Normal physiology L-Arginine vasopressin (AVP, ADH), a nonapeptide, is synthesized in the bodies of magnocellular neurons in the paired supraoptic nuclei and paraventricular (lateral to the 3 rd Other effects of AVP: acts at a pressor agent at supraphysiologic levels (as in severe hypovolemia) by binding V1 receptors on blood vessels, which increase intracellular calcium and cause smooth muscle contraction. Regulation of AVP release: Integration of AVP secretion and thirst maintains plasma osmolality tightly at 280-290 mOsm/kg.
  • Plasma osmolality: Anterior hypothalamic osmoreceptors (anterior to the 3 rd Plasma volume: Baroreceptors (stimulated by hypervolemia, inhibited by hypovolemia) inhibit AVP release via cranial nerves IX, X. The atrial cardiopulmonary low-pressure baroreceptors are less sensitive than osmoreceptors, requiring a 5-10% decrease in blood volume before AVP is released. Severe hypovolemia, however, triggers the sino-aortic high-pressure baroreceptors to cause exponential increases in AVP, which may be high enough to exert a pressor effect. Interaction of osmo- and baroreceptors: A decrease in left atrial pressure (as in hypovolemia, hypotension) leads to a reduction of the osmotic threshold and increases the sensitivity for osmotic AVP release. Volume expansion dampens the sensitivity for osmotic AVP release.

    Diabetes Insipidus
    Diabetes inspidus (DI) is a disorder that causes increased urination ( polyuria ) and increased thirst . It is caused by either a decrease in the hormone arginine vasopressin ( central DI ), or because the kidney does not respond correctly to the hormone ( nephrogenic DI ). Arginine vasopressin is a hormone that normally stimulates the collecting ducts of the kidney to reabsorb water, so if it is not able to work correctly, the kidney produces a very large volume of dilute urine. Arginine vasopressin is normally produced in the hypothalamus , and so there can be a decrease in the its production caused by any condition that disrupts the hypothalamus, including brain tumors and head trauma, and some inherited conditions (septo-optic dysplasia). Nephrogenic DI can either be inherited or it can be a side effect of some medications, including Lithium, and some chronic conditions, such as sickle cell anemia, pyelonephritis, and urinary tract obstructions.

    • CAT - Daily intake in excess of 100 mL/kg for a cat is abnormal. This would be about 12 oz or 1.5 cup of water for an 8 pound cat.
    • DOG - Daily intake in excess of 90 mL/kg for a dog is abnormal. This is about 1.38 oz per pound … so if your dog weighs 23 pounds, consumption of more than a quart of water a day would be abnormal. • Urine has low specific gravity (SG): usually less than 1.012 and often less than 1.008

    When we adopted her in 1990, she seemed like a normal enough three year old cat. She was shy but sweet. She purred and napped. She loved tummy rubs. Then, about three years after she came to live with us, she started drinking. A LOT! We took her to the vet. Maybe a kidney infection … so we tried antibiotics. She was worse. She seemed to live in her drinking bowl. She slept next to it. She demanded ice every time the refrigerator door opened. She lost nearly a third of her body weight!     Diabetes insipidus (DI) is an uncommon condition that occurs when the kidneys are unable to conserve water as they perform their function of filtering blood.     (Central Diabetes Insipidus; Vasopressin-Sensitive Diabetes Insipidus)
    A temporary or chronic disorder of the neurohypophyseal system due to deficiency of vasopressin (ADH) and characterized by excretion of excessive quantities of very dilute (but otherwise normal) urine and by excessive thirst. Central or vasopressin (ADH)-sensitive diabetes insipidus, which is a hypothalamic-pituitary disorder, is referred to in this discussion as diabetes insipidus (DI) to distinguish it from nephrogenic diabetes insipidus (NDI), in which the kidney is ADH-resistant (see Ch. 229 ). Polyuria may result from DI (a deficiency of ADH), from NDI, or from compulsive or habitual (psychogenic) water drinking (physiologic suppression of ADHalso termed primary polydipsia or dipsogenic diabetes insipidus).
    Etiology and Pathophysiology
    DI may be complete or partial, permanent or temporary. All of the pathologic lesions associated with DI involve the supraoptic and paraventricular nuclei of the hypothalamus or a major portion of the pituitary stalk. Simple destruction of the posterior lobe of the pituitary leads to temporary, unsustained DI. The posterior lobe is the major site of ADH storage and release, but ADH is synthesized within the hypothalamus. Newly synthesized hormone can still be released into the circulation as long as the hypothalamic nuclei and part of the neurohypophyseal tract are intact. Only about 10% of neurosecretory neurons must remain intact to avoid central DI.

    An inability to concentrate urine due to lack of response of the renal tubules to ADH, with otherwise normal renal function. (See also Diabetes Insipidus in Ch. 7.) Nephrogenic diabetes insipidus (NDI) is an X-linked, probably recessive hereditary disease. Homozygotes (all males) are completely unresponsive to ADH (vasopressin). Heterozygous females show normal or slightly impaired responsiveness to ADH. Acquired NDI
    Pathophysiology
    Normally, the kidney alters the concentration of urine to maintain plasma osmolality and ECF volume. In the presence of ADH and a medullary osmotic gradient (established by the countercurrent mechanism), water resorption by the collecting duct is increased and a concentrated urine is excreted. Without ADH, a large amount of dilute urine is excreted. In NDI, formation and secretion of ADH by the posterior pituitary are normal, but the nephron is unresponsive to the hormone.

    Diabetes insipidus (DI) results from abnormalities in the production or use (two main types) of the hormone arginine vasopressin. The main symptoms are excessive thirst and massive urination. The excess urine flow is devoid of sugar. There are no blood glucose abnormalities, and in fact there is nothing in common with diabetes mellitus except the excess urination when untreated. Diabetes insipidus caused by failure to produce vasopressin. This is known as neurogenic DI (or central DI, or pituitary DI). It can be treated with hormone replacement (by nasal spray or other routes). DI caused by failure to use vasopressin (nephrogenic DI) is more difficult to treat, but several drugs are available which help. DI is much less common than diabetes mellitus, though a few people have discussed it on misc.health.diabetes

Information Diabetes Insipidus Background Information Diabetes insipidus should not be confused with diabetes mellitus which is linked to high blood sugar. Diabetes insipidus is not. Diabetes insipidus is a disease that is caused by a decreased amount of ADH (the anti-diuretic hormone) which is also known as vasopressin. The two main types of diabetes insipidus are central and nephrogenic. Central diabetes insipidus occurs when there is a decreased production of ADH by the posterior pituitary. ADH is usually secreted by the posterior pituitary into the bloodstream where it acts upon the kidneys to maintain normal water balance. Patients with central diabetes insipidus may have the disease if they have pituitary tumors or infections because compression could occur or there could be side effects of surgical exploration. Patients having meningitis may have central diabetes insipidus. Nephrogenic diabetes insipidus has a normal production of ADH, or even an increased production, yet the kidneys are resistant to the hormone. This kind of diabetes may be hereditary or may be brought on by such medicines as lithium. Symptoms Symptoms can occur quite abruptly, often after some form of trauma. When this occurs there is severe thirst, polyuria and nocturia. There is often a large output of dilute urine. There is often high volume urination at night of this watery urine and sometimes dehydration occurs with bouts of dizziness.
