Diabetes insipidus is a condition caused by lack of antidiuretic hormone (ADH). In children, diabetes insipidus is most often caused by destruction of the pituitary by a tumor. Systemic disease such as encephalitis, tuberculosis, serious systemic fungal infections, and leukemia are also occasional causes. There are rare hereditary cases as well. Severe head trauma or brain surgery can cause temporary or even permanent diabetes insipidus. A child with full blown diabetes insipidus loses water in the urine at a prodigious rate (1 to 3 gallons a day); this water must be replaced by drinking or death would rapidly follow. Truly insatiable thirst is the result. Children with diabetes insipidus drink equally prodigious amounts of water. Deprived of access to normal sources, they will literally drink from the toilet if necessary. Treatment of diabetes insipidus is by hormone replacement. This has been made infinitely more practical by the availability of synthetically produced ADH, either as a nasal spray or in tablets (these preparations of course are most often prescribed in pediatric medicine for the treatment of bedwetting

What is diabetes insipidus?

Diabetes insipidus is a condition that results from insufficient production of the antidiuretic hormone (ADH), a hormone that helps the kidneys and body conserve the correct amount of water. Normally, the antidiuretic hormone controls the kidneys' output of urine. It is secreted by the hypothalamus (a small gland located at the base of the brain) and stored in the pituitary gland and then released into the bloodstream. ADH is secreted to decrease the amount of urine output so that dehydration does not occur. Diabetes insipidus, however, causes excessive production of very diluted urine and excessive thirst. The disease is categorized into groups:
  • central diabetes insipidus - insufficient production or secretion of ADH; can be a result of damage to the pituitary gland caused by head injuries, genetic disorders, and other diseases.
    nephrogenic diabetes insipidus - lack of kidney response to normal levels of ADH: can be caused by drugs or chronic disorders, such as kidney failure, sickle cell disease, or polycystic kidney disease.

Causes, incidence, and risk factors: Antidiuretic hormone ADH vasopressin ) is a hormone produced in the hypothalamus of the brain. It concentrates the urine by triggering the kidneys to reabsorb water into the blood stream rather than excreting water into the urine.
Nephrogenic diabetes insipidus involves a defect in the kidney tubules (the portion of the kidneys that causes water to be excreted or reabsorbed). The specific kidney defect is usually a partial or complete failure of special receptors located on or within the kidney tubules to respond to ADH, the hormone that transmits the instruction to concentrate the urine to the inside of the cells.

Diabetes insipidus (DI) is characterized by excretion of large amounts of dilute urine, which disrupts your body's water regulation. To make up for lost water, you may feel the need to drink large amounts of water. You are likely to urinate frequently, even at night, which can disrupt sleep or, on occasion, cause bedwetting. Because of the excretion of abnormally large volumes of dilute urine, you may quickly become dehydrated if you do not drink enough water. Children with DI may be irritable or listless and, in some cases, may have fever, vomiting, or diarrhea. DI should not be confused with diabetes mellitus, which results from insulin deficiency or resistance. Diabetes insipidus and diabetes mellitus are unrelated, although they can have similar signs and symptoms, like excessive thirst and excessive urination.
Damage to the pituitary gland can be caused by different diseases as well as by head injuries, neurosurgery, or genetic disorders. To treat the resulting ADH deficiency, a synthetic hormone called desmopressin can be taken by an injection, a nasal spray, or a pill. While taking desmopressin, you should drink fluids or water only when you are thirsty and not at other times. This is because the drug prevents water excretion and water can build up now that your kidneys are making less urine and are less responsive to changes in body fluids.

Causes, incidence, and risk factors: Diabetes insipidus (DI) is an uncommon condition that occurs when the kidneys are unable to conserve water as they perform their function of filtering blood. The amount of water conserved is controlled by antidiuretic hormone (ADH, also called vasopressin). It is a hormone produced in a region of the brain called the hypothalamus. ADH is then stored and released from the pituitary gland, a small gland at the base of the brain. DI caused by a lack of ADH is called central diabetes insipidus. When DI is caused by failure of the kidneys to respond to ADH, the condition is called nephrogenic diabetes insipidus. The major symptoms of diabetes insipidus are excessive urination and extreme thirst. The sensation of thirst stimulates patients to drink large amounts of water to compensate for water lost in the urine. Caroli syndrome is a congenital malformation characterized by multifocal dilatations of segmental bile ducts.1 These dilated portions form cysts of variable sizes separated by regularly dilated portions of the bile ducts. The distribution of these cysts may be diffuse, involving the whole intrahepatic biliary tree, or it may be localized in a part of the liver.1 The diffuse type is usually associated with congenital hepatic fibrosis,2 transmitted as an autosomal recessive trait and may also be associated with a renal malformation.3 The localized type is not associated with congenital hepatic fibrosis or renal malformations and is not inherited.2 It may be associated with choledochal cyst.4

Diabetes Insipidus Diabetes insipidus is a disorder that causes intense thirst and greatly increased output of very dilute urine. It occurs either because you are producing too little antidiuretic hormone (ADH), or vasopressin, or because your kidneys are not responding normally to the presence of ADH. You are most likely to develop diabetes insipidus in childhood or early adulthood. More males than females develop the condition. If you have diabetes insipidus, you may urinate as much as 16 to 24 liters of liquid per day. You may need to urinate as frequently as every half-hour to 60 minutes, all day and night. As a result, you will crave liquids, especially very cold water, and will drink huge quantities. If you are unable to keep up with your own thirst or if an accident or illness prevents you from drinking sufficient quantities, you may become dehydrated. Diabetes insipidus is not the same condition as diabetes mellitus , which relates to the level of insulin in the body and the body's ability to use insulin to turn sugar into energy.
