation affecting the pilosebaceous units of the flexures. We report a case of Dowling-Degos disease in association with acne inversa. The association of these diseases has been described several times and it appears not to be coincidental. Pathogenetic mechanisms leading to this coincidence are discussed.
