What is Dengue Fever?
Dengue Fever is an illness caused by infection with one of four serotypes of a virus belonging to the genus Flavivirus. Infection with one of these serotypes does not provide cross-protective immunity, so a person living in an area where Dengue Fever transmission occurs can have four dengue fever infections during his or her lifetime.
Who gets Dengue Fever?
Dengue Fever occurs primarily in tropical areas, including the Caribbean and Mexico in North America. Anyone living in or visiting tropical areas may become ill with Dengue Fever.
How is Dengue Fever spread?
Dengue Fever is spread through the bite of certain species of mosquitoes, but only if they are infected with the Dengue Fever virus. Capable species of mosquitoes pick up the Dengue Fever infection by biting a human who is ill with Dengue Fever. Two capable mosquito vectors are present in the Americas, including the United States, Aedes aegypti, and Aedes albopictus. Nearly all cases of Dengue Fever which are diagnosed in the United States are a direct result of recent travel to tropical areas outside the United States.

In Florida, we are continually reminded about the possibility that Dengue Fever and other arthropod-borne diseases could be brought into the state by infected mosquitoes arriving on airplanes. Most recently there is some mystery about the introduction of West Nile Virus into New York. Could it have been brought in by a stowaway mosquito? T. H. D. Griffitts, M.D., Director of Malaria Control Studies for the State Board of Health penned an interesting article on the subject that appeared in the February 1933 issue of Florida Health Notes. Some excerpts follow:

Owing to the potential danger, at least, of the spread of yellow fever foci of infection in one country to another by means of the rapid transit by aircraft, the United States Public Health Service has carried on some exact experiments to determine how long mosquitoes will remain aboard airplanes. Clinical Description
Dengue fever is an acute febrile illness characterized by frontal headache, retro-obital pain, muscle, joint pain and rash. The principal vector is the Aedes aegypti mosquito. Breeding and transmission usually occur in tropical or subtropical countries. Severe manifestations, eg. dengue haemorrhagic fever and dengue shock syndrome are rare but can be fatal.
Incidence 2.5 billion people worldwide are at risk with 60 million cases, and a case fatality of 30,000 yearly reported. Virology 4 serotypes have been identified: DEN 1, DEN 2, DEN 3 and DEN 4. Serious disease occurs with DEN 2, DEN 3, and in second attacks due to enhancing antibodies. Transmission The disease is transmitted by the Aedes mosquito, Aedes aegypti. It is a day biter and breeds indoor and outdoor in clean water. All eggs laid by an infected female Aedes mosquito are infected. The mosquito has a flight of 200 meters.

By Kelly Simmons, Managing Editor and Senior Writer The mosquito that is a carrier of the disease dengue fever has been detected in 3 out of 10 homes in selected colonias in the border state of Chihuahua, according to Cesar Ponce, the Chief of Epidemiology and Preventive Medicine with the state Secretary of Health. The insect has been detected in seven colonias in the capital city and twelve in Ciudad Juarez after a thorough search by health officials. It is estimated by the Health Department that 35 percent of the community living in the affected colonias in Cd. Chihuahua could contract the disease, if it were present in the local mosquito population. The increased risk of dengue fever has been blamed on record rains throughout the region over the past two months. The rains have exacerbated conditions under which the virus spreads. At this point, the Health Department has not received any reports of cases of dengue fever, nor have any mosquitos been found who have been infected with the disease, but efforts to eradicate the carrier have begun. The mosquito, Aedes Aegyptes, is a small black and white insect with stripes on its torso and legs. It carries two types of dengue fever: classic dengue and hemorragic dengue. This mosquito, like others, breeds in rain puddles and standing water that collects inside automobile tires, flower pots, and refuse. It rests inside homes, closed buildings, and other dark places such as dairy barns, sinks, tubs, and abandoned structures.

    Graphic: How the Virus Spreads As a TIME correspondent in Mexico, India, Pakistan and other countries where rogue germs abound, Tim McGirk has had his share of firsthand epidemiological experience. But dengue fever holds a special, painful place in his memory. "There's a reason they call it break-bone fever," says McGirk, who contracted the virus in New Delhi and suffered for three weeks. "You feel like there's this deep, painful itching happening inside your bones. You're on this horrible roller-coaster ride of hot spells and chills, like you're being shaken around. It just racks your body." Millions of others are being taken on the same ride. According to the World Health Organization (WHO), dengue infects more than 100 million people a year—and the mosquito-borne sickness is on the rise. Once confined to tropical regions like Southeast Asia, 100 countries are now classified as dengue risk areas, up from nine prior to 1970. "Dengue is being seen in places it hasn't been seen before," says Dr. John Simon, a tropical disease specialist in Hong Kong, where a furor over the fever has been roiling local newspapers since the first locally contracted case was reported last month. "Dengue is a huge problem."

    The disease is typified by the sudden onset of a severe fever, chills, headache, pain behind the eyes (retro-orbital pain), photophobia, and joint and muscle pain. The victim may appear flushed at the onset of the fever and a rash may appear on the torso after 3-4 days and spread to the rest of the body. Lymph nodes are often inflamed.

    What is dengue fever?
    Dengue fever, an acute mosquito-borne febrile disease, is caused by the dengue viruses. The disease is mainly tropical in origin but occasionally residents or visitors from other countries may arrive in this country with dengue fever. Although cases originating in the United States are virtually unknown, outbreaks have recently been reported in parts of the Caribbean and Central and South America. Who gets dengue fever?
    Dengue fever may occur in people of all ages exposed to infected mosquitoes. The disease primarily occurs in tropical Asia and the Caribbean, usually during the rainy seasons in areas with high numbers of infected mosquitoes. How is dengue fever spread?
    Dengue is spread by the bite of a mosquito infected with a dengue viruses. What are the symptoms of dengue fever?
    Dengue fever is characterized by high fever, severe headache, backache, joint pains, nausea and vomiting, eye pain and rash. Dengue hemorrhagic fever is first characterized by a fever that lasts from 5 to 7 days with symptoms that can occur with many other illnesses (e.g., nausea, vomiting, abdominal pain, and headache). This stage is followed by hemorrhagic manifestations, tendency to bruise easily or other types of skin hemorrhages, bleeding nose or gums, and possibly internal bleeding. Circulatory failure and shock may occur, which if untreated, can result in death.

    Dengue fever is a disease caused by one of a number of viruses that are carried by mosquitoes. These mosquitoes then transmit the virus to humans.
    Description
    The virus that causes dengue fever is called an arbovirus, which stands for arthropod-borne virus. Mosquitoes are a type of arthropod. In a number of regions, mosquitoes carry this virus and are responsible for passing it along to humans. These regions include the Middle East, the far East, Africa, and the Caribbean Islands. In these locations, the dengue fever arbovirus is endemic, meaning that the virus naturally and consistently lives in that location. The disease only shows up in the United States sporadically. In order to understand how dengue fever is transmitted, several terms need to be defined. The word "host" means an animal (including a human) that can be infected with a particular disease. The word "vector" means an organism that can carry a particular disease-causing agent (like a virus or bacteria) without actually developing the disease. The vector can then pass the virus or bacteria on to a new host.

    Hawaii reports outbreak of dengue fever
    Local health officials are working to control the spread of first endemic transmission of the tropical disease since World War II.
    By Victoria Stagg Elliott AMNews staff. Oct. 15, 2001. The Hawaii Dept. of Health is struggling to control an outbreak of dengue fever in a rural area of east Maui, according to the state agency. Since the middle of September, 18 cases have been confirmed. None of those with the infection had a history of travel to dengue fever areas. The outbreak is being considered the first endemic transmission on the island in more than 50 years. With this article Dengue fever: rare and difficult to diagnose Links "We'd never seen folks who hadn't traveled become infected," said Paul Effler, MD, MPH, a state epidemiologist in Hawaii. "It's puzzling to us. Why this time? Why this location?" The new situation is being watched closely by public health officials around the country, including Texas. Although there have been no reported cases in the state, this year or last, recent outbreaks south of the border have the health department there braced for possible impending trouble.

    Dengue Fever Outbreak Hits Islands

    News Editor An outbreak of a virus that causes victims to suffer severe symptoms has been reported on four major islands. The Department of Health is concerned with more than a hundred suspected cases of Dengue Fever being reported on Maui, Kaua'i, O'ahu, and the Big Island. Although laboratory test have confirmed only thirty-four cases on Maui and one case on Kauai, the outbreak has health officials worried. The virus is believed to have been introduced to Hawai'i by people vacationing in Samoa and French Polynesia then returning to Hawai'i after being infected. Dengue fever is a virus that is spread by mosquitoes, more specifically the Aedes aegypti, a day-biting insect that prefers humans. The disease cannot be spread by direct human contact, and usually is not fatal. Only about 5% of the cases result in death, and the fatality risks are highest in children and young adults. Symptoms of Dengue include fever, severe headaches, eye pain, chills, body and joint pains, nausea and/or vomiting, transient rashes, and occasionally shock and hemorrhage. Anyone exhibiting these symptoms is urged to see a doctor immediately. The shock and hemorrhage can lead to death. Currently, there is no vaccine available for Dengue fever.

    Texas Records Dengue Fever Death
    The Texas Department of Health (TDH) has confirmed that a South Texas girl has died from dengue fever. The girl's death earlier this month is the first caused by dengue fever in Texas in several decades. Health officials believe she contracted the viral mosquito-borne illness during a visit to Mexico. TDH officials said the girl died from dengue hemorrhagic fever, a variation of dengue fever marked by internal bleeding. They said medical confidentiality requirements prevented the release of more specific information about the girl. TDH has confirmed 51 cases of dengue fever this year. Health officials have determined that 16 of the cases were contracted in South Texas. The other 35 cases are believed to have been contracted in Mexico or Brazil. The South Texas girl who died is the only confirmed case of hemorrhagic dengue fever in the state this year. The 16 Texas-acquired cases of dengue fever this year were contracted in Cameron, Hidalgo, Starr, Webb and Willacy counties. Dengue fever is prevalent in many South and Central American countries and in Asia. Texas is the only state in the United States that has recorded locally-acquired cases of dengue fever in recent years.

It's an epidemic on Maui that threatens the entire state. The Education Director for the Maui County Department of Health says this outbreak on Maui most likely originated from off-island. GI: "It had to come from somewhere else because right now what we're looking at is transmission of people getting the disease here - the infection here - who have not gone anywhere. So that could be someone from some place other than here. Probably some other country." JK: And it really could not have happened at a worse time.
